The whole phrase has me stumped! I understand what it means but cannot find a suitable equivalent in English.

Explanation:
A sense of excessive responsibility is linked with OCD and perfectionism. Hypervigilent people cannot turn off their mental energy. It makes sense that they would be related, but I can't vouch for this particular English phrase being routinely used.

Explanation:
This pertains to someone with PTSD - Post Traumatic Stress Disorder - what it really says is: the hypervigilance: for instance, you should not touch a person with PTSD who is asleep, they may hit you or worse; they sit with their back towards the wall in public places, like a restaurant, etc.
The excessive responsibility is: saying it that way is a euphemism, it means extremely controlling; nothing can or will happen without that person approving it, whatever it is, often in a very unreasonable manner.
