Es un paper de psicología, sobre temperamento culposo. No me queda claro si el término a utilizar es "guilty" o si en psicología existe un término más específico.

Yo usaría "guilty" con toda tranquilidad. Mi impresión es que el paciente tiene una culpa reprimida, que no es capaz de verbalizar.
