sign of devotion
Explanation:
Another option.

"...some pilgrims go the last mile (of pilgrimage) on their knees as a sign of devotion."

I think this may convey the meaning of "Ofrecimiento hecho a Dios o a sus santos de ejecutar una obra piadosa." (RAE)

Cheers,
Gloria

neutral  Parrot: nope, it may simply be a vow to walk in the procession. The renewal of baptismal vows on Easter goes for everybody at a vigil.

over 25,000 persons exhibiting/displaying some sign of a promise kept

Explanation:
(1) about the promise:

I once met a pilgrim in Tenerife (they are recognizable because they carry long staffs) who thought I was also a pilgrim, and he asked me what promise I was fulfilling with my pilgrimage. It seems this is common practice here. One goes on pilgrimage as a sort of exchange with the Virgin: you honour her with the pilgrimage and in return she helps you with whatever you ask her for help with, for instance passing an exam.

This practice of fulfilling a promise seems to be the survival of a tradition that goes back to much earlier times. Here is an example where the promised pilgrimage was made following the recovery of a sick child's health:

(2) about the "señal":

as a sign of the promise kept, in some places the pilgrims may carry lighted candles (in night-time processions), walk barefoot or on their knees, or wear a special costume, which you can see on this page, along with this explanation:

I'm not sure exactly what "sign" the 25,000 persons are exhibiting in your case so I've left the suggested translation non-specific.

Reference information:
