This looks like an administrative note, used for filing purposes.
Forma is used in Mexican to mean 'formulario' - 'form' - see reference below
It probably refers to the fee for issuing the certificate, and the sum plus taxes paid by the recipient. 'En recibo' seems to mean 'by way of receipt' but in my view could be left out of the translation.
NO: probably just means 'number', i.e. the reference number of the certificate.
