Summary and Explanation of the Test
Thyroxine (3,5,3',5'-tetraiodothyronine, L-thyroxine or T4) is a hormone synthesized and secreted by the thyroid gland and plays an important role in regulating metabolism. Secretion into the circulation is in response to the pituitary hormone TSH (thyroid stimulating hormone) and is regulated by a negative feedback mechanism involving the thyroid gland, pituitary gland, and hypothalamus.1,2
In the circulation, 99.95% of T4 is reversibly bound to transport proteins, primarily thyroxine-binding globulin (TBG) and to a lesser extent albumin and thyroxine-binding prealbumin (TBPA). The remaining T4 is not bound to transport proteins, but is free in the circulation. This unbound fraction, or free T4 (FT4), is both metabolically active and a precursor to triiodothyronine (T3).1,3-5
Free T4 levels correlate with T4 secretion and metabolism. In hypothyroidism and hyperthyroidism, FT4 levels parallel changes in total T4 levels.4 Measuring free T4 is useful when altered levels of total T4 occur due to changes in T4 binding proteins, especially TBG. TBG levels remain relatively constant in healthy individuals, but certain conditions, such as normal pregnancy and steroid therapy, can alter these levels. In these conditions, free T4 levels are unchanged, while total T4 levels parallel the changes in TBG.
Limitations
The anticonvulsant drug phenytoin may interfere with total and free T4 levels due to competition for TBG binding sites.
FT4 values may be decreased in patients with non-thyroidal conditions and in patients taking carbamazepine.
Performance of this assay has not been established with neonatal specimens.
Heterophilic antibodies in human serum can react with reagent immunoglobulins, interfering with in vitro immunoassays. Patients routinely exposed to animals or to animal serum products can be prone to this interference and anomalous values may be observed. Additional information may be required for diagnosis.
Thyroid autoantibodies in human serum may interfere and cause falsely elevated FT4 results.

Microbiological effectiveness
D 10 has the following characteristics of performance:

Application
D 10 is used as a 2% solution for 60 minutes for disinfection. To make 1 litre of ready-touse
solution, pour 20 ml of D 10 out of the container using the measuring beaker/dosing head and fill up
to 1 litre with water.
Fill 10 to max. 20 ml of D 10 concentrate into the receiving tank of the suction system before use. Suck a
small quantity of D 10 ready-to-use solution through the system between two applications. Empty the
receiving tank after use (e.g. in the evening) and put all contaminated parts (receiving tank, tubings,
lids, etc.) into the D 10 ready-to-use solution. Observe the contact time. Afterwards, rinse with water.
To disinfect the surfaces of the suction system, we recommend wipe disinfection with a D 10 ready-touse
solution or disinfection wipes, e.g. B 60, if the surfaces are resistant to alcohols. Observe the directions
for use from the device’s manufacturer.
Use a 2% solution of D 10 for disinfecting inhalators according to the directions for use from the device’s
manufacturer.
Do not mix D 10 with other disinfectants or cleansing agents.
Chemical composition
D 10 is based on a combination of quaternary ammonium compounds, alkaline cleansing agents, complexing
agents, special antifoaming agents and adjuvants in aqueous solution. 100 g D 10 contains:
4.4 g dimethyl-dioctyl-ammonium chloride and 0.6 g benzyl-dimethyl-dodecyl-ammonium chloride.

Aplicare
D10 este utilizat ca soluţie 2% timp de 60 minutes pentru dezinfecţie. Pentru a obţine 1 litru de soluţie gata de utilizat , turnaţi 20 ml de D10 din container utilizând o cana gradată/ cu cap dozator si umpleţi până la 1 litru cu apă.
Umpleţi 10 până la maximum 20 ml de concentrat D10 in tancul de stocare al instalaţiei de aspiraţie înainte de utilizare. Aspiraţi o cantitate mică de soluţie de D10 gata de utilizat , prin instalaţie între două aplicări. Goliţi tancul de stocare după utilizare (de ex seara) si puneţi toate piesele contaminate (tanc de stocare, tubulaturi, capace) în soluţie D10 gata preparată. Măsuraţi timpul de contact. După aceea , clătiţi cu apă.
Pentru a dezinfecta suprafeţele instalaţiei de aspiraţie , vă recomandăm dezinfectarea cu servet / lavetă cu soluţie gata preparată D 10 sau servete / lavete dezinfectante, ex B 60 ,dacă suprafeţele sunt rezistente la alcool. Observaţi indicaţiile de utilizare din instrucţiunile producătorului.
Utilizaţi o soluţie de D10 2% pentru dezinfectarea inhalatorilor conform indicaţiilor de utilizare ale producătorului aparatului.
Nu amestecaţi D 10 cu alţi dezinfectanţi sau agenţi de curăţare.
Compoziţie chimică
D10 se bazează pe o combinatie de compuşi cuaternari ai amoniului, agenţi de curăţare alcalini, agenţi de complexare, agenţi speciali antispumă şi adjuvanti în soluţie apoasă. 100 g D10 conţin :

Surface preparation standards
The condition of surface of the steel in building stage, the surface treatment necessary and the primer to be used will be critical to the success of the subsequent paint system. If the standard of the surface cleaning required for application of primer is not achieved then there will be a risk of failure or reduced life of the paint system.
There are various grade for surface preparation:
• By scraping following by wire brushing ”St” grades: St1, St2 , St3.
• By blast cleaning ”Sa” grades : Sa1; Sa 2; Sa2 ½, Sa 3.

2.3 Propulsion and Manoeuvrability
ERRVs should be capable in calm conditions of a speed of at least 10 knots and should as a minimum, be equipped with one of the following propulsions conditions:
a) A 360 degree azimuth bow thruster unit and either single screw propulsion with reversing gear box or variable pitch control propeller. The bow thruster unit should be capable of producing an ahead speed of 4 knots and should independent of the main engine for its source of power;
b) Twin screw propulsion and bow thruster such that ERRV is capable in calm conditions of 4 knots in the ahead direction with one main propulsion unit out of action ;
c) Equivalent main propulsion and manoeuvring systems providing similar levels of redundancy and manoeuvring capabilities.
The EERV operator should ensure and the surveyor be satisfied by demonstration, that EERV may be readily manoeuvred by one person.
To assist in the provision of such manoeuvrability, all EERVs should be fitted with a full bridge control system of the main engines and thrusters.
It should be possible to turn the ERRV without headway or stern way and to hold it in a desired position and on set headling while carrying out rescue operations, with sufficient accuracy to avoid hazard to the EERV, other units or persons in the water , in all but exceptional weather.

2.3 Propulsie şi manevrabilitate
Navele de salvare şi de interventie rapida (ERRV) trebuie să fie capabile în conditţii de mare calmă (să aibe viteza) de cel puţin 10 noduri şi trebuie, ca cerinţă minimală, să fie echipate cu una din următoarele configuraţii :
a) Un echipament propulsor prova azimutal de 360 de grade şi fie o propulsie cu o singură elice cu cutie de viteze reversibilă fie elice cu pas variabil. Echipamentul propulsor prova trebuie să fie capabil să producă o viteză de înaintare de 4 noduri şi trebuie să fie independent de motorul principal pentru sursa sa de putere/ alimentare;
b) Propulsie cu două elice şi propulsor prova astfel încât navele ERRV să fie capabile în condiţii de mare calmă (să aibe viteza) de cel puţin 4 noduri în directia de înaintare cu un echipament de propulsie principal scos din funcţiune;
c) Instalaţii echivalente de propulsie principală şi manevră ce asigură nivele similare de redundanţă .
Operatorul navei ERRV trebuie să asigure şi inspectorul să fie satisfăcut / mulţumit de demonstraţie, că nava ERRV să poată fi manevrată cu uşurinţă de o singură persoană . Pentru a asigura această manevrabilitate, toate navele ERRV trebuie prevăzute cu sistem complet de control pe punte a motoarelor principale şi a propulsoarelor.
Trebuie să existe posibilitatea ca navele ERRV să poata gira/ roti înainte şi înapoi şi să se menţină într-o poziţie dorită şi un curs stabil în timp ce efectuează operatii de salvare, cu o precizie suficientă pentru a evita punerea în pericol a navei ERRV, sau a altor echipamente sau persoane din apă, totul în condiţii meteo excepţionale.
