My attempt: The above [surgery] was performed due to suffering from RADICULAR SYNDROME C6 LEFT caused by a (GREAT HERNIATED DISC/RUPTURED DIS???) C5-C6 LEFT. (I said "performed, past tense" because it gives the date the surgery was done.)
Also, is my word order in English ok? Or should it be "left C6"? Any doctors out there?
