Can't really be a "credit note" as it is not separate, but is included in an already issued invoice.

It would appear that a correction was due because the person was underpaid for his or her services in the original, but it's a little strange that the party paying for services is issuing such a document (or perhaps they are sending it only as a reference?). Do you have more information about the circumstances? The only reason I have ever issued such a document was because I had calculated IVA at 16% on the original invoice after it had risen to 18% and I had to correct it.
