I'm not sure what the "Acta nº 1" is referring to. Can anyone explain what this means here?

| Note from asker to answererI'm not really convinced that "certificate" was what I needed here, but it's what I ended up using. My concern was over confusion with "certificate of insurance," which does not refer to the policy itself, but is just (usually) a single sheet of paper acting as proof that there IS a policy. It usually just shows the types of coverage, insurers, limits and effective dates (a template for a Certificate of Insurance can be seen here: http://workstudy.berkeley.edu/Certificate%20of%20Insurance.htm). It was clear from my context that they were talking about the policy itself.
