There seems to be a lack of clarity in this sentence and I would appreciate some help here.

Todos este material y los diferentes mensajes, les hicieron replantearse el tratamiento: los niveles de azoles les preocupan y lo ven un riesgo, además es tan caro su control que a partir de entonces la pauta ha sido: ó solo Ambisome ó combian Ambisome+Voriconazol, pero nunca solo. En agosto las dos aspergilosis que tuvieron....

Asker: My problem is that the sentence [to me] = From now on the guideline is to either use AmBisone on its own, or a combination of AmBisome and Voriconazole, but never on its own. Isn't there a contradiction here? As they have said use AmBisome on its own earlier??
