I am unsure how to tackle "a descartar", and also "componente de edad". The former appears again in the translation as follows,

Ritmo sinusal a 89 1pm, eje QRS -7°, Latidos ventriculares prematuros, QS en II y a VF a descartar IAM inferior antiguo.

"Sinus rhythm 89 bpm, QRS axis -7°, Premature ventricular beats, QS in II and at VF when "disregarding" previous inferior AMI."

agree Robert Forstag: I would agree with Celia's caution. A doctor's note "to rule out X" likely means that s/he is directing that tests be done to determine whether X exists or not (if it does not, only then can it be "ruled out")
