I have an admissions form for the emergency department of a national medical centre in Mexico. It contains doctor's notes. At the top of the page is a small box with all the basic details required such as patient's name, time and date of admission, doctor's name etc. Next to "No. Afiliación" (membership no.) is the word "Calidad", alongside which someone has stamped some kind of reference number. Any ideas? |
