Die Antirefluxchirurgie ist der einzige kurative
Therapieansatz bei der gastroösophagealen
Refluxerkrankung [6]. Während
sowohl die 360°-Nissen-Fundoplikatio
als auch die partielle Fundoplikatio
– anterior wie posterior – zu einer effektiven
Refluxkontrolle führen, ist die Nissen-
Fundoplikatio im postoperativen Verlauf
häufiger mit Nebeneffekten wie Dysphagie,
Gas-bloat-Syndrom und Schwierigkeiten
beim Erbrechen und Aufstoßen
assoziiert [1, 14, 20]. Im Tier-Modell
konnten die laparoskopische Nissen-
Fundoplikatio wie die partielle anteriore
Semifundoplikatio die Kompetenz des
unteren Ösophagussphinkters wieder herstellen
und den Ruhedruck des Sphinkters
erhöhen [22]. Bei ähnlich guten Langzeitergebnissen
hinsichtlich der Beseitigung
des gastroösophagealen Refluxes reduzierte
jedoch die Semifundoplikatio in
verschiedenen Studien die Wahrscheinlichkeit
der unerwünschten Symptome
[9, 13, 20, 21, 23].
Über die Langzeitergebnisse der unterschiedlichen
Schweregrade der Refluxösophagitis
verglichen mit der nichterosiven
Refluxerkrankung nach laparoskopischer
partieller Fundoplikatio existieren in der
vorliegenden Literatur kaum Angaben.
Ziel unserer Studie war die Untersuchung
der Langzeitergebnisse der laparoskopischen
anterioren Semifundo plikatio bei
der nichterosiven (NERD) und erosiven
Refluxerkrankung (ERD).
Untersuchungsmethode
Patienten
Eingeschlossen in die vorliegende Studie
wurden Patienten mit laparoskopischer
anteriorer Semifundoplikatio als Erstoperation
bei gastroösophagealer Refluxerkrankung
und einer minimalen Verlaufsbeobachtung
von 6 Monaten postoperativ.
Patienten mitBarrett-Ösophagus wurden
ausgeschlossen. Die laparoskopische
Technik wurde in unserer Klinik erstmals
1997 eingesetzt, die letzte Befragung der
Patienten erfolgte im Januar 2006. Die
Studie umfasst somit den Zeitraum zwischen
Mai 1997 und Juli 2005, in dem 246
Patienten mit gastroösophagealem Reflux
in der Klinik für Allgemein- und Abdominalchirurgie
der Johannes-Gutenberg-
Universität Mainz operiert wurden.
Methodik
Bei allen Patienten wurde präoperativ eine
endoskopische Untersuchung des oberen
Gastrointestinaltraktes durchgeführt. Der
Schweregrad der Refluxösophagitis wurde nach Savary und Miller (Grad I–IV)
klassifiziert. Bei Patienten mit atypischen
Beschwerden erfolgte eine Magen-Darm-
Passage mit dem Nachweis einer Hiatushernie
oder eines Refluxes. Weiterhin
war eine 24-Stunden-pH-Metrie bzw. eine
Ösophagusmanometrie bei Patienten
mit unschlüssigen Befunden indiziert.
Eine hintere Hiatusplastik mit nichtresorbierbarem
Nahtmaterial (2–3 Nähte)
wurde bei allen Patienten durchgeführt.
Unter Kalibrierung durch einen 13-Charriere-
Magenschlauch erfolgte die anteriore
180°-Semifundoplikatio ohne Durchtrennung
der Vasa gastricae breves.
Dabei wurde der Magenfundus spannungsfrei
auf die vordere Ösophaguswand
gelegt und auf deren rechte Zirkumferenz
mit 4–6 nichtresorbierbaren Nähten
fixiert, wobei eine apikale Naht das
Fundoplikat am Zwerchfell verankerte.
Der klinische Verlauf postoperativ
wurde anhand eines standardisierten Fragebogens
bei 190 Patienten, die die Einschlusskriterien
erfüllten, erfasst. Dabei
wurde neben postoperativen Refluxsymptomen,
der Einnahme von Protonenpumpeninhibitoren,
ösophagealen und abdominellen
Symptomen, einer neuen Unverträglichkeit
von Nahrungsmitteln,
Stuhlunregelmäßigkeiten und Gewichtsveränderungen
der modifizierte symptomatische
DeMeester-Score bestimmt
[2, 8]. Dieser Score wurde berechnet, indem
die Häufigkeit der Symptome Sodbrennen,
saures Aufstoßen und Dysphagie
(0=nicht vorhanden, 1=gelegentlich,
2=häufig, 3=täglich) summiert wurde.
Somit konnte ein Score von minimal
0 (= keine Symptome) bis 9 (= maximale
Symptome) erreicht werden. Weiterhin
wurden die Patienten anhand einer Analogskala
von 0–10 nach der Zufriedenheit
mit der chirurgischen Behandlung befragt
(0=unzufrieden, 10=voll zufrieden).

Nonerosive and erosive gastroesophageal reflux disease
Long-term results of the laparoscopic anterior semifundoplicationn
The anti-reflux surgery is the only curative therapy approach of gastroesophageal reflux disease. Whereas both the 360°-Nissen-fundoplication and the partial fundoplication –anterior and posterior- lead to an effective reflux monitoring, the postoperative Nissen-fundoplicationn is more often connected with side effects as dysphagia, gas-bloat-syndrome and difficulties in vomitting and belching [1, 14, 20]. In animal studies the laparoscopic Nissen-fundoplication and partial anterior semi-fundoplication could restore the competence of the lower esophageal sphincter and increase the sphincter resting pressure [22]. Semi-fundoplication showed in miscellaneous studies, however, comparably good long-term-results in deleting gastroesophageal reflux and produced less negative side effects [9, 13, 20, 21, 23].
Present literature hardly offers information about the long-term-results of the various degrees of severity of peptic esophagitis compared with nonerosive reflux disease after laparascopic fundoplicationn. The goal of our study was the examination of the long-term-results of the laparoscopic anterior semi-fundoplicationn of nonerosive (NERD) and erosive reflux disease (ERD).
Research design
Patients
Patients who were included in this study had had laparoscopic anterior semi-fundoplicationn as primary surgery for gastrooesophageal reflux disease and a minimum six months post-operative observation was conducted. Patients with Barrett’s oesophagus were excluded. In our clinic the laparoscopic technology was first performed in 1997, the last survey was made in January 2006. The study thus consists of the time period between May 1997 and July 2005 in which 246 patients with gastroesophageal reflux had operations in Klinik für Allgemein- und Abdominalchirurgie der Johannes-Gutenberg- Universität Mainz, Germany.
Methods
All patients had a preoperative endoscopic examination of the upper gastrointestinal tract. The degree of severity of peptic esophagitis was classified according to Savary and Miller. Patients with atypical complaints had upper gastrointestinal x-ray series which showed hiatus hernia or reflux to be responsible. Moreover, a 24-hour pH measurement and oesophageal manometre was performed on patients who had irresolute clinical findings.
Posterior hiatus plastics with non-absorbing suture was performed on all patients (2-3 stitches). Under calibration the anterior 180 ° semi-fundoplicationn was performed with a 13 Charriere gastric tube without severing the vasae gastricae breves. While performing the surgery the gastric fundus was tension-free attached to the posterior oesophagus wall and attached onto its right circumference with 4-6 non-absorbing stitches and the fundoplication was fixed onto the midriff by an apical stitch.
The postoperative clinical procedure was conceived by a standardised questionnaire filled in by the 190 eligible patients. Next to postoperative reflux syndromes, administration of proton inhibitors, oesophageal and abdominal syndromes, a new irreconcilability of food, irregularities of bowel movements and variations of body weight the questionnaire also included the modified symptomatic DeMeester-score [2,8]. The score was calculated by summing up the frequency of heartburn, sour belching and dysphagia (0=non, 1=occasionally, 2=frequently, 3=daily). Thus the resuld could reach a score of the minimum 0 (= no symptoms) until the maximum of 9 (= maximal symptoms). Furthermore the patients were asked according to an analogous scale from 0-10 whether they were satisfied with the surgical treatment (0= not satisfied, 100= fully satisfied)
