Las drogas antiplaquetarias se indican para prevenir futuros eventos adversos cardiovasculares. En esta revisión, se resumen los beneficios y efectos adversos de cuatro drogas antiplaquetarias comunes: ácido acetilsalicíclico, (Aspirin®, etc.), ticlopidina (Ticlid®), clopidogrel (Plavix®) y dipiridamol (Aggrenox®). Existe una revisión sistemática que fue publicada en 19941 por la Antiplatelet Trialists Collaboration (ATC) (Colaboración de Ensayistas Antiplaquetarios) y que presenta la evidencia básica.
¿Cuáles son las implicancias clínicas de la revisión sistemática de la ATC?1
Prevención secundaria:
Son pacientes con enfermedad vascular oclusiva comprobada que tienen aumento de riesgo para un evento futuro.
El tratamiento farmacológico con drogas antiplaquetarias, en su mayoría ácido acetilsalicílico (ASS) brinda un beneficio claro (RRA en eventos vasculares= 3,3%, NNT= 30 durante 2,5 años) en prevención secundaria.
El tratamiento con drogas antiplaquetarias beneficia a los pacientes con sospecha y/o antecedentes de infarto agudo de miocardio, angina estable o inestable, ACV, accidente isquémico transitorio (AIT), cirugía de bypass, angioplastia o enfermedad vascular periférica. El ACV isquémico puede incluirse en la lista. El AAS 160 a 300 mg administrado dentro de las 48 hs de producido el ACV isquémico confirmado reduce significativamente el riesgo de muerte o dependencia (RRA= 1,3%, NNT = 77 durante 3 meses) y de ACV recurrente (RRA= 0,5 %, NNT = 200 durante 3 meses).2
El tratamiento más ampliamente probado es ASS en dosis medias de 75 a 325 mg por día. Ninguna otra droga o tratamiento previene mejor la mortalidad, el infarto de miocardio o el ACV. En el análisis de la ATC existe una tendencia hacia una mayor efectividad de dosis media de AAS comparadas con las dosis más altas. De la misma manera, un estudio reciente mostró que los pacientes con 81 o 325 mg tenían una mayor reducción de la incidencia combinada de ACV, infarto de miocardio y muerte dentro de los 3 meses de endarterectomía carotidea que los pacientes que usaban 650 o 1300 mg de AAS.3
