Hi - I have actually had to deliver the trans in question and put my own variation on Lydia and Rita's answer. What I hated about this phrase is that it seemed so vague! Just FYI here is the paragraph:"Que a las 01:20 horas, comunica los facultativos medicos, que el citado ha fallecido, solicitando que se pasa aviso a la Autoridad Judicial y medico forense. Adjuntandose parte de esencia. Por lo que se pasa aviso a dichas Autoridades."

Dani, could you provide the full paragraph? From the context provided it appears that the police is requesting that someone be notified of a death and that pertinent documents are attached to the request of notification. Perhaps more context will clarify.
