"jefe civil (Ven): registrar, Vital Records Official"

Even though this is Nicaragua, it obviously deals with records.

"acta: certificate"

"acuerdo: 1. agreement. 2. decsion ruling [of a court]. 3. resolution [adopted by shareholders or board of directors]

Obviously number 2 is applicable in this context.
