That's why the SIP card is called "SIP". It's the health card. I live in Valencia and I have one myself. I thought it was national, but apparently it's only Valencia.
Everyone who uses the public health services here has to have one, and you have to give the number on all kinds of forms. This is such a familiar experience to me that's it's the first thing I thought of here. But I agree with Ben; it's hard to understand why they would be asking for this number on a debit card application.

El SIP se crea en 1999 por el Decreto 126/1999, de 16 de agosto, del Gobierno Valenciano, con el objetivo de mejorar la relación de la ciudadanía con la administración, ser de utilidad en la planificación, gestión administrativa, ordenación de recursos sanitarios y servir de soporte para la gestión de la tarjeta sanitaria individual SIP.
Todas las personas están identificadas en el SIP por un código de identificación personal único, exclusivo y permanente: el número SIP
La información del SIP se actualiza fundamentalmente por los operarios que se conectan online a la aplicación desde los centros asistenciales, de primaria y especializada. También se actualiza mediante cruces informáticos con bases de datos procedentes de instituciones que aportan datos de interés relacionados con el aseguramiento (Bases de datos de Tesorería y Mutualidades Administrativas).
