Spanish term or phrase: aplicar un cuestionario
What I'm looking for here is the best English collocation in the frame "__________ a/the... questionnaire" in the sense of "aplicar". Please don't answer "apply a questionnaire" (unless of course you're quite sure that's the best answer)! As a native English speaker a feel like I should know the answer (unhesitatingly even!), but hours in front of a screen and years of life as a polyglot make my mind go fluffy at times like this, and I am in one of those phases, I think. The best I've come up with is "administer a questionnaire". Would someone like to confirm that this is the right answer (if it is) or else point me back in the direction of real English (if it ain't).

The immediate context:
"El cuestionario fue aplicado en el mes de mayo 2006 por un grupo de personas entrenadas a tal efecto."

Also (I know this is really a forum question, but what the hell), if you know of a really good on-line reference source for quickly resolving typical collocation issues like this one, I'd love to be told about it!

Thank you everybody. In the last resort I agree with nedra, even though this amounts to agreeing with myself :-) Sorry about that! I wanted to see if there was a better translation. You all gave me lots of valid alternatives, but "administer" still seems to go best in the context, so nedra gets the KudoZ this time.

Si lo que quieres es rellenar tu cuestionario, es así.
Quizás quieras "solicitar" un cuestionario, en ese caso equivale a "apply"
Si lo que quieres es entregarlo, yo diría present/submit/hand out...,De lo que estoy segura es de que en español "no se aplica" un cuestionario, en todo caso se solicita.
En fin, a todos nos pasan estas cosas. Ojalá acierte.

Explanation:
introduced/ incorporated(into the study) in May 2006 by a group of individuals trained in that field.

"applied" could also be used as well.

I also like implemented but from what I can see there was a study being conducted and they eventually applied the questionnaire to the mix.

I think implement is quite adequate, but other options could be "put into effect" or simply "used"
