In the following sentence, in connection with a crane accident on a ship:

Al no sufrir danos de magnitud la nave, como tampoco las personas que se encontraban en las cercanías del lugar del accidente, no se instruyó una Investigación Sumaria Adminstrativa, considerando lo estipulada en al Art. ***** (etc.)

Does this mean that an investigation was ordered, or not ordered (no se .....)?

Or "no investigation [...] was conducted / undertaken".

"Fail to" implies an improper omission: that it should have been done but was not done. This does not seem to be the case here. Things were done "by the book", as Lisa says: the circumstances did not warrant an investigation, "considerando lo estipulada en al Art. *****".

"Instruir" means to carry out a legal or administrative procedure: "tramitar un procedimiento administrativo o judicial" (DRAE).
