La concentration de cholestérol-HDL sérique (C-HDL) est un élément clé de l’évaluation biologique du risque d’athérosclérose. Un résultat erroné de dosage du C-HDL peut modifier l’estimation du risque global et en conséquence la prise en charge thérapeutique du patient. Compte tenu de l’apparition sur le marché de nouvelles méthodes par dosage direct, et des recommandations internationales et nationales en matière de diagnostic et du suivi des dyslipidémies (Afssaps, NCEP), l’Afssaps a mis en place un contrôle du marché portant sur ces dispositifs.
Les évaluations ont été réalisées sur un panel constitué de façon à inclure les valeurs limites entre lesquelles une qualité analytique est indispensable à une bonne prise en charge des patients. Vingt réactifs de dosage de C-HDL par méthode directe ont été recensés sur le marché, et 38 adaptations ont été évaluées selon un protocole établi par un groupe d’experts.
Les résultats de l’évaluation technique montrent des performances adaptées en terme d’exactitude pour les concentrations à l’intérieur des valeurs cibles décisionnelles pour 21 adaptations ; la précision est conforme pour l’ensemble des dispositifs. Une classification des couples automate-réactif est proposée selon les valeurs de l’erreur totale observée, et de l’impact clinique susceptible d’en résulter.
Concernant les notices et au regard de la directive 98/79/CE, des non-conformités et ou remarques ont été relevées, notamment : l’inexactitude ou l’absence d’indication sur le système de standardisation, les seuils de risques, les résultats de performances ou les interférences.

Serum cholesterol-HDL concentration (C-HDL) is a key component of the biological evaluation of the risk of atherosclerosis. An erroneous result in dosing C-HDL can modify the estimate of total risk and consequently the therapeutic management of the patient. Taking into account the appearance on the market of new methods of direct assay, and of the international and national recommendations regarding diagnosis and of the follow-up of dyslipidemias (Afssaps, NCEP), Afssaps has set up a control of the market for these devices.
The evaluations were carried out on a panel made up to include the limiting values between which analytical quality is essential for correct patient management. Twenty C-HDL assay reagents by direct method were listed on the market, and 38 adaptations were evaluated according to a protocol drawn up by an expert group.
The results of the technical assessment demonstrate appropriate performance in terms of accuracy for concentrations within the decisional target values for 21 adaptations; the accuracy is compliant for all of the devices. A classification of the reagent - automated analyzer couples is proposed according to the total error value observed, and the clinical impact likely to result from it.
Concerning the information leaflet and with regard to directive 98/79/CE, certain nonconformities and/or remarks were raised, in particular: inaccuracy or absence of indication on the standardization system , risk thresholds and the results of performances or interferences.

Contexte : La Dégénérescence Maculaire Liée à l’Age (DMLA) est la première cause de malvoyance chez les patients âgés dans les pays industrialisés. La DMLA néovasculaire ou « humide », caractérisée par la présence de néovaisseaux choroïdiens, représente la forme la plus agressive de la maladie. Sa prévalence est de 3,3% chez les patients de plus de 65 ans en Europe, et elle augmente avec l’âge. Les traitements ont évolué depuis 30 ans du laser thermique à la photothérapie dynamique (PDT). Récemment sont apparues les injections intraoculaires d’anticorps monoclonaux anti-angiogéniques (anti-VEGF). Deux études ont permis la consécration du ranibizumab à une posologie d’une injection intravitréenne par mois pendant 2 ans en cas de néovaisseaux rétrofovéolaires. Le ranibizumab aurait une affinité rétinienne plus importante et une pénétration facilitée par rapport au bevacizumab, molécule mère de plus grosse taille dont il est issu. Cependant, de nombreuses publications de séries de cas semblent montrer d’une part l’efficacité du bevacizumab et d’autre part un profil de tolérance satisfaisant. Les avantages théoriques du bevacizumab sont représentés 1) par son coût (variant théoriquement d’un facteur 4 à 50 fois plus faible), élément de santé publique à considérer sérieusement vu la fréquence de la DMLA néovasculaire et le vieillissement de la population lié à l’accroissement de l’espérance de vie ; 2) par la possibilité éventuelle de diminuer la fréquence et le nombre des injections, car la taille du bevacizumab permettrait une demi-vie prolongée dans l’œil en comparaison du ranibizumab, ce qui sous-entend une diminution du risque global et une diminution supplémentaire du coût de prise en charge des patients.
Objectif principal :
L’objectif de l’étude est de montrer la non-infériorité en terme d’efficacité clinique à 12 mois du bevacizumab par rapport au ranibizumab sur l’acuité visuelle de patients atteints de DMLA néovasculaire rétrofovéolaire.
Objectifs secondaires :
o Evaluer et comparer l’efficacité des traitements par bevacizumab et ranibizumab en terme :
- D’évolution de l’acuité visuelle,
- De diminution de la taille de la lésion néovasculaire,
- De disparition de la zone de diffusion,
- D’amélioration des critères OCT (tomographie par cohérence optique) ;
o Evaluer et comparer la tolérance des traitements par bevacizumab et ranibizumab au niveau local et systémique ;
o Décrire et comparer les schémas posologiques (nombre d’injections et délai de réinjection) dans les deux groupes ;
o Réaliser une étude du profil pharmacocinétique des médicaments (sous groupe de 20 patients) ;
o Réaliser une modélisation médico-économique de l’impact lié à l’intervention thérapeutique, à partir des résultats des deux groupes de randomisation.
Critère de jugement principal :
Proportion de patients ayant perdu moins de 15 lettres d’acuité visuelle à 12 mois post initiation du
traitement par rapport à l’acuité visuelle initiale, l’acuité visuelle étant mesurée sur l’échelle « Early Treatment Diabetic Retinopathy Study » (ETDRS) à une distance initiale de 4 mètres.
Méthodologie : Etude clinique thérapeutique de non-infériorité, de phase III multicentrique française, prospective, randomisée, en double insu.
Produit testé : Bevacizumab, administré par voie intravitréenne à la posologie de 1,25 mg par injection, au maximum 1 fois par mois.
Traitement de référence : Ranibizumab, administré par voie intravitréenne à la posologie de 0,50 mg par injection, au maximum 1 fois par mois.
Patients :
- Critères d’inclusion :
o Patient d’âge supérieur ou égal à 50 ans
o Présentant une DMLA néovasculaire de localisation rétrofovéolaire quel que soit le sous-type, unilatérale ou bilatérale (l’œil inclus sera le moins atteint mais les 2 yeux auront le même traitement)
o Meilleure acuité visuelle corrigée pour l’œil étudié comprise entre 20/40 (5/10ème) et 20/320 (0,6/10ème) (équivalent Snellen), en échelle ETDRS (Early Treatment of Diabetic Retinopathy Study)
o Taille de la lésion < 3 diamètres papillaires (DP) dans sa plus grande dimension linéaire (soit < 5400 μm) ou < 9 surfaces papillaires (SP) (1 surface papillaire = 2,54 mm²)
o En cas de néovaisseaux occultes isolés (« occult ») ou prédominants (« minimally classic »), nécessité de la preuve d’une évolutivité récente de la lésion : baisse de l’acuité visuelle d’au moins une ligne Snellen (ou équivalent) dans les 6 derniers mois OU apparition d’une hémorragie sous-rétinienne associée à un néovaisseau choroïdien au cours du dernier mois OU augmentation de la taille de la lésion (> 10%) à l’angiographie fluorescéïnique au cours du dernier mois par comparaison avec les 6 derniers mois OU apparition de critères OCT à type d’œdème maculaire, de décollement séreux du neuro-épithélium, de décollement de l’épithélium pigmenté au cours du dernier mois
- Nombre de patients : 300 patients seront inclus par groupe, soit un total de 600 patients.
Intervention :
Tous les patients inclus seront randomisés en deux groupes :
le groupe I recevra des injections intraoculaires (dans le vitré) de ranibizumab.
le groupe II recevra des injections intraoculaires (dans le vitré) de bevacizumab.
Aucun autre traitement curatif de la DMLA ne sera utilisé.
Les patients bénéficieront d’une injection mensuelle systématique pendant les 3 premiers mois, puis seront surveillés de façon standardisée (une consultation par mois pendant 9 mois, schéma de suivi établi par consensus d’experts). En cas de perte de plus de 5 lettres d’acuité visuelle sur l’échelle ETDRS liée à l’évolution du néovaisseau et non à de l’atrophie/fibrose chorio-rétinienne isolée ET/OU en présence de critères OCT de persistance de la maladie par rapport à la mesure précédente, une nouvelle administration du traitement sera effectuée sans dépasser la fréquence d’une injection intravitréenne par mois.
Balance bénéfice / risque : La balance bénéfice / risque pour les patients participants à cette étude est acceptable, puisque nous faisons l’hypothèse que le bevacizumab a une efficacité suffisante et non inférieure et des effets indésirables similaires au ranibizumab du fait de sa structure moléculaire.

Context: Age-related Macular Degeneration (AMD) is the first cause of visual impairment in elderly patients in the industrialized countries. Neovascular or wet AMD, characterized by the presence of chorodial neovascularisation (CNV), represents the most aggressive form of the disease. Its prevalence is 3.3% among patients more than 65 years old in Europe, and this increases with age. Treatments have evolved over 30 years from the thermal laser to dynamic phototherapy (DPT). Intraocular injections of antiangiogenic monoclonal antibody (anti-VEGF) have appeared recently. Two studies have established the use of ranibizumab at a posology of one intravitreal injection per month over 2 years in the event of retrofoveolar neovessels. Ranibizumab is supposed to have greater retinal affinity and easier penetration compared to bevacizumab, the larger sized parent molecule from which it is derived. However, numerous publications of series of cases appear to demonstrate on the one hand the effectiveness of bevacizumab and on the other hand a satisfying tolerance profile. The theoretical advantages of bevacizumab are 1) its cost (theoretically varying from 4 to 50 times less), a factor of public health to be considered seriously considering the frequency of neovascular AMD and the ageing of the population related to the increase in life expectancy; 2) the possibility of decreasing the frequency and the number of injections, because the size of bevacizumab permits a prolonged half-life in the eye in comparison with ranibizumab, implying a reduction in total risk and an additional reduction in the cost of patient management.
Main aim:
The aim of the study is to demonstrate non-inferiority in clinical terms of effectiveness after 12 months of bevacizumab compared to ranibizumab on the vision of patients affected by neovascular retrofoveolar AMD.
Secondary objectives:
o To evaluate and compare the effectiveness of treatments by bevacizumab and ranibizumab in terms of:
- evolution of visual acuity,
- reduction in the size of the neovascular lesion,
- disappearance of the diffusion zone,
- improvement of the OCT criteria (Optical Coherence Tomography (OCT));
o To evaluate and compare the tolerance of the treatments by bevacizumab and ranibizumab at the local and systemic level;
o To describe and compare the posological scheme (number of injections and time before re-injection) in the two groups;
o To study the pharmacokinetic profile of the medicinal products (sub-group of 20 patients);
o To create a medico-economic model of the impact related to the therapeutic intervention, using the results of the two randomization groups.
Main judgment criterion:
The proportion of patients having lost less than 15 letters of vision acuity in 12 months post initiation of
treatment compared to initial vision, the vision being measured on the “Early Treatment Diabetic Retinopathy Study” (ETDRS) scale at an initial distance of 4 metres.
Methodology: Clinical therapeutic non-inferiority study, French phase III multi-centred, prospective, randomized, double blind.
Product tested: Bevacizumab, administered by intravitreal route at a posology of 1.25 mg per injection, maximum once a month.
Treatment of reference: Ranibizumab, administered by intravitreal route at a posology of 0.50 mg per injection, maximum once a month.
Patients:

o In the event of isolated occult neovessels (“occult”) or predominant (“minimally classic”), proof required of recent development of the lesion: loss of visual acuity of at least one Snellen line (or equivalent) in the last 6 months OR appearance of a subretinal haemorrhage associated with a choroidal neovessel during the last month OR increase in the size of the lesion (> 10%) using fluoresceinic angiography during the last month by comparison with the last 6 months OR appearance of OCT criteria of macular oedema type, serous separation of the neuro-epithelium, separation of the pigmented epithelium during the last month.
- Number of patients: 300 patients will be included by group, that is to say a total of 600 patients.
Intervention:
All the patients included will be randomized in two groups:
group I will receive intraocular injections (in the vitreous body) of ranibizumab.
group II will receive intraocular injections (in the vitreous body) of bevacizumab.
No other treatment for AMD will be used.
The patients will receive a systematic monthly injection during the first 3 months, then will be supervised in a standardized way (one consultation per month for 9 months, follow-up scheme established by expert consensus). In the event of the loss of more than 5 letters of visual acuity on the ETDRS scale linked to neovessel development and not to atrophy/ isolated chorioretinal fibrosis AND/OR in the presence of OCT criteria indicating persistence of the disease compared to preceding measurement, the treatment will be re-administered without exceeding the frequency of one intravitreal injection per month.
Benefit/risk ratio: The benefit/risk ratio for the patients participating in this study is acceptable, since it is assumed that bevacizumab is non-inferior and sufficiently effective and gives rise to undesirable effects similar to those of ranibizumab because of its molecular structure.
Total duration of the study: 31 months (December 2007 - June 2010)
• Recruitment of patients: 18 months
• Follow-up of the patients (individual time of participation in the study): maximum 13 months
Expected repercussions:
For the majority of the neovascular AMD, the use of bevacizumab should lead to:
• Obtaining an equivalent or even better effectiveness profile (improving visual prognosis).
• Decreasing the number of injections.

En cas de vague de chaleur, deux complications de gravité croissante sont à craindre, le syndrome d’épuisement-déshydratation et le coup de chaleur.
- le syndrome d’épuisement-déshydratation est la conséquence de l’altération du métabolisme hydrosodé provoqué notamment par la perte sudorale ; il apparaît en quelques jours.
- le coup de chaleur résulte d’une défaillance aiguë de la thermorégulation et constitue une urgence médicale extrême car il est à la fois d’apparition très rapide (1 à 6 heures) et d’évolution fatale (en moins de 24 heures) s’il n’est pas rapidement pris en charge.
De nombreux facteurs de risque individuels parmi lesquels l’âge extrême (nourrisson, enfant, personne âgée), les pathologies chroniques et les médicaments qui leur sont associés, peuvent altérer l’adaptation de l’organisme en cas de « stress thermique ». La personne âgée est particulièrement vulnérable en situation de forte chaleur climatique en raison d’une altération de sa capacité à ressentir la soif, d’un moindre contrôle de l’homéostasie du métabolisme hydrosodé et d’une diminution de ses capacités de thermorégulation par la transpiration. L’existence d’une dépendance (à l’extrême le confinement au lit) est un facteur de risque majeur. L’isolement social accroît tous ces risques.
Les données de la littérature actuellement disponibles ne permettent pas de considérer les médicaments comme facteurs de risque déclenchant des états pathologiques induits par la chaleur lorsqu’ils sont correctement utilisés. On peut seulement expliquer et prévoir, sur la base de leurs mécanismes d’actions, des effets potentiellement délétères de certains médicaments sur l’adaptation de l’organisme à la chaleur.
C’est donc la connaissance de l’ensemble des facteurs de risque, y compris le traitement médicamenteux déjà en place, qui permettra de proposer les mesures de prévention et de suivi adaptées à mettre en oeuvre en cas de vague de chaleur pour limiter les risques de survenue de syndrome d’épuisement-déshydratation ou de coup de chaleur chez les sujets fragilisés et sous traitement médicamenteux.
L’adaptation d’un traitement médicamenteux en cours doit être envisagée au cas par cas. Toute diminution de la posologie ou tout arrêt d’un médicament, doit être un acte raisonné qui doit prendre en compte la pathologie traitée, le risque de syndrome de sevrage et le risque d’effet indésirable. The existence of dependency (at the extreme, confinement to bed) is a factor of major risk. Social isolation increases all these risks.
The currently available data in the literature does not make it possible to consider drugs as factors of risk triggering off pathological states induced by heat when they are correctly used. One can only explain and envisage, on the basis of their mechanisms of action, the potentially noxious effects of certain drugs on the adaptation of the organism to heat.
It is therefore the knowledge of all the risk factors, including the drug treatment already in place, which will enable the setting-up of preventive measures and adapted follow-up to implement in the event of a heat wave to limit the risks of the occurrence of exhaustion-dehydration syndrome or heat stroke in weakened subjects and under treatment by drugs.
The adaptation of drug treatment in progress must be considered for each individual case. Le shampooing Etrivex 500 microgrammes/g doit être appliqué directement sur le cuir chevelu sec une fois par jour ; les lésions doivent être enduites et massées avec soin. Une quantité équivalente à une demi cuillerée à soupe de produit
(environ 7.5 ml) par application est suffisante pour couvrir tout le cuir chevelu. Le shampooing Etrivex 500 microgrammes/g doit être alors gardé sur le cuir chevelu sans occlusion pendant 15 minutes avant rinçage. Il faut se laver soigneusement les mains après application. Après 15 minutes, le produit doit être rincé avec de l'eau et/ou les cheveux peuvent être lavés avec une quantité supplémentaire de shampooing doux si nécessaire pour faciliter le lavage., Les cheveux peuvent ensuite être séchés normalement.
La durée de traitement doit être limitée à un maximum de 4 semaines. Etrivex 500 micrograms/g shampoo should be applied directly on dry scalp once daily taking care to well cover and massage the lesions. An amount equivalent to around a half tablespoon
(around 7.5 ml) per application is sufficient to cover all the scalp. Etrivex 500 micrograms/g shampoo should be then kept in place without covering for 15 minutes before rinsing. Hands should be washed carefully after application. After 15 minutes, the product must be rinsed with water and / or hair can be washed by using an additional amount of regular shampoo if needed to facilitate washing. Then, hair can be dried as usual.
The treatment duration should be limited to a maximum of 4 weeks. As soon as clinical results are observed, applications should be spaced out or replaced, if needed, by an alternative treatment. If no improvement is seen within four weeks, reassessment of the diagnosis may be necessary.
Repeated courses of Etrivex 500 micrograms/g shampoo may be used to control exacerbations provided the patient is under regular medical supervision.
Paediatric population
The experience in the paediatric population is limited. Etrivex 500 micrograms/g shampoo is not recommended for use in children and adolescents below 18 years of age. It is contraindicated in children under 2 years of age (see sections 4.3 and 4.4).

Use during pregnancy
There are no adequate data from the use of alendronic acid in pregnant women. Alendronic acid given during pregnancy in rats caused dystocia related to hypocalcaemia (see 5.3 'Preclinical safety data'). Given the indication, alendronic acid should not be used during pregnancy.
Use during lactation
It is not known whether alendronic acid is excreted into human breast milk. Given the indication, alendronic acid should not be used by breast-feeding women.
4.7 Effects on Ability to Drive and Use Machines

4.8 Undesirable Effects
In a one-year study in post-menopausal women with osteoporosis the overall safety profiles of
Alendronic acid 70 mg Tablets (n=519) and alendronic acid 10 mg/day (n=370) were similar.
In two three-year studies of virtually identical design, in post-menopausal women (alendronic acid 10 mg: n=196, placebo: n=397) the overall safety profiles of alendronic acid 10 mg/day and placebo were similar.
Adverse experiences reported by the investigators as possibly, probably or definitely drug-related are presented below if they occurred in 1% in either treatment group in the one-year study, or in 1% of patients treated with alendronic acid 10 mg/day and at a greater incidence than in patients given placebo in the three-year studies:

4.8 Effets indésirables
Dans une étude d'un an chez des femmes post-ménopausiques souffrant d'ostéoporose, les profils de sécurité globaux des comprimés d'acide alendronique 70mg (n=519) et d’acide alendronique 10 mg/jour (n=370) étaient similaires.
Dans deux études de trois ans de conception pratiquement identique, chez des femmes post-ménopausiques (acide alendronique 10 mg: n=196, placebo : n=397) les profils de sécurité globaux de l'acide alendronique 10 mg/jour et du placebo étaient similaires.
Les effets indésirables rapportés par les investigateurs comme étant possiblement, probablement, ou certainement liés au médicament sont
présentés ci-dessous s'ils se produisaient dans ^ 1% dans l'un ou l'autre groupe de traitement dans l'étude d'un an, ou dans S 1% des
patients traités avec de l'acide alendronique 10 mg/jour et à une plus grande incidence que chez les patients recevant le
placebo dans les études de trois ans ;

L’entérocolite nécrosante aiguë ou EAN est une nécrose partielle ou totale de l’intestin grêle et/ou du colon, de topographie variable, à point de départ muqueux, d’évolution transmurale, sans oblitération vasculaire mésentérique (1).
L’Entérocolite aiguë nécrosante est une pathologie multifactorielle où de nombreux éléments peuvent seuls ou en association, entraîner un trouble de la micro-circulation de la paroi de l'intestin. Cette altération de la micro-circulation aboutit à terme à une nécrose infectieuse de la paroi intestinale.
Cette nécrose s’installe et s’étend rapidement dans tout l’intestin grêle et le côlon laissant indemne le mésentère et le mésocôlon et ne comporte aucune thrombose artérielle ou veineuse.
Elle n’est donc distinguée des autres formes de colites aiguës graves (colites pseudomembraneuses, ulcéreuses, ischémiques, amibiennes) que par une analyse anatomique per-opératoire ou nécropsique. Les EAN s’observent de façon épidémique ou sporadique. Les premières épidémies ont été décrites en 1946 dans la région de Hambourg.
L’atteinte intestinale est essentiellement jéjunale. La responsabilité pathogène des différents groupes de germes de type Clostridium welchii perfringens et de leur toxine est mise en évidence. Les auteurs anglo-saxons rapportent en 1961 sous le terme de « necrotizing colitis » six observations d’EAN et à cette occasion décrivent les principaux aspects anatomo-cliniques et thérapeutiques de cette affection gravissime (2).
Les germes peuvent provoquer une véritable gangrène intestinale ou plus sournoisement, une altération circulatoire au niveau muqueux et sous-muqueux. C'est la conséquence d'une ingestion d'aliments souillés par des anaérobies virulents: c'est la forme pseudo-épidémique ou maladie de Hambourg apparaissant volontiers chez des sujets malnutris, avec un rythme saisonnier et touchant avec prédilection l'intestin grêle. Mais cela reste exceptionnel.
L'entérocolite dans les pays développés, est dans la majeure partie des cas, de type endogène. Certaines conditions locales et générales sont à l'origine d'une prolifération de la flore microbienne saprophyte. C'est le mode d'action d'une insuffisance cardiaque, d'une intervention chirurgicale, d'un polytraumatisme, d’une allergie, d’une distension mécanique, voire d'une chute des défenses immunitaires lors d'un diabète. | Translation - English
1 - ACUTE NECROTIZING ENTEROCOLITIS
Necrotizing enterocolitis or NEC is a partial or total necrosis of the small intestine and/or of the colon, of variable topography, with a mucosal starting point, of transmural progression, without mesenteric vascular obliteration (1).
Acute necrotizing enterocolitis is a multifactorial pathology in which many elements can, on their own or in association, lead to a disorder of the microcirculation of the wall of the intestine. This deterioration in microcirculation leads to an infectious necrosis of the intestinal wall in the long term.
The necrosis settles and extends quickly through the small intestine and colon leaving the mesentery and the mesocolon intact and not causing artrial or veinous thromboses.
It can only then be distuinguished from the other forms of serious acute colitides (pseudomembranous, ulcerous, ischaemic, amoebic colitis) by intraoperative period anatomical analysis or at autopsy. NEC is observed in an epidemic or sporadic manner. The first epidemics were described in 1946 in the region of Hamburg.
The affected intestine is primarily jejunal. The pathogenic role of the various groups of germs of Clostridium welchii perfringens and their toxin is highlighted. The anglo-saxons report in 1961 under the term of “necrotizing colitis” six observations of NEC and describe the principal anatomo-clinical and therapeutic aspects of this extremely serious affection (2).
The germs can cause a genuine intestinal gangrene or more insidiously, a circulatory deterioration of the mucous and submucous. This is the result of the ingestion of food soiled by virulent anaerobes: it is the pseudo-epidemic form or Hamburg disease appearing readily in malnourished subjects, with seasonal rhythm and targeting primarily the small intestine. But this remains exceptional.
Enterocolitis in the developed countries is endogenous in most cases. Certain local and general conditions are at the origin of a proliferation of the saprophytic microbial flora. These include for example cardiac failure, a surgical operation, a polytraumatism, an allergy, mechanical distension, even a drop in immunizing defences during diabetes. Mucosal and submucosal anoxia favours the colonization of the wall by germs which become virulent.
Whatever the mechanism is, the germs locally cause gangrene and the release of endotoxins which pass into the general circulation. This leads to deep shock, worsening the intestinal anoxia.
1.1 Clinical picture (1)
The clinical symptomatology is not very evocative, and do not reflect the gravity of the intra-peritoneal lesions. The progression occurs in two distinct periods. The initial period, less than 24 hours, is marked by functional digestive signs: paroxystic pains, stinking or bloody diarrhoea and vomiting. In the established phase, there is severe deterioration of the general state, contrasting with an abdominal syndrome which is often less alarming. The pain is localized preferentially on the left side, with no referral dorsally, vomiting is present in 20% of cases, the diarrhoea can then disappear and the picture is one of asthenic peritonitis with silent abdomen. Spontaneous progression is towards death in the context of septic shock 2 to 3 days.
