1) hay alguna fórmula para "se deja constancia" así como we hereby..?
2) si no dicen licenciada en qué, qué se pone? Me fijé en preguntas anteriores pero en ningún caso apareció como en mi texto.

Si se sabe el nombre de la lic., puedes ponerla, y si no, algún otra referencia como (the lawyer, si es abogada), (the undersigned) si es que suscribe al documento, (the bearer) si ella misma es la interesada que pide este documento, etc.
