Choice of Health Care Agent
No one can predict when a serious illness or accident might happen. When it does, you might need someone else to speak for you or make medical decisions for you. If you plan now, you can increase the chances that the medical treatment you receive will be the treatment you want.
In Maryland, Virginia, and D.C., you can choose someone to be your “Health Care Agent,” which is the same as a "Medical Power of Attorney" or "Health Care Proxy." The health care agent is the person you trust to make medical decisions for you if you do not want to or are unable to make them yourself. These decisions should be based on your personal values and wishes.
What kind of decisions can my health care agent make?
Your agent can:
• Talk with healthcare providers about your condition
• See medical records and approve who else can see them
• Give permission for medical tests, medications, surgery, or other treatments
• Make decisions about treatments designed to keep you alive if you are near death or not likely to recover
• Choose among options for where you receive care and who your health care providers are
• Agree to donate your organs if you have not decided for yourself
• Sign for you to give permission for any of the above
Whom should I choose to be my health care agent?
You can pick a family member as a health care agent, but you don’t have to. Remember, your agent will have the power to make treatment decisions, even if other people close to you might urge a different decision.
Choose a family member or friend who:
• Is at least 18 years old
• Knows you well
• Can be there for you when you need it
• You trust to do what is best for you and is willing to carry out your wishes
• Would be comfortable talking with your healthcare providers and asking them questions if necessary
• Would not be too emotionally upset to carry out your wishes if you became very sick
• Is willing to accept this important role
In case the person who is your first choice is not able to carry out this role when the time comes, you should choose a “backup” agent. You may want to choose a third person for extra security in case neither of your first two choices is available.
What will happen if I do not choose a health care agent?
If you are too sick to make your own medical decisions, and have never written down the name of your choice of a health care agent, your doctors will ask your closest family member to make decisions for you. There are four reasons why this may be a problem:
1) The closest family member available at the time might not be the person you would want to make decisions for you.
2) Some family members might not be able or willing to make decisions as you would want them to.
3) Family members may disagree with one another about the best decisions.
4) Family members cannot legally make all of the decisions that may be needed. Some decisions may have to be made by someone appointed by a judge or possibly someone who does not know you at all if you have not named a healthcare agent.
What do I do with this form after I fill it out and sign it?
1. Give a copy to your health care agent and to each of your backup agents.
2. Give another copy to your doctor.
3. Take a copy with you when you are admitted to a hospital.
4. Show it to your family and friends and others who care for you.
*You must sign this form and two witnesses must also sign it before it is official.
What if I change my mind?
You may change your mind at any time, as long as you are competent. To do so, fill out a new form and date and sign it with witnesses. Then make sure everyone you gave the old form to has a copy of the new one.
You are not required to use this form. There are other forms available or you can write your own statement. If you have questions about other forms, please talk them over with the person who gave you this one, your doctor, social worker, friends or family.
Considerations for You and Your Health Care Agent
What are the things you want your agent to know?
Your agent will need to think about conversations you have had, your personality and how you handled other important issues in the past. It is important to think and talk with your agent and your family about such things as:
• The things that are most important to you in your life
• Whether you would rather be at home or in a hospital for the last days or weeks of your life
• How important it is to you to avoid pain and suffering
• Whether it is more important to you to live longer or avoid prolonged disability or suffering
• The way you would like to be remembered
• Your religious beliefs
• Your hopes for your family if you are very sick or dying
Write down here the most important thoughts you have about these things to help your agent make good decisions for you. You should also talk about these things with your health care agent, backup agents, and family so that everyone understands your wishes. This can avoid problems in the future.

Etoposide is a semi-synthetic derivative of podophyllotoxin that is effective against many cancers including small
cell lung cancer. We report a case of etoposide-induced anaphylaxis in a 51-year-old woman who tolerated etoposide
during her first cycle chemotherapy regimen. During the second cycle, she complained of generalized urticaria
and dyspnea 5 minutes after being infused with etoposide. She recovered completely with antihistamine, corticosteroid
and fluid replacement. The intradermal skin test with etoposide showed a clear positive immediate
reaction. This case suggests that etoposide can induce IgE-mediated anaphylaxis.

서 론
Etoposide는 podophyllotoxin의 반합성유도체로 성인
에서 다양한 암의 치료에 이용되는 항암제로 사용되고 있
으며1, cisplatin과의 병합요법은 소세포폐암의 표준치료
로 이용되고 있다2. 현재 널리 사용되고 있는 podophyllotoxin
etoposide는 경구제제와 주사제가 있으며, 골수기
능을 억제하는 주된 부작용을 제외하고는 일반적으로 큰
부작용 없이 사용할 수 있다3,4. Etoposide처럼 podophyllotoxin
반합성유도체로 유사한 항암범위를 갖는 teniposide는
주로 소아암에서 사용되며 teniposide에 대한 과민
반응은 잘 알려진 부작용 중의 하나이나, etoposide에서
는 드문 것으로 알려져 있다1. 하지만, 일부에서는 드물지
만 안면홍조, 호흡곤란, 두드러기, 저혈압 및 혈관부종을
특징으로 하는 과민반응이 보고되고 있다3-8. 현재까지 국
내에서는 문헌검색상 이에 대한 보고가 없어, 저자들은
소세포암에 대한 치료로 etoposide와 cisplatin의 병합요
법을 시행한 환자에서 아나필락시스를 경험하였기에 증
례를 보고하는 바이다.
증 례
환 자: 박○○, 51세, 여자
주 소: 전신성 두드러기, 호흡곤란
과거력 및 사회력: 환자는 10년 전 교통사고로 우측 쇄
골골절에 대한 수술을 시행하였고, 금속에 대한 접촉성 피
부염의 과거력이 있었다. 그 외의 알레르기 질환을 포함한
다른 특별한 질환의 과거력이 없었고, 비흡연자이었다.
현병력: 환자는 2009년 2월부터 지속된 기침 증상으로
2009년 3월 인근병원에 방문하여 시행한 흉부 전산화단층
촬영에서 폐 좌상엽의 5.8×3.2 cm 크기의 종양을 발견하

으며 뇌자기공명영상, 양전자방출단층촬영 검사 등을 시
행한 결과 병기는 제한기였다. 이후 항암치료 위해 본원
으로 전원되었고, 2009년 4월 3일부터 5일까지 1주기 EP
(Etoposide 100 mg/m2＋Cisplatin 60 mg/m2) 요법과 함
께 흉부방사선치료 병행요법을 시작하였다. 1주기 항암치
료 시 환자는 특별한 문제없었고, 3주 후 2주기 항암치료
를 시행하였다. 항암치료 첫날 전 처치로 항구토제와 함
께 dexamethasone 10 mg을 정주한 뒤 etoposide 투여를
시작하였고, 5분 후 환자는 전신성 두드러기, 가려움 및
호흡곤란을 호소하였다. 이에 즉시 etoposide 투여를 중
지하였으며, 에피네프린, 항히스타민제와 함께 산소 및 수
액치료를 시작하였다. 응급처치 후 더 이상의 증상악화는
보이지 않았다.
신체검사 및 검사실 소견: Etoposide 투여한 뒤 증상
발현 시 활력 증후는 혈압 150/90 mm/Hg, 호흡수 24회/
분, 맥박수 108회/분, 체온 36.1oC이었고, 양측 폐야에서
천명음이 청진되었다. 검사실 소견에서 전체혈구계산에
서 백혈구 4,400/mm3, 호산구 40/mm3, 헤모글로빈 10.2
g/dL, 혈소판 34만/mm3으로 경한 빈혈 소견을 보였으나,
간기능 검사, 신장기능 검사, 전해질 검사, 심전도 검사
등은 정상이었다.
피부반응 검사: 환자는 etoposide에 대한 아나필락시스
가 의심되어 2주기 항암치료 시 etoposide는 사용하지 않
았고, cisplatin만 사용하였으며 특별한 문제없이 퇴원하였
다. 퇴원 3주 후 아나필락시스의 원인 약물을 확인하기 위
해 피부반응 검사를 시행하였다. 환자와 보호자에게 동의
서를 받은 뒤, etoposide와 cisplatin를 생리식염수에 희석
하여 0.1 mg/mL을 1：1 용액으로 하였고 양성 대조군은
히스타민용액(1.0 mg/mL), 음성 대조군은 생리식염수로
하였다. 우선 환자의 등에 피부단자검사를 하였으나 모두
음성이었고, 이어서 피내반응검사를 1：100, 1：10, 1：1
의 순서로 시행하였다. 히스타민의 팽진 크기는 19.6×
18.8 mm이었고, cisplatin의 경우 1：1 용액까지 모두 음
성이었으나 etoposide는 1：100, 1：10 용액에서 발적을
동반한 팽진이 각각 9.2×8.4 mm, 18.6×15.6 mm으로
양성 소견 보이면서 전신 가려움증을 호소하여 더 이상의
검사는 진행하지 않았다(Figure 1).
치료 및 경과: 환자는 임상양상 및 피부 반응 검사 결과
etoposide에 의한 아나필락시스로 진단되었다. 이후 외래
에서 환자에게 향후 치료계획에 대해 etoposide를 탈감작
하여 EP 요법을 지속하는 방법, etoposide를 사용하지 않
는 다른 항암요법으로의 변경하는 방법에 대해 설명을 하
였다. 환자 및 보호자는 아나필락시스에 대한 두려움이
커서 다른 항암요법으로의 변경을 원하였고, 일단 2주기
항암치료 시 계획되었던 방사선 치료를 마친 후 irinotecan과
cisplatin의 병합 치료를 시행하였다.
고 찰
본 증례는 etoposide 투여 후 전신성 두드러기, 호흡곤
란, 천명음 등의 함께 etoposide에 대한 피부반응 검사 양
성 소견을 통하여 아나필락시스가 증명된 경우로 문헌 검
색상 국내에서는 이에 대한 보고가 없다.
Etoposide는 1∼3%의 빈도로 과민반응을 초래하는 것
으로 알려져 있으며, 대부분의 경우 etoposide 투여 후 5
∼10분 내에 증상이 나타나 투여를 중단하면 완전히 회복
된다1,4,9. Etoposide와 관련한 과민반응의 정확한 기전은
아직까지는 명확하지 않다. 대부분의 환자에서는 etoposide를
처음 투여 후 과민반응이 나타나3-5,7-9, 이런 etoposide에
대한 즉각적인 반응이 immunoglobulin E (IgE) 매
개성이 아닌 etoposide에 대한 유사 아나필락시스 반응으
로 생각되기도 한다1,9. 하지만 처음 투여 시에 특별한 문
제가 없었으나 2회 이상 투여한 경우 과민반응이 나타나
는 경우도 있다6,7. 이는 제 1형 과민반응인 아나필락시스
반응의 가능성을 시사하며, 본 증례에서도 1주기 항암치
료 시에는 etoposide와 관련한 부작용이 발생하지 않았으
나 2주기 때는 과민반응을 보였고 etoposide에 대한 피부
반응검사에서 양성소견으로 IgE 매개성의 아나필락시스

반응을 생각할 수 있었다. 한편, 일부 보고에서는 etoposide
주사제제는 polysobate 80 첨가제가 포함되어 있어,
이 polysobate 80이 과민반응의 원인일 것으로 생각되었
다1. 이는 polysorbate 80을 함유하지 않는 경구용 etoposide는
주사용 etoposide와는 달리 과민반응에 대한 증례
가 없고1, 동물 실험에서 polysorbate 80이 히스타민을 분
비하도록 유도하여 과민반응을 초래하는 사실에 근거를
두고 있다10.
Teniposide 또한 podophyllotoxin 반합성유도체로 약
6.5%의 빈도로 과민반응이 발생한다11. Teniposide가 유
사한 podophyllotoxin 반합성유도체인 etoposide에 비해
과민반응이 더 많이 발생하는 이유는 잘 알려 지지 않았으
나, teniposide는 etoposide보다 더 높은 농도로 활성 대
사물질이 발생할 수 있고 teniposide는 etoposide 주사제
와 달리 cremophor EL 첨가제가 포함되는 차이 때문으로
설명하기도 한다1. 하지만, 실제로 이런 차이 때문에 두
약제에서 과민반응의 빈도 차이가 나는지에 대해서는 논
란이 있으며, 추가 연구가 필요하다.
Etoposide와 관련한 과민반응에 대한 위험인자는 잘 알
려져 있지 않지만, 일부 보고에서는 약물 알레르기가 있었
던 환자에서 발생한 경우들이 있었다7. 알레르기 질환의
과거력이 있는 환자에서 etoposide가 포함된 항암치료 시
과민반응에 대해 주의가 필요할 것으로 생각되나, 일부
증례3,4와 본 증례처럼 특별한 알레르기 질환의 과거력이
없는 환자에서도 과민반응은 발생할 수 있다.
Etoposide에 대한 과민반응이 발생한 경우 추후 항암치
료의 유지 여부는 중요한 문제이다. 만일 etoposide의 사
용이 반드시 필요한 경우라면 스테로이드제, 항히스타민
제의 전처치와 함께 etoposide의 주입속도를 천천히 하여
주입하는 방법이 있다8,9. 다른 방법으로는 etoposide의 전
구약제이며 polysorbate 80을 함유하지 않는 etoposide
phosphate를 사용하는 것으로, 처음 사용시에는 스테로
이드제 및 항히스타민제로 전처치를 하고 이 후부터는 전
처치 없이 과민반응이 나타나지 않고 안전하게 사용한 보
고가 있다3,4. 이번 증례에서는 환자에게 두 가지 치료 방
법에 대해 설명을 한 상태이나 etoposide 사용으로 인한
아나필락시스에 대한 두려움이 큰 상태로 환자는 항암요
법의 변경을 선택하였다.

A Case of Anaphylaxis after the Treatment with Etoposide in a Patient with Small Cell Lung Cancer

Etoposide is a semi-synthetic derivative of podophyllotoxin that is effective against many cancers including small cell lung cancer. We report a case of etoposide-induced anaphylaxis in a 51-year-old woman who tolerated etoposide during her first cycle chemotherapy regimen. During the second cycle, she complained of generalized urticaria and dyspnea 5 minutes after being infused with etoposide. She recovered completely with antihistamine, corticosteroid and fluid replacement. The intradermal skin test with etoposide showed a clear positive reaction immediately. This case suggests that etoposide can induce IgE-mediated anaphylaxis.

Introduction
Etoposide, a semi-synthetic derivative of podophyllotoxin, is used as an antitumor agent against various adult cancers1 and its combination therapy with cisplatin is known as the standard treatment for small cell lung cancer.2 Podophyllotoxin etoposide which is currently widely used is available in a form of oral agent or injection. In general, it can be used without serious side effects except mainly bone marrow depression.3,4 As a semi-synthetic derivative of podophyllotoxin like etoposide, teniposide has a similar range of antitumor effect. Teniposide is chiefly used against children cancer and is well known to show anaphylaxis as one of side effects, while it is known that etoposide rarely shows this symptom.1 However, anaphylaxis characterized with hot flush, dyspnea, urticaria, hypotension and angioedema is reported rarely in some cases.3-8 In Korea, no report regarding this issue has been found from literature search until now. Therefore, we report a case of anaphylaxis occurred to the patient who was being treated with the combination therapy of etoposide and cisplatin for small cell lung cancer.

Patient: Park OO, 51 years old, Female
Chief Complaints: Generalized urticaria, Dyspnea
A history and social history: The patient had a surgery for the right side collarbone fracture from car accident 10 years ago and had a history of contact dermatitis to metals. She had no past history of other specific diseases including allergy and was not a smoker.
Current history: The patient took a chest CT scan at a local hospital in March, 2009 because of a persistent cough symptom since February, 2009. From the scan, a 5.8×3.2 cm size of tumor was detected at the left lung upper lobe and the patient was diagnosed with small cell lung cancer from a bronchial endoscopic biopsy. Based on the test results of brain MRI and positron emission tomography (PET), it was determined that the stage was the limited stage. Thereafter, the patient was transferred to the present hospital. The first cycle EP (Etoposide 100 mg/m2＋Cisplatin 60 mg/m2) treatment combined with chest radiation therapy was carried out from April 3, 2009 to April 5, 2009. During the first cycle of chemotherapy, the patient did not show any particular problem. The second cycle of chemotherapy began after 3 weeks. As pretreatment, dexamethasone 10 mg with antiemetics was injected to the patient on the first day of chemotherapy. However, the patient complained of generalized urticaria, pruritus, and dyspnea 5 minutes after being injected with etoposide. Accordingly, the etoposide injection was immediately stopped and air and fluid replacement along with epinephrine and antihistamines was followed. There was no further development of the symptom after urgent care.
Physical examinations and laboratory report: When the symptom occurred after etoposide injection, the vital signs were as follows: blood pressure 150/90 mm/Hg, breathing rate 24 times/ min, pulse rate 108 times/min, body temperature 36.1oC and stridor was detected from both pulmonary areas. Based on the laboratory report of complete blood count (CBC)-white blood cell 4,400/mm3, eosinocyte 40/mm3, hemoglobin 10.2 g/dL, and platelet 340,000/mm3, a light anemia was detected whereas other test results of liver function, renal function, electrolyte, and ECG were normal.
Skin test: The patient was discharged without any problem after the second cycle of chemotherapy, where only cisplatin was used because of the suspicion of etoposide-induced anaphylaxis. Skin test was done to confirm the source causing anaphylaxis 3 weeks after discharge. After obtained the consent form from both the patient and her patron, 1 : 1 solution was prepared with 0.1 mg/mL of each solution of etoposide and cisplatin diluted into physiological saline. Histamine solution (1.0 mg/mL) for positive control and physiological saline for negative control were employed. First of all, the skin prick test on the patient's back showed a negative reaction. A series of intradermal tests were followed in the order of 1：100, 1：10, and 1：1. The size of histamine wheal was 19.6×18.8 mm. In case of cisplatin, the result was negative up to 1：1 solution. For the case of etoposide, however, the result was positive in both 1: 100 and 1: 10 solutions, where the wheals along with skin flares were observed with sizes of 9.2×8.4 mm and 18.6×15.6 mm, respectively. The test needed to stop as the patient complained of generalized pruritus (Figure 1).
Treatment and progress: The patient was diagnosed with anaphylaxis induced by etoposide based on the clinical appearance and skin test result. Henceforth, future treatment option was introduced to the patient in outpatient clinic: either to continue EP therapy with using desensitized etoposide or to switch to another cancer treatment method which excludes the involvement of etoposide.

Both the patient and her patron wanted to change to another cancer treatment method out of fear of anaphylaxis. Therefore, for the time being, the combination therapy of irinotecan and cisplatin was carried out after finishing radiation therapy as planed for the second cycle of chemotherapy.
Discussion
Based on the literature search, no such a case was reported in Korea as this one, where anaphylaxis was confirmed by the positive result of skin test with etoposide along with the occurrence of generalized urticaria, dyspnea, and stridor after etoposide injection.
It is known that etoposide causes anaphylaxis with frequency of 1∼3%. The symptom occurs 5 ∼10 min after injecting etoposide and disappears completely as the injection is stopped.1,4,9 The mechanism of anaphylaxis related with etoposide is not clearly known. To most patients, anaphylaxis occurs after the first etoposide injection.3-5, 7-9 This immediate reaction with etoposide is considered to be a kind of anaphylaxis, not immunoglobulin E (IgE)-mediated, but etoposide induced.1,9 However, there are some cases where anaphylaxis occurs only after more than two etoposide injections while no indication of anaphylaxis for the first injection.6,7 This indicates the possibility of anaphylaxis of Type 1 hypersensitivity reaction. In the present case, the symptom of anaphylaxis appeared during the second cycle of chemotherapy while no side effect related with etoposide occurred during the first cycle. Based on this observation along with the positive result of skin test with etoposide, IgE-mediated anaphylaxis was considered. In the mean time, it was reported that the polysorbate 80 added into the etoposide injection as an additive could be the reason for the anaphylaxis.1 This suggestion was based on the fact that, unlike the etoposide injection, no anaphylaxis has been reported in case of oral etoposide in which no polysorbate is added1and that polysorbate 80 in the animal test induces to secrete histamine, resulting in causing the anaphylaxis.10
Teniposide, a semi-synthetic derivative of podophyllotoxin, also induces anaphylaxis with frequency of 6.5%.11 The reason is not found why teniposide induces anaphylaxis with a higher frequency than etoposide which is a similar semi-synthetic derivative of podophyllotoxin. It was suggested that this could be due to the fact that teniposide produces higher concentration of active metabolic agents than etoposide and due to the difference that teniposide contains cremophor EL additives while etoposide does not.1 It is controversial to conclude that the difference in frequency of anaphylaxis with two agents is due to the above mentioned reasons, however, and further research is needed.
Even though the risk factor of anaphylaxis related with etoposide is not well known, there were some cases of occurrence to the patients who had drug allergies according to some reports.7 Upon carrying out chemotherapy including etoposide, a caution for anaphylaxis is needed for those who have allergy history. But, anaphylaxis can occur to those who have no allergy history as in some cases3,4 and this one.
It is critical to decide how to carry out follow-up chemotherapy when etoposide-induced anaphylaxis occurs. If it is necessary to use etoposide, etoposide can be injected slowly along with pretreatment of steroid and antihistamine agents.8,9 As another option, etoposide phosphate, a precursor of etoposide, that contains no polysorbate 80 can be used. Some successful cases were reported, in which no anaphylaxis occurred with using etoposide phosphate along with pretreatment of steroid and antihistamine agents for the first time while with no pretreatment thereafter.3,4 In the present case, the patient decided to switch to another cancer treatment method because of the great fear of anaphylaxis caused with etoposide even though these two treatment options were suggested.
