First time they smoked se entiende clarísimo. No obstante, tu propuesta es la más literal. "Drag", efectivamente es calada. "Have a drag" es dar una calada. Debajo te pongo un ejemplo sacado de una página gay sobre el cáncer de pulmón, en la que un chico habla cómo adquirió el hábito.
