Spanish term or phrase: anamnesis por aparatos
From a clinical trial, in a section explaing the measures to be taken before including volunteers:

Se tomará la historia clínica que incluirá antecedentes personales y ***anamnesis por aparatos***

Anamnesis is a case history, but what about the "por aparatos" bit?

Many thanks

history [by system(s)]
Explanation:
Hi David,

It would be "history by system(s)", but "by system" is just not used in the US; not sure about the UK/Australia, etc. The "system" bit is sometimes used, however, to refer to the physical exam (see example below).

From your text, I'd skip it...

Good luck,

Elena

Documentation of the Physical Exam is typically **grouped by body system, such as Head, Eyes, Ears, Nose and Throat (often abbreviated "HEENT"), Respiratory, Genito-Urinary, etc.**

Explanation:
por aparatos, i.e. aparato respiratorio, circulatorio, digestivo, etc.

I think that "per system" would be a good choice for this one.
Cheers,

Karina ;)
