Is it possible to deduce from the name whether the nurse is male or female? That might help to narrow down the possible meaning of "activo".

If the nurse is female, "activo" is presumably not an adjective applying to her.

My doubt about Emma's interpretation is that if it is a first-person verb, of which XXX is the direct object, you would have expected "Activo a XXX".

Another one of your questions shows that the doctor is writting the discharge report in first person...

Sometimes an "activo" refers to a person who is working at the time. For example: They sent four "activos" to the scene of the crime.

Might it just be the 1st person singular, i.e., I order/arrange for nurse XXX with ID no. YYY to organise the patient's discharge home?
