II. CHARACTERISTICS OF OPIDN
The underlying pathology in OPIDN involves bilaterally symmetrical degeneration of sensory and motor axons in distal regions of peripheral nerves and spinal cord tracts [1] Generally, the longest, largest diameter fibers tend to be preferentially affected. The most prominent lesions are often found in the dorsal columns of the cervical spinal cord, especially in fasciculus gracilis. Injury to this tract results in specific sensory deficits, including loss of recognition of limb position and vibration sensitivity [1,2]. Pathogenesis studies indicate that the primary lesion in OPIDN is the axon rather than mielin sheath or the cell body of the neuron, and that demielination occurs secondarily to axonal degeneration. The process has been likened to a “chemical transection” of the axon, with subsequent Wallerian-type degeneration. Signs and symptoms of axonopathy appear after a delay of about 8 days following absorbtion of an effective dose of an OPIDN-producing (neuropathic) OP compound and will consist of abnormal sensations (paresthesia) in the extremities, including numbness and tingling. Incoordination of movement (ataxia) develops at about the same time as the sensory disturbances and may progress to partial flaccid paralysis (paresis) by day 10-21. Recovery from severe disease is usually poor, and there is no specific treatment [1,2].
III. NTE AND MECHANISM OF OPIDN INITIATION
Considerable evidence suggests that a neuronal protein, neuropathy target
esterase (neurotoxic esterase, NTE), is the primary target molecule in OPIDN
[3,25]. The disease is thought to be initiated by organophosphorylation and subsequent rapid and specific modification (aging) of the inhibited enzyme (Scheme 1). Aging involves postinhibitory loss of a substitutent from the inhibitor, resulting in a negatively charged phosphyl moiety covalently attached to the active site serine [3,4,7,26,27] This charged adduct renders the inhibited enzyme intractable to reactivation, even by strong nucleophiles like KF or oximes. Among classes of NTE inhibitors, phosphates, phosphonates, and phosphoroamidates cause OPIDN (Group A) (Fig.2). These compounds all contain at least one -O- or -NH- bridge linking an R-group to phosphorus, and are therefore capable of undergoing the aging reaction [1,4,12,28,29]. Whereas inhibition of NTE by phosphinates, sulfonates, and carbamates (Group B), is not accompanied by aging, OPIDN does not result. Moreover, these non-aging inhibitors afford protection against subsequently administered neuropathic OP compounds, apparently by preventing the formation of the required concentration of organophosphrylated and aged NTE [25,26,30]. These observations lead Martin Johnson to postulate that only NTE inhibitors which can undergo the aging reaction are neuropathic and that generation of a negatively charged group at the active site of NTE is essential for initiation of OPIDN [31]. The latter event is proposed to induce a toxic gain function in NTE that leads as a result to the axon degeneration [25,30,32]. The complete list of OPs tested in the hen for OPIDN can be found in reviews of Johnson [3,4,6,8,11,12]. In contrast to AChE, aging of the phosphylated NTE likely occurs by SN2 reaction [3]. For at leasts some OPs, for example DFP, the side group that is lost in the aging reaction translocates to a locus on the enzyme known as Z [3,33,34]. The phosphoroamide analogue of DFP mipafox, (iC3H7NH)2P(O)F, produces OPIDN in vivo, just as DFP. Recently it was shown [35] that aging of mipafoxinhibited NTE occurs by phosphoramido proton loss, not by classical side-group loss. These finding highlight the creation of a negative charge on the phosphyl adduct as the defining characteristic of a neuropathic compound. In experimental animals, initiation of OPIDN requires the concerted inhibition and aging of greater than 70% of neural NTE after single exposures [3,7,31]. The threshold inhibition level in humans is not known, although available data suggest that it is comparable to that observed in animal models [36]. It is important to realize that the mechanism and target for OPIDN are entirely different from the acute cholinergic neurotoxicity of OP compounds. OPIDN depends on a particular type of chemical modification of NTE rather than mere inhibition of its enzymatic activity. Inhibition of NTE is necessary, but not sufficient condition for OPIDN. Aging of the inhibited enzyme results in the toxicological outcome. The situation with NTE is completely different from that with AChE. Inhibition of sufficient amount of AChE results in cholinergic toxicity, regardless of whether or not aging of the inhibited AChE occurs. Aging of the inhibited AChE does not alter the type of toxic response, but it does change the options available for therapy against cholinergic toxicity – oximes are ineffective if aging of the enzyme has occurred. Moreover, oximes do not appear to affect the clinical course of OPIDN following administration of a neuropathic OP compound, except to allow survival of an othervise lethal dose of a compound that also has cholinergic toxicity. Early significant NTE inhibition after OP administration along with protection from OPIDN by reversible or non-aging NTE inhibitors, provide substantial evidence for a relationship between NTE inhibition and OPIDN [3,26,30,37]. The relationship between NTE inhibition/aging and development OPIDN has potential to be exploited as a biomarker [3,26,38]. Inhibition of brain NTE within hours of exposure to a neuropathic OP compound predicts the potential for OPIDN to appear in susceptible animal models (e.g., adult hens) after a delay of 1 to 3 weeks.
V. PHYSIOLOGICAL FUNCTION OF NTE
The primary sequence of human NTE [41] is highly similar (41% identity) to the Swiss Cheese protein (SWS) in neurons of Drosophila. SWS is involved in
neural development, and sws mutant flies show age-dependent neurodegeneration,
massive apoptosis and early death [45]. Similarly, NTE mRNA was shown to
express strongly in neural ganglia in mice starting at early embryonic development till day 13 postcoitum, consistent with a possible role for NTE in vertebrate neural development [48]. Knockout of the NTE gene was embryonic lethal in mice at an early embryonic age [49]. However the distribution of NTE in tissues suggested that this protein may have, in addition to potential vital roles in the nervous system, more general functions [50]. In adult animals NTE is present in the nervous system and in a variety of non-neural tissues [3,27] including gut, lymphocytes, skeletal muscles, kidney, and placenta [3,34]. NTE is also widely expressed during fetal development [48]. Recently Moser et al [50] has reported that NTE is essential for formation of extraembryonic tissues, especially in placenta, and its elimination, not loss of NTE expression by neurons, results in massive cell death within the embrio. To investigate NTE function in vivo, in adult nervous system, K. Akassoglow et al [51] used the cre/loxP site-specific recombinant strategy to generate mice with a specific deletion of NTE in neuronal tissues. Conditional inactivation of the mouse NTE gene resulted in elimination of NTE protein in the central nervous system. Loss of NTE function caused prominent neuronal pathology in the hippocampus and thalamus and also defects in the cerebellum. Absence of NTE resulted in disruption of the ER and vacuolar pathology in the nervous system. Thus, these results identify a physiological role for NTE in the nervous system and indicate that a loss-of-function mechanism may contribute to neurodegenerative diseases characterized by vacuolation and neuronal loss. Because NTE is closely associated with the ER and results in cellular damage when it as absent from neurons, it is plausible that abnormalities in protein folding, transport and degradation are directly influenced by NTE perturbations [51].
VI. ASSESSMENT OF NEUROPATHIC POTENTIAL OF OPs.
Although the mechanism of OP-induced neuropathy in adult vertebrates is still unclear, the relationship between NTE inhibition/aging and development of
OPIDN can be used as a biomarker [3,26,38]. Inhibition of >70% of brain NTE
within hours of exposure to ageable OP compounds, e.g. phosphates, phosphonates and phosphoramidates, produces OPIDN in adult hens after 1 to 3 weeks. NTE tests are now the essential component of regulatory tests recommended both by EPA an OECD. A great advantage is that every degree of NTE inhibition that is found in autopsy samples provides some guidance – the neuropathic effect of a given dose can be predicted to be positive, marginal or thoroughly negative. NTE inhibition has proved to be an excellent endpoint for in vitro assess ment of the neuropathic potential of ageable OP compounds, e.g., phosphates, phosphonates, and phosphoramidates [2,3,40]. Moreover, the relative potency of an OP compound or its active metabolite to inhibit NTE versus AChE (RIP) has been shown to correlate with the ratio between the neuropathic dose and the LD50 [52]. Initially, the correlation was based on the I50 (the inhibitor concentration required to inhibit 50% of the activity of the enzyme in vitro after preincubation for a fixed time under defined incubation conditions) as the index of inhibitory potency. However, kinetic approaches are thought to be more valid than fixedtime methods for determinations of inhibitory potencies of OP compounds against esterases [26,53-55]. When inhibition proceeds by pseudo-first-order kinetics, it is valid to calculate the I50 from the ki using the relationship, I50 = 0.693/(ki t), where t = time of preincubation of the enzyme with the inhibitor [56]. Under these conditions, either the RIP = ki(NTE)/ki(AChE) ratio or the corresponding I50(AChE)/I50(NTE) ratio can be used as a convenient index of the probable neuropathic potential of the compound [3,26,40,57-59]. Values of the ratio ki(NTE)/ ki(AChE) > 1 indicate that the dose required to produce OPIDN is less than the LD50, whereas values < 1 correspond to doses greater than the LD50 being required to produce OPIDN [3,26,40]. Furthermore, according to Johnson [3], compounds for which the ratio ki(NTE)/ki(AChE) is 0.05 should be subjected to careful toxicological study, because neuropathies caused by intoxication with such compounds may develop after successful therapy for acute cholinergic poisoning.
VII. BIOMONITORING OF HUMAN EXPOSURE TO NEUROPATHIC
OP COMPOUNDS
Rapid and specific detection of human exposure to chemical agents and
accurate diagnosis of chemically induced disease are essential components of
minimizing effects of terrorist acts on civilian populations. NTE has also been found in circulating lymphocytes and platelets [77-80], and lymphocyte NTE has been used or proposed for use as an accessible biomarker of animal and human exposure to neuropathic OP compounds [25,80-84]. Furthermore, lymphocyte NTE inhibition has been suggested as a predictor of OPIDN or an adjunct for its early diagnosis [5,80,85-87]. Despite the potential utility of lymphocyte NTE as a biomarker, the time, resources and relatively high sample volumes required to separate blood components mitigate against using isolated lymphocytes routinely to monitor NTE activity. Thus, if the need were to arise to assess exposures of individuals to neuropathic OP compounds, it would be advantageous to be able to assay NTE in small volumes of whole blood [88]. Measurement of NTE activity has classically been done by colorimetric determination of phenol released by hydrolysis of the substrate, phenyl valerate [89,90]. The absorbance maximum of the red phenol chromophore overlaps substantially with that of whole blood homogenates and dilution of the blood to remove the interfering absorbance decreases NTE activity below the detection limit of the colorimetric assay [88]. Thus, the colorimetric assay cannot be used to assay NTE in whole blood. The problems inherent in a colorimetric NTE assay can be eliminated by using an amperometric technique to detect phenol. To create a fast and simple method for monitoring of human exposure to neuropathic OPs, a principal new approach to NTE activity analysis has been developed in joint study of the Institite of Physiologically Active Compounds Russian Acad. Sci. and Chemical Department of Moscow State University [88,91,92]. Recently, a new biosensor for NTE assay was introduced using a tyrosinase carbon-paste electrode to detect phenol produced by the hydrolysis of phenyl valerate. In this type biosensor phenol is quantified by measuring electroreduction of the generated o-quinone on a graphite electrode (Fig. 6) [88,91]. The tyrosinase carbon-paste electrode improved the sensitivity of the NTE assay 10-fold compared to the colorimetric method or an earlier amperometric technique based on oxygen detection [92]. Moreover, the new electrode operates in a flow-injection mode that requires only 2 to 3 min per analysis [88].

II. ХАРАКТЕРИСТИКИ НА OPIDN
Патологията, лежащата в основата на OPIDN, включва двустранно симетрична дегенерация на сетивните и моторни аксони в дисталните области на периферните нерви и в пътищата на гръбначния стилб [1]. В общи линии, по-дългите влакна и тези с по-голям диаметър се засягат по-лесно. Често най-тежките лезии се откриват в дорзалните колони на шийния отдел на гръбначния стълб, особено във fasciculus gracilis. Нараняването на този път води до специфични сетивни дефицити, включително загуба на разпознаването на позицията на крайник и вирбационна чувствителност [1,2]. Проучвания върху патогенезата посочват, че първоначалната лезия при OPIDN е на аксона, а не на миелиновата обвивка или на клетъчното тяло на неврона, а демиелинизацията настъпва вторично на аксоналната дегенерация. Процесът е оприличаван на „химично срязване” на аксона с последваща дегенерация тип Wallerian. Признаци и симптоми на аксонопатията се появяват около 8 дни след приемането на ефективна доза ОФ съединение, предизвикващо OPIDN (невропатично), и се изразяват в ненормална сетивност (парестезии) в крайниците, включително липса на чувствителност и тръпнене. Горе долу по същото време, с настъпване на сетивните нарушения се развива и некоординираност на движенията (атаксия), която може да прогресира до частична вяла парализа (пареза) към 10-21-ия ден. Възстановяването от тежко засягане обикновено е незадоволително и липсва специфично лечение [1,2].
III. NTE И МЕХАНИЗЪМ НА ИНИЦИАЦИЯТА НА OPIDN
Значителни доказателства говорят в полза на това, че един невронален протеин, невропатична таргетна естераза (невротоксична естераза, NTE), е главната таргетна молекула при OPIDN [3,25]. Предполага се, че заболяването се инициира от органофосфорилиране и последващо бързо и специфично модифициране (състаряване) на инхибирания ензим (Схема 1). Състаряването включва постинхибиторна загуба на част от инхибитора, водеща до образуването на отрицателно зареден фосфилен остатък, ковалентно свързан към серин в активния център [3,4,7,26,27]. Така заредената област прави инхибирания ензим неподатлив към реактивиране, дори от силни нуклеофили като KF или оксими. Сред класовете NTE инхибитори фосфатите, фосфонатите и фосфороамидатите предизвикват OPIDN (Група А) (Фиг. 2). Всички тези съединения съдържат поне един -О- или -NH- мост, свързващ R-група с фосфор, и следователно могат да претърпят реакцията на състаряване [1,4,12,28,29]. Тъй като инхибирането на NTE от фосфинати, сулфонати и карбамати (Група Б) не се съпътства от състаряване, не се стига до OPIDN. Предполага се, че последното индуцира увеличаване на токсичната функция на NTE, което води до аксонална дегенерация [25,30,32]. Важно е да се разбере, че механизмът и таргетът при OPIDN са съвсем различни от тези при острата холинергична невротоксичност на ОФ съединенията. OPIDN зависи от определен тип химична модификация на NTE, а не от обикновено инхибиране на ензимната му активност. Инхибирането на NTE е необходимо, но не достатъчно условие за OPIDN. Състаряването на инхибирания ензим води до токсикологични последствия. Ситуацията при NTE е напълно различна от тази при AChE. Инхибирането на достатъчно количество AChE води до холинергична токсичност, независимо от това, дали настъпва състаряване на инхибирания AChE. Състаряването на инхибирания AChE не променя типа на токсичния отговор, но променя наличните възможности за терапия срещу холинергичната токсичност – оксимите са неефективни, ако е настъпило състаряване на ензима. Освен това оксимите не повлияват клиничното протичане на OPIDN, предизвикана от въвеждането на невропатично ОФ съединение, освен, че водят до оцеляване при приемането на иначе летална доза съединение, което също има холинергична токсичност. Ранното значимо инхибиране на NTE след приемане на ОФ, съчетано с протекция от OPIDN чрез обратими или несъстаряващи NTE инхибитори предоставят сериозни доказателства относно връзката между NTE инхибирането и OPIDN [3,26,30,37]. Връзката между инхибирането/състаряването на NTE има потенциала да се използва като биомаркер [3,26,38]. Инхибирането на мозъчен NTE в рамките на часове след експозицията към невропатично ОФ съединение предсказва потенциала за появата на OPIDN у податливи животински модели (напр. възрастни кокошки) след забавяне от 1 до 3 седмици.
V. ФИЗИОЛОГИЧНА ФУНКЦИЯ НА NTE
Първичната последователност на човешки NTE [41] е много подобна (41% идентичност) на Swiss Cheese протеина (SWS) в невроните на Drosophila. SWS участва в невралното развитие, а мухите мутантни по sws показват невродегенерация, свързана с възрастта, масивна апоптоза и ранна смърт [45]. По подобен начин е показано, че mRNA за NTE се експресира силно в нервни ганглии на мишка от ранното ембрионално развитие до 13-ти посткоитален ден, което съответства на възможна роля на NTE в невралното развитие на гръбначните [48]. Нокаут на NTE гена при мишки в ранна ембрионална възраст е ембрионално летален [49]. Въпреки това разпределението на NTE в тъканите подсказва, че протеинът може да има освен потенциално жизненоважна роля в нервната система, също така и по-общи функции [50]. У възрастни животни NTE се намира в нервната система и в редица ненервни тъкани [3,27], включително черва, лимфоцити, скелетни мускули, бъбрек и плацента [3,34]. Експресията на NTE е разпространена също така през феталното развитие [48]. Наскоро Moser et al [50] съобщават, че NTE е жизненоважен за формирането на извънембрионални тъкани, особено в плацентата, и неговото елиминиране там, а не само загубата на NTE експресията в невроните, води до масивна клетъчна смърт в ембриона. За да изследват функцията на NTE in vivo в зряла нервна система K. Akassoglow et al [51] използват специфична за локуса cre/loxP рекомбинантна стратегия за да създадат мишки със специфична делеция на NTE в невроналните тъкани. Условното инактивиране на гена NTE у мишки води до елиминиране на NTE протеина в централната нервна система. Загубата на функцията на NTE предизвиква сериозна патология в хипокампуса и таламуса, а също и дефекти в церебелума. Липсата на NTE води до нарушения в ЕР и развитие на вакуоларна патология в нервната система. По този начин резултатите демонстрират физиологична роля на NTE в нервната система и показват, че един механизъм на загуба на функцията може да спомогне за развитието на невродегенеративни болести, характеризирани чрез вакуолизиране и невронална загуба. Тъй като NTE е тясно свързан с ЕР и неговата липса в невроните води до клетъчно увреждане, възможно е смущения в NTE директно да предизвикват аномалии в нагъването, транспорта и разграждането на протеините [51].
VI. ОЦЕНКА НА НЕВРОПАТИЧНИЯ ПОТЕНЦИАЛ НА ОФ
Въпреки че механизмът на индуцираната от ОФ невропатия е все още неясен, връзката между инхибирането/състаряването на NTE и развитието на OPIDN може да се използва като биомаркер [3,26,38]. Инхибирането на >70% от мозъчния NTE в рамките на часове след експозицията към състаряващи ОФ съединения, напр. фосфати, фосфонати и фосфороамидати, предизвиква OPIDN у възрастни кокошки след 1 до 3 седмици. В момента NTE тестовете са основният компонент на регулаторните тестове, препоръчвани от EPA и OECD. Голямо предимство е това, че всяка степен на NTE инхибиране открита в аутопсионен материал предоставя някакво напътствие – невропатичният ефект на дадена доза може да се предвиди като положителен, граничен или напълно отрицателен. Първоначално корелацията беше базирана на I50 (концентрацията на инхибитор, необходима да инхибира 50% от активността на ензима in vitro след преинкубация за определено време и при определени условия на инкубация) като показател на инхибиторна сила. Въпреки това се счита, че кинетичните подходи са по-подходящи от методите с фиксирано време за определяне на инхибиторната сила на ОФ съединения срещу тази на естерази [26,53-55]. Когато инхибирането се последва от псевдокинетика от първи ред, може да се изчисли I50 от ki, използвайки взаимоотношението I50 = 0.693/(ki t), където t = времето на преинкубация на ензима с инхибитора [56].
