1. Giriş
Osteoartrit (OA), tüm dünyada artritin en yaygın şeklidir. Tanımlanmasındaki sorunlardan dolayı, osteoartritin insidansı tam bir kesinlikle belirlenememektedir. Osteoartritin semptomları spesifik değildir ve radyolojik değişiklikler, başlangıç zamanı tam olarak belirlenemeyen, dereceli olarak ilerleyen bir patolojik süreci yansıtmaktadır. Şiddetli osteoartritin insidansı için olası bir tahmin, osteoartritin radyolojik değerlendirmesinde kullanılan Kellgren skorundan, klinik semptomların ortaya çıkışından önce veya sonra elde edilebilmektedir3.
Osteoartritli hastalar, tipik olarak ağırlık altına girmekle artan ve istirahatle hafifleyen ağrının yanı sıra, sabah sertliği ve tutulan eklemin inaktivite periyotları sonrasında sertleşmesiyle karakterize semptomlar göstermektedir. Fiziksel incelemede, çoğunlukla palpasyonda hassasiyet, kemik büyümeleri, hareketle krepitasyon ve/veya eklem hareketinin kısıtlanmış olması tipiktir. Romatoid artritten (RA) ve diğer enflamatuvar artritlerde olduğundan farklı olarak, enflamasyon, eğer mevcutsa, genellikle hafif ve etkilenen eklemle sınırlıdır. 1.Introduction
Osteoarthritis (OA) is the most common type of arthritis in the world. Because of problems finding a precise definition for the disease, the incidence of Osteoarthritis can not be determined exactly. The symptoms of Osteoarthritis are not specific and radiological changes reflect a gradually increasing pathological process, of which the start time can not be determined definitively. A probable estimation of the incidence of severe Osteoarthritis can be obtained from the Kellgren score, used for the radiologic evaluation of Osteoarthritis, after or before the appearance of the clinical symptoms.
Osteoarthritis patients show characteristic symptoms of pain which increases with lifting weight and decreases with rest, morning stiffness and joint stiffness, which is exacerbated after inactive periods. On physical examination, tenderness to palpation, bone growth, crepitation with movement, and/or the limitation of joint movement are typical findings. Being different than rheumatoid arthritis (RA) and the other inflammatory arthritides, inflammation, if it exists, is generally light and is limited to the affected joint. Even though the causes of Osteoarthritis are not understood completely, biochemical changes which affect subchondral bone and articular cartilage and genetic factors seem to play important roles in the pathogenesis.
For the medical treatment of Osteoarthritis, pharmacological, non pharmacological, intra-articular and surgical treatments can be considered.
Non pharmacological treatment:
• Training (training of the patient, the family or the spouse)
• Social support (phone contacts)
• Physiotherapy (aerobic exercises muscle strengthening, patellar bandage)

John Cobb, 59, a former commercial fisherman who is disabled with cirrhosis of the liver and emphysema, lives in a studio apartment in Greensboro, N.C., on a fixed monthly income of $674. He has been hoping to receive more government assistance, and in February, he did.
It came in the form of a free cellphone and free service.
Mr. Cobb became one of a small but rapidly growing number of low-income Americans benefiting from a new wrinkle to a decades-old federal law that provided them with subsidized landline telephone service.
In a twist, wireless carriers are receiving subsidies to provide people like Mr. Cobb with a phone and typically 68 minutes of talk time each month. It is a form of wireless welfare that puts a societal stamp on the central role played by the mobile device.
Mr. Cobb’s cellphone is a Motorola 175. “I feel so much safer when I drive. If I get sick, I can call someone. If I break down, I can call someone,” Mr. Cobb said. “It’s a necessity.”
The users are not the only ones receiving government assistance. Telecommunications industry analysts said the program, while in its infancy, could benefit mobile phone carriers, who face a steep challenge of their own: most Americans already own a cellphone, so the poor represent a last
