The educational challenge
Doctors are judged both as healers and as professionals, and when they do not fulfil their obligations in either role both they and the profession suffer. When the medical profession was smaller, more homogeneous, and had more truly shared values—and when the issues were simpler—professional values could be imparted during the process of "socialisation" of doctors in training. The profession is now diverse, as in almost every country doctors come from various cultural, ethnic, and economic backgrounds. Though this represents an advance in terms of equity and fairness, it makes the transmission of common values more difficult and, in our opinion, requires explicit teaching of the role of both the healer and of the professional. As Irvine noted, the leader in medicine retains a critical role,1 but this leader must teach professionalism in a structured way in addition to demonstrating professional values in everyday life.
Medical schools, teaching hospitals, and those responsible for continuing medical education should teach professionalism as a subject formally identified in the curriculum. The material to be taught will change in different cultures and certainly with time. The teaching of professionalism should include several components.
(1) Identifiable educational content in the undergraduate medical school curriculum devoted to professionalism, which should be reinforced in postgraduate programmes and in continuing medical education. The subject should be part of the evaluation of all students.
(2) The concept that to be a professional is not a right but a privilege with a long history and tradition of healing and service.
(3) The separate but linked concepts of the physician as healer, and the physician as professional, and the fact that society uses professional status as a means of organising the delivery of services.
(4) A clear definition of professionalism and its characteristics.
(5) Professionalism as an ideal to be pursued, emphasising its inherent moral value. The concept of altruism and "calling" must be highlighted as essential to professionalism.
(6) An understanding that proper professional behaviour is essential for the healer to function fully and to maintain the trust of patients and society.
(7) Knowledge of codes of ethics governing the conduct of both the healer and the professional, as well as the philosophical and historical derivations of these codes.
(8) The essential nature of the autonomy of the individual doctor, along with the legitimate limitations that have always existed. The degree of autonomy will vary in different societies, but a minimum is required for a doctor to exercise the necessary independent judgment to best serve the patient.
(9) The nature of the collective autonomy of the profession, along with its legitimate and inherent limitations.
(10) Relevant material drawn from sociology, philosophy, economics, political science, and medical ethics as related to professionalism, including interpretations of both the historical course of events and of doctors' behaviour that are critical of the medical profession. | Translation - Spanish
El desafío educativo
A los médicos se los juzga en su condición de sanadores y de profesionales, y cuando no cumplen con sus obligaciones en sus respectivos roles, tanto ellos como su profesión resultan afectados. Cuando la profesión médica era más reducida, más homogénea y compartía valores en forma más verdadera, y cuando los temas a tratar eran más simples, los valores profesionales se impartían durante el proceso de "socialización" de los médicos en formación. La profesión es ahora diversa y en casi todos los países los médicos provienen de diversos entornos culturales, étnicos y económicos. Aunque esto constituye un avance en términos de equidad y justicia, también dificulta la transmisión de valores comunes y, en mi opinión, hace imperativo que se enseñe tanto el rol del sanador como el del profesional. Tal como señalara Irvine, el líder en medicina conserva un rol de vital importancia, pero a su vez debe enseñar el profesionalismo de manera estructurada además de evidenciar sus valores profesionales todos los días.
Las facultades de medicina, los hospitales universitarios y aquellas personas responsables de la educación médica permanente deben enseñar el profesionalismo como materia reconocida formalmente dentro de los programas educativos. El material a enseñar deberá variar según cada cultura y los tiempos que corren. La enseñanza del profesionalismo debe incluir varios elementos:
(1) Un contenido educativo reconocible dentro de los programas de las facultades de medicina; contenido que deberá estar orientado al profesionalismo y ser consolidado en los programas de especialización y de educación médica permanente. Dicha materia debe ser parte de la evaluación de todos los estudiantes.
(2) El concepto de que ser un profesional no es un derecho sino un privilegio con una larga tradición de curación y servicio.
(3) Los conceptos diferentes pero unidos a la vez del médico como sanador y del médico como profesional, y el hecho de que la sociedad utilice al estatus de profesión como un medio para organizar la prestación de servicios.
(4) Una definición clara de profesionalismo y sus características.
(5) El profesionalismo como ideal a alcanzar, con un énfasis en su valor moral inherente. Los conceptos de altruismo y de "vocación" deben considerarse inherentes al profesionalismo.
(6) El entendimiento de que el comportamiento profesional adecuado es esencial para que el sanador pueda desarrollar su tarea plenamente y para conservar la confianza de los pacientes y de la sociedad.
(7) El conocimiento de códigos de ética que regulan la conducta del sanador y del profesional, así como también las derivaciones filosóficas e históricas de dicho códigos.
(8) La naturaleza esencial de la autonomía de cada médico, junto con las legítimas limitaciones que han existido siempre. El grado de autonomía variará dependiendo de cada sociedad, pero es necesario establecer un mínimo para que los médicos puedan aplicar su buen juicio en pos de la mejor atención del paciente.
(9) La naturaleza de la autonomía colectiva de la profesión, junto con sus limitaciones legítimas e inherentes.
(10) Material pertinente extraído de la sociología, filosofía, economía, ciencia política y ética médica que se relacione con el profesionalismo, incluyendo las interpretaciones tanto del transcurso de los hechos como del comportamiento de los médicos que sean importantes para la profesión médica. |
