Alpha-1-antitrypsin (AAT) deficiency is a genetic condition
that increases the risk of developing lung and liver disease,
as well as other associated conditions. Most treatment of
affected individuals is not specifically directed at AAT
deficiency but focuses on the resultant disease state. The
only currently available specific therapeutic agent—
namely, intravenous augmentation with plasma derived
AAT protein—is marketed in a limited number of countries.
Treatments aimed at correcting the underlying genetic
abnormality, supplementing or modifying the gene
product, and halting or reversing organ injury are now
beginning to emerge. These innovative approaches may
prove effective at modifying or eliminating diseases
association with AAT deficiency.

Alpha-1 antitrypsin deficiency, also known
as a1-proteinase inhibitor deficiency or,
more simply, Alpha-1, is a genetic condition
that increases the risk of developing a
variety of diseases including pulmonary emphysema
and cirrhosis of the liver. It is caused by
mutations in the gene coding for the 52 kDa
glycoprotein a1-antitrypsin (AAT),1 2 the body’s
major serine proteinase inhibitor or serpin.3 This
gene is located in the long arm of chromosome
14 of the human genome.4 When first described
in 1963,5 AAT deficiency was looked upon as a
vanishingly rare condition that left affected
individuals with precocious severe emphysema.
Now it is understood to be a genetic condition
with a relatively high prevalence that can have
various clinical presentations, ranging from no
health effects through more typical chronic lung
or liver disease in the elderly to the more classic
neonatal cirrhosis or precocious emphysema of
young adults.6–8
Over 100 allelic variants of this gene have been
identified and 34 of them have been associated
with a quantitative or functional deficiency of
circulating AAT.9 In its classic form, an inherited
mutation of the AAT gene causes the build up of
abnormal AAT within the hepatocytes of the
liver. The liver is the major source of circulating
AAT and this transport problem leads to low
levels of AAT in the blood and tissues.
The proteinase inhibitor or Pi system has been
used to name the various mutations of the AAT
gene.10 The normal genotype is Pi M and the
classic severe deficiency is associated with the

La Deficiencia de Alfa1-antitripsina, también conocida como deficiencia del inhibidor de proteinasa α1 o simplemente Alfa-1, es una enfermedad genética que aumenta el riesgo de desarrollar una variedad de patologías incluyendo enfisema pulmonar y cirrosis hepática. Está causada por mutaciones en el gen que codifica a la glicoproteína de 52 kDa α1-antitripsina (AAT)1, 2, la principal serpina3 del organismo. Este gen está localizado en el brazo largo del cromosoma 14 del genoma humano4. Cuando la Deficiencia de AAT fue descrita por primera vez en 19635, se la veía como una rara enfermedad que afectaba a individuos jóvenes con enfisema severo. Actualmente se la considera una condición genética con una prevalencia relativamente alta, que puede tener varias presentaciones clínicas que van desde ausencia de efectos sobre la salud, pasando por la típica enfermedad crónica del pulmón o hígado en los individuos mayores, hasta la más clásica cirrosis neonatal o enfisema precoz en los adultos jóvenes6-8.
Se han identificado más de 100 variantes alélicas de este gen de las cuales 34 han sido asociadas con una deficiencia cuantitativa o funcional de la AAT circulante9. En su forma clásica, una mutación heredada en el gen AAT causa la fabricación de una AAT anormal en los hepatocitos del hígado, que se trasporta a la circulación con menor velocidad. El
hígado es la fuente principal de la AAT circulante y este problema de transporte conduce a bajos niveles de AAT en la sangre y los tejidos.

The molecular basis of a1-antitrypsin deficiency is reviewed
and is shown to be due to the accumulation of mutant
protein as ordered polymers within the endoplasmic
reticulum of hepatocytes. The current goals are to
determine the cellular response to polymeric a1-antitrypsin

Alpha1-antitrypsin (AAT) deficiency was
reported in an Alaskan girl who died 800
years ago1 and may have accounted for the
premature death of Frederic Chopin in 1849.2 3 It
was first described as a clinical entity in 1963 by
Laurell and Eriksson who noted an absence of
the a1 band on serum protein electrophoresis.4
The major function of AAT is to protect the
tissues against the enzyme neutrophil elastase.5 6
Its role in protecting the lungs against proteolytic
attack is underscored by the association of
plasma deficiency with early onset panacinar
emphysema.7 This finding, together with the
observation that the intrapulmonary instillation
of elastolytic enzymes also results in emphysema,
8–11 gave rise to the proteinase-antiproteinase
hypothesis of lung disease. In health there
is a balance between proteinases and antiproteinases,
but when proteinases are in excess, tissue
destruction will ensue. The proteinase-antiproteinase
hypothesis was developed over 35 years
ago and still remains central to our understanding
of the pathogenesis of lung disease.
We review here the molecular mechanisms that
underlie AAT deficiency and show how an
understanding of this mechanism has allowed
us to explain the deficiency of other members of
the serine proteinase inhibitor or serpin superfamily.
These include the deficiency of antithrombin,
C1 inhibitor, a1-antichymotrypsin,
and neuroserpin in association with thrombosis,
angio-oedema, airflow obstruction, and dementia,
respectively. We have grouped these conditions
together as the ‘‘serpinopathies’’.12–14 Their
common pathophysiology provides a platform for
the development of strategies to treat the
associated clinical syndromes.
STRUCTURE AND FUNCTION OF a1-ANTITRYPSIN (AAT)
AAT is a 394 amino acid, 52 kDa, acute phase
glycoprotein encoded on chromosome 14q31–
32.1.15–17 It is synthesised by hepatocytes18 19 and
secreted into the plasma at a concentration of
1.9–3.5 mg/ml. It is also synthesised by and
secreted from macrophages20 and intestinal21 and
bronchial epithelial cells.22 The protein was
originally named because of its ability to inhibit
pancreatic trypsin.23 Subsequently it has been
found to be an effective inhibitor of a variety of
other proteinases including neutrophil elastase,5
cathepsin G,5 and proteinase 3.24 The broad
spectrum of proteinase inhibition gave rise to
its alternative name of a1-proteinase inhibitor,25
although this too is inaccurate as other proteins
in the a1 band of serum (such as a1-antichymotrypsin)
are also proteinase inhibitors.
Crystal structures have shown that AAT is
composed of three b-sheets (A–C) and an
exposed mobile reactive loop (fig 1) that presents
a peptide sequence as a pseudosubstrate for the
target proteinase.26–30 The critical amino acids
within this loop are the P1–P19 residues, methionine
serine, as these act as a ‘‘bait’’ for
neutrophil elastase.31 After docking, the enzyme
cleaves the P1–P19 peptide bond of AAT32 and the
proteinase is inactivated by a mousetrap action
(fig 1) that swings it from the upper to the lower
pole of the protein in association with the
insertion of the reactive loop as an extra strand
in b-sheet A.33–37 This altered conformation of
AAT bound to its target enzyme is then recognised
by hepatic receptors and cleared from the
circulation.38
The remarkable mouse trap action of AAT is
central to its role as an effective inhibitor of
serine proteinases. Paradoxically, it is also its
Achilles heel as point mutations in these mobile
domains make the molecule vulnerable to
aberrant conformational transitions such as the
one that underlies AAT deficiency.
a1-ANTITRYPSIN (AAT) DEFICIENCY
AAT deficiency is the most widely recognised
abnormality of a proteinase inhibitor that causes
lung disease. Over 70 naturally occurring variants
have been described and characterised by
their migration on isoelectric focusing gels—the
proteinase inhibitor or Pi system.39 The commonest
deficiency variants, S and Z, result from point
mutations in the AAT gene40–42 and are so named
as they make the protein migrate more slowly
than normal M AAT. Mutations that cause more
rapid migration of AAT are labelled A to L.

La deficiencia de alfa1-antitripsina (AAT) se reportó en una niña oriunda de Alaska que falleció hace 800 años1 y también puede haber dado cuenta de la muerte prematura de Federico Chopin en 18492, 3. Fue descrita por primera vez como una entidad clínica en 1963 por Laurell y Ericksson, quienes notaron la ausencia de la banda αı (alfa1) en la electroforesis de
proteínas séricas4. La principal función de la AAT es proteger a los tejidos de la elastasa, enzima producida por los neutrófilos5,6. Su rol de proteger los pulmones contra un ataque proteolítico está avalado por la asociación del déficit plasmático y la aparición temprana de enfisema panacinar7. Este hallazgo, junto con la observación que la instilación intrapulmonar de
enzimas elastolíticas también resulta en enfisema8-11, llevó a formular la hipótesis de proteasa-antiproteasa para la enfermedad pulmonar. Normalmente hay un balance entre las proteasas y antiproteasas, pero cuando hay un exceso de proteasas se produce la destrucción del tejido. Esta hipótesis se desarrolló hace más de 35 años y sigue siendo central para nuestra comprensión de la patogénesis de la enfermedad pulmonar. En este artículo hacemos una revisión de los mecanismos moleculares
que están detrás de la deficiencia de AAT y mostramos cómo la comprensión de este mecanismo nos ha permitido explicar la deficiencia de otros miembros de la superfamilia de los inhibidores de las serin proteasas o serpinas (por sus siglas en inglés, serine protease inhibitors). Estos incluyen la deficiencia de antitrombina, inhibidor C1, alfa1-antiquimotripsina y neuroserpina en asociación con trombosis, angioedema, obstrucción del flujo aéreo y demencia, respectivamente. Hemos agrupado estas condiciones bajo el nombre de “serinopatías”12-14. Su fisiopatología en común provee una plataforma para el desarrollo de estrategias para tratar los síndromes clínicos asociados.
ESTRUCTURA Y FUNCION DE LA ALFA1-ANTITRIPSINA (AAT)
La AAT es una glicoproteína de fase aguda, de 52 kDa y 394 aminoácidos que está codificada en el cromosoma 14q31-32.115-17. La sintetizan los hepatocitos18-19 y es secretada al plasma a una concentración de 1.9-3.5 mg/ml. También la sintetizan y secretan macrófagos20, células intestinales21 y células epiteliales bronquiales22. Inicialmente se la denominó así por su habilidad de inhibir a la tripsina pancreática23. Posteriormente se encontró que era un efectivo inhibidor de una
variedad de otras proteasas incluyendo la elastasa de los neutrófilos5, catepsina G5 y proteasa 324. El amplio espectro de inhibición de proteasas dio pie a su nombre alternativo de inhibidor de la proteasa alfa1
25, a pesar que es muy poco exacto ya que hay otras proteínas en la banda α del suero que son también inhibidores de proteasa (como la
alfa1-antiquimotripsina).
Estudios cristalográficos han mostrado que la AAT está compuesta por tres láminas β (A-C) y un lazo reactivo expuesto móvil (Fig. 1) que tiene una secuencia peptídica que sirve como pseudo sustrato para la proteasa26-30. Los aminoácidos críticos dentro de este lazo son los residuos P1-P1’ metionina serina, ya que actúan como “anzuelo” de la elastasa de los
neutrófilos31. Una vez atrapada, la enzima rompe la unión peptídica P1-P1’ de la AAT32 y la proteasa se inactiva por una acción de tipo ratonera (Fig.1), que la hace girar desde el polo superior al inferior de la proteína en asociación con la inserción del lazo reactivo como una hebra extra en la lámina β A33-37. Esta conformación alterada de la AAT unida a su
sustrato enzimático es luego reconocida por receptores hepáticos y eliminada de la circulación38.
Esta asombrosa acción de ratonera de la AAT es central para su rol de inhibidor efectivo de serin proteasas. Paradójicamente, es también su talón de Aquiles ya que mutaciones puntuales en esos dominios móviles hacen a la molécula vulnerable a transiciones conformacionales aberrantes como la que subyace en la deficiencia de AAT.
DEFICIENCIA DE ALFA1-ANTITRIPSINA (AAT)
La deficiencia de AAT es la anormalidad más ampliamente identificada de un inhibidor de proteasa que causa enfermedad pulmonar. Se han descrito más de 70 variantes que ocurren naturalmente y han sido caracterizadas por su migración en geles de isoelectroenfoque--el sistema de inhibidor de proteasa o sistema Pi39. EVALUACION DEL ESTADO DE OXIGENACION EN PACIENTES CON ENFERMEDAD PULMONAR OBSTRUCTIVA CRONICA (EPOC)
RESUMEN
Se evaluó el estado de oxigenación en dos grupos de pacientes con enfermedad pulmonar obstructiva crónica: (G1): pacientes crónicos hemodinámicamente estables y (G2): pacientes con falla respiratoria aguda (F.R.A).
Se tomaron simultáneamente muestras de sangre arterial y venosa-mixta, midiéndose pH, pCO2, pO2, concentración de Hemoglobina y Oxihemoglobina %.
Se calculó el gradiente alvéolo-arterial, contenido, diferencia arterio-venosa, transporte, consumo y extracción de O2. Se midió el volumen minuto por termodilución. Se compararon los valores de ambos grupos expresando los resultados como Media ± DS. Utilizamos correlación lineal, y para la comparación entre grupos, el test “t” no apareado. Se aceptó como significativa p< 0.05.
Resultados: para G2: Qt = 6.5 ± 1.8 l/min, DO2 = 973 ± 440 ml/min, VO2 = 227 ± 66 ml/mim.
Se calculó la regresión VO2 vs. DO2 (r = 0.58. P <0.001). Difirieron significativamente entre grupos (p<0.001): paO2, FOxiaHb%, p50, a/A(%), cHb, CtaO2, CtvO2, Dif a-v. (p<0.05): FOxivHb% y No Significativo: pH, paCO2, pvO2, ERO2%.
Conclusiones: Los pacientes con F.R.A tienen disminuido en el CtaO2 por la anemia. Se demuestra que en el EPOC estable la Dif a-v es mayor, mientras que los pacientes críticos tienen comprometida la extracción tisular de O2. La regresión lineal VO2/DO2 indicaría una deuda de O2 por dependencia patológica.

EVALUATION OF THE OXYGENATION STATUS IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE (COPD)
SUMMARY
Oxygenation status was evaluated in two groups of patients with chronic obstructive pulmonary disease: (G1): hemodinamically stable chronic patients and (G2): patients with acute respiratory failure (ARF).
Samples of arterial and mixed venous blood were drawn at the same time and pH, pCO2, pO2, Hemoglobin concentration and percent Oxyhemoglobin were measured.
The following calculations were made: alveolar-arterial oxygen gradient, oxygen content, arteriovenous oxygen difference, transport, consumption and oxygen extraction. Cardiac output was measured by thermodilution. The values for both groups were compared and the results were expressed as Mean ± SD. Linear correlation and unpaired “t “ test for comparison between groups were used. A significance level of p<0.05 was used.
Results: for G2: Qt= 6.5 ± 1.8 l/min, DO2 = 973 ± 440 ml/min, VO2 = 227 ± 66 ml/min.
Regression was calculated VO2 vs. DO2 (r = 0.58. p< 0.001). Significative differences between the groups (p<0.001) were found: paO2, FOxiaHb% (arterial blood oxyhemoglobin fraction %), p50, a/A (%), cHb, CtaO2 (arterial oxygen content), CtvO2 (mixed venous oxygen content), av difference. Conclusions: Patients with ARF have a decreased CtaO2 because of anemia. It is proved that av difference is larger in stable COPD, while tissue oxygen extraction is compromised in critically ill patients. VO2 /DO2 linear regression would indicate an O2 debt due to a pathological dependency.

INMUNOFLUORESCENCIA CON CRITHIDIA LUCILIAE PARA DETECCIÓN DE ANTICUERPOS ANTI-DNA. IMÁGENES ATÍPICAS Y SU RELACIÓN CON ENFERMEDAD DE CHAGAS Y LEISHMANIASIS

RESUMEN
Los anticuerpos anti-DNA nativo pueden detectarse por inmunofluorescencia indirecta con Crithidia luciliae, la cual posee un kinetoplasto que contiene DNA de doble cadena, observándose tinción fluorescente del mismo. Pueden observarse imágenes fluorescentes atípicas en otras estructuras. Como la Crithidia luciliae pertenece a la familia Tripanosomatidae, donde existen patógenos para el hombre, como Tripanosoma cruzi, y Leishmania spp, se consideró que dichas imágenes pudieran ser debidas a reacciones cruzadas. Se realizaron estudios serológicos para Chagas a 105 muestras séricas con solicitud de anti-DNA provenientes del Hospital de Clínicas de Buenos Aires y del Hospital Escuela de Corrientes que presentaban imágenes atípicas. En Buenos Aires la serología para Chagas fue positiva en el 64.7% y en Corrientes en el 78.3% de las muestras con imagen atípica caracterizada por fluorescencia conjunta de membrana y flagelo. Como controles se estudiaron con Crithidia luciliae 57 muestras de pacientes con colagenopatías y 50 de dadores de sangre. Ninguna de ellas presentó la imagen conjunta salvo dos pacientes lúpicas que también eran chagásicas. Además se estudiaron 54 muestras de pacientes chagásicos y todas presentaron la imagen conjunta. Se estudiaron 46 muestras de seroteca de pacientes con leishmaniasis de los cuales 28 estaban coinfectados con Tripanosoma cruzi. La imagen fluorescente conjunta se observó en el 88.0% de las muestras de leishmaniasis y en el 89.3% de las de coinfectados. Los resultados sugieren que la Crithidia luciliae podría utilizarse como un sustrato alternativo, económico y de bajo riesgo, para el diagnóstico serológico de Chagas, aunque no discrimina la infección por Leishmania. En la determinación de anti-DNA, si se observaran imágenes atípicas, sería conveniente la realización de estudios para Chagas y leishmaniasis, especialmente en zona endémica.

SUMMARY
Anti-native DNA antibodies can be detected by indirect immunofluorescence with Crithidia Luciliae, which has a kinetoplast containing double stranded DNA. This is observed by fluorescent staining. Other structures can be stained as well, showing atypical fluorescent images. As Crithidia Luciliae belongs to the Tripanosomatidae family, where human pathogens like Trypanosome Cruzi and Leishmania spp are also present, it was considered that theses images could be caused by cross-reactions. Serological studies for Chagas were performed in 105 serum samples with a request of anti-DNA antibodies and presence of atypical images. The study was performed in two centers: Hospital de Clínicas from Buenos Aires and the Hospital Escuela from Corrientes. In Buenos Aires, the serology for Chagas was positive in 64% of the samples whereas in Corrientes 78,3% of the samples were positive and had the atypical image characterized by fluorescence present both in membrane and flagellum (joint image). As controls, 57 samples from patients with collagenopathies and 50 samples from blood donors were tested with Crithidia Luciliae.. Non of them presented the joint image except for two patients with Lupus that were also chagasic. In addition, 54 samples from chagasic patients were studied and all of them presented the joint image. 46 samples from a serum library from patients with Leishmaniasis were studied, 28 of them co-infected with Trypanosome Cruzi. The fluorescent joint image was observed in 88,0% of the samples with Leishmaniasis and in 89,3% of the co-infected samples. The results suggest that Crithidia Luciliae could be used as an alternative substrate, economical and of low risk, for the serological diagnosis of Chagas Disease even though it does not discriminate for Leishmania infection. In the detection of anti-DNA antibodies, if atypical images were observed, it would be convenient to perform tests for Chagas and Leishmaniasis, specially in endemic areas.
