L’insufficienza cardiaca rappresenta una delle principali cause di morte nei paesi industrializzati con tendenza a colpire una percentuale sempre maggiore della popolazione. Secondo una stima dell’American Heart Association (AHA) risalente al 2003 si rigistra un’incidenza di 565.000 nuovi casi ogni anno di insufficienza cardiaca la cui causa è rappresentata nel 70% dei casi dalla patologia coronarica [1,2]. Il più importante fattore determinante la sopravvivenza a lungo termine è costituito dal grado di disfunzione ventricolare sinistra [3-5], pertanto è di primaria importanza la valutazione della vitalità miocardica residua nei pazienti con malattia coronarica cronica e disfunzione ventricolare sinistra. Numerose tecniche sono in grado di identificare il tessuto vitale mediante differenti parametri: l’Eco-stress con Dobutamina valuta la riserva contrattile; la 201Tl SPECT permette di studiare l’integrità della membrana dei miocardiociti; la 99mTc SPECT studia invece l’attività mitocondriale; la 18FDG-PET fornisce una valutazione del metabolismo in relazione alla captazione glucidica, da correlare alla perfusione basale (mismatch perfusione/metabolismo); infine la Risonanza Magnetica è in grado di dimostrare direttamente la presenza della cicatrice post-infartuale. | Translation - Italian
Heart failure is one of the main causes of death in industrialised countries with a trend to affect an increasing percentage of population. According to a 2003 study by the American Heart Association (AHA), the rate of new cases of heart failure amounts to 565,000 per year; 70% of the cases are caused by the coronary pathology [1,2]. The most important factor determining long-term survival is the grade of left ventricular dysfunction [3-5]; it is therefore crucial to asses the residual myocardial viability of patients affected by chronic coronary disease and left ventricular dysfunction. There are many techniques to identify the viable tissue through different parameters: the dobutamine stress echocardiogram evaluates the contractile reserve; the 201Tl SPECT allows to study the integrity of the myocardial membrane; the 99mTc SPECT, indeed, studies the mitochondrial activity; the 18FDG-PET evaluates the metabolism in relation with the glucose uptake to be linked to the basal perfusion – perfusion/metabolism mismatch; finally, the MRI scan can directly demonstrate the presence of the postinfarction scar. |
