How can I express this without having to write 50 million words? I presume it's referring to the 'problems' one has within a risk factor group which influences their position on the scale of 'risk', if you know what I mean... !

Personally, I think retaining the word problematic in English is best even though it demands a slight sleight-of-hands. Problematic means involving problems and I don't know any other way to translate it here.
