Barcelona. Coronary bypass surgery, Informe Operatorio. Term appears only once:
"Desclampaje aórtico. Purgado de raíz aórtica. Clamp parcial de aorta y pinza sacabocados."

A "sacabocados" or "saca-bocado" (the latter in Portuguese) is an instrument that gouges or "gnaws" away at bits of hardened tissue. They are most often used in neuro or head and neck procedures, but have many uses in thoracic surgery as well.
