I am translating a document from Hindi into English and it contains some urdu words or phrases which I don't fully understand.

What does it mean?

and

Explanation:
I strongly suggest that it is Statement on Oath.

Bayaan Halfi minjanib (Suresh), this would mean Statement on Oath of/from (Suresh). There are two types of affidavits, one attested by Notary Public and another by Oath Commissioner. Notary Public attestation is for outside proceedings whereas Oath Commissioner attestation is for court proceedings. A court always ask the plaintiff or defendant to file his statement on Oath attested by an Oath Commisssioner.

In your case, it is Statement on Oath for sure. Let me take some benefit being LL.B.
