English translation:warning signs/alarm symptoms
Explanation:
If the explanation provided by Kenneth Shockley is correct, and I think it is, this seems to be the equivalent of what in English is called the "wait-and-watch" approach. According to the Free Dictionary, wait-and-watch, or watchful waiting, means: "A stance in which a condition is closely monitored, but treatment withheld until Sx appear or change; WW is appropriate when there is a short–eg, ≤ 10 yrs–life expectancy, and/or the lesion being watched has minimal aggressiveness" This seems to agree with the definition of "pausas de alarma" on the scribd link provided by Kenneth. The patient is discharged but put on a wait-and-watch list; doctors inform the patient of the warning signs (alarm symptoms) before release from the hospital. In case these warning signs occur, the patient must immediately seek medical assistance. See links.

(the patient receives) alarm signs

Explanation:
As it says here at the first link: Pautas de alarma significa que decidimos esperar al paciente para ver como evoluciona, pero que no lo dejamos en bolas.. le damos pautas de alarma para que reconsulte.. es decir… si tiene vómitos, si tiene
...fiebre, si le duele la cabeza… si le pica el … vuelve a la consulta… Esa simple frase de cinco palabras ( reconsulta según pautas de alarma..."
From Linguee: ( medihelpservices.com):At discharge, the patient will receive written instructions on home medication, recommendations and alarm signs.

agree  Kenneth Shockley: I understand the vulgar expression "en bolas" on the scribd link to mean, in this case, "unprepared"; in order to be prepared, the patient needs to be informed, as you say, of the warning signs or symptoms that would be cause for alarm.
