Ejection Fraction and Heart Disease

The ejection fraction is an important tool in diagnosing heart failure, and determining how well your heart is pumping with each beat. It is a measurement of the amount of blood the left ventricle pumps out, or ejects, with each contraction, or beat.
Since the left ventricle of the heart is the largest and the main pumping chamber, the term ejection fraction, or EF, generally refers to the left ventricular ejection fraction. The EF consists of two separate calculations, one for each side of the heart, resulting in a separate percentage for the left ventricle (LVEF) and the right ventricle (RVEF), but in almost all cases, the measure of the LVEF is the important number.
A healthy heart has a normal LVEF, expressed as a percentage, and ranges from 50-75%. This is an indication that the left ventricles pumps out between 50 and 75% of its blood volume with each heartbeat. Medical experts consider an EF of less than 50 to be below normal, and can be due to damage to the heart muscle from a heart attack, heart muscle disease (cardiomyopathy) or other less common conditions. An EF of 35-40% may confirm a diagnosis of systolic heart failure. If the EF is less than 35%, the patient can be at risk of having life-threatening arrythmias, or irregularities in the heartbeat, and this can cause sudden cardiac arrest and death. For these patients, a physician may recommend an ICD, or implantable cardiac defibrillator. It is interesting to note that the EF can change, up and down, depending on the specific heart condition and treatment.
The term heart failure, sounds as if the heart is no longer working, as if it is failing. A more accurate explanation is that the heart attempts to adapt, when it is unable to pump adequate oxygen and nutrients through the blood to meet the needs of the body. The heart's chambers stretch to hold more blood volume, and hormones release into the bloodstream to improve the pumping ability of the heart. This provides temporary relief, but over time, the heart muscle walls weaken and/or stiffen.
Patients with diastolic heart failure may have a normal ejection fraction. When the heart muscle has thickened and stiffened from disease, the ventricle holds a smaller than normal amount of blood, and although the percentage of what is pumped out may be normal, the volume is too low to provide adequate perfusion to the body.
There are a number of tests available to measure ejection fraction. Examples of these tests include ultrasound of the heart, known also as an echocardiogram. This is the most frequently performed test, as it is noninvasive and can be done at the bedside or in a physician's office. There are also Cat Scans, MRI's, nuclear medicine scans, also known as MUGA scans, also known as nuclear stress testing, and finally cardiac catheterization. A cardiac catheterization is not typically performed to simply measure the ejection fraction of the heart; however if a patient is undergoing this invasive procedure for other diagnostic or treatment reasons, then the EF is measured as part of the procedure.
The importance of knowing the ejection fraction percentage relates to the diagnosis of heart disease, particularly heart failure. There are numerous modalities for the treatment of heart failure, and the physician and patient can discuss these together to determine the best treatment options for each individual.

At what point might you need a psychologist?
A psychologist is often called “a doctor of the soul”. This is effectively what characterises them: they are the professional that looks after the troubles, complex or not, which affect our emotions, our stability, our well-being, and the “state of our soul”.
When a feeling of uneasiness persists, when it affects our daily routine, our relationships and our work, and we can’t manage to identify the cause of it, or to resolve it by ourselves, a psychologist is then someone we are advised to talk to to try to put things “straight”.
Numerous physical symptoms also have psychological causes: putting your back out, headaches, continuous fatigue, and stomach ache are some of the pains which can be bodily manifestations of stress, anxiety or depression. A psychologist can help you to find the source of these troubles and work through the problems to relieve and solve them.
A psychotherapist will enable you to shed a light on the often unknown causes of your own mechanisms and reactions, and the work of a therapist will help you to bring them into focus and find the tools within you to handle your problems better, or even eradicate them. Therapy is a joint and participative process, where success largely depends on the will of the patient.
What happens at the first meeting?
At the first interview, the therapist will ask you some general questions about yourself and will ask you the reason(s) for you taking this step. They will decide with you what type of psychotherapy suits you. Operation und Maßnahmen:
xxx: Kolposkopie mit Essigprobe: o.B. xxx: offene operative Laparoskopie: Spülzytologie, Laparoskopische Adnektomie bds., Adhäsiolyse. Hinterer Scheidenwandplastik, Labien-PEs beidseits
Histologie:

im vorliegenden Material kein Tumorzellnachweis; Diagnostische Einstufung: negative Befunde: Röntgen-Thorax 1 E pa: Entgegen der auswärtigen Durchleuchtungsaufnahmen vom xxx lässt sich zwar der Stent in Projektion auf die LAD, aber nicht der verbliebene Führungsdraht nachweisen. Eher ist anzunehmen, dass dieser Draht aufgrund seines geringen Durchmessers nicht detektierbar ist, als dass er disloziert ist. Gegebenenfalls ergänzende Durchleuchtung sinnvoll je nach Rücksprache mit der Kardiologie. Das Herz normal groß und konfiguriert. Die Aorta ist elongiert und vermehrt schattendicht. Hili gefäßtypisch. Keine Mediastinalverbreiterung.
Seitengleiche Lungenbelüftung. Kein Erguss, kein frisches Infiltrat, keine pulmonale Stauung. Spondylosis deformans der BWS.
Internistisches Konsil:
Patientin selbst nicht gesehen, Fall wird mir von der Anänsthesie und der Tochter der Patientin (Ärztin im Hause) vorgetragen. Patientin soll präoperativ vor einer laparaskopischen Gyn-OP bei uns zum kardialen OP-Risiko beurteilt werden. Auffällig in der OP-Vorbereitung ein abgerissener
Koronardraht, der bei einer Stentintervention (soweit erkennbar Bifurkationsstenting der LAD) wohl mit eingestentet und beim Extraktionsversuch in Höhe von BWK 11 abgerissen war und dort belassen wurde. Die Patientin war die letzten 11 Jahre unter ASS ohne weitere Probleme. Aktuell werden nach Aussage der Tochter auch keine kardialen Beschwerden angegeben, die
Belastbarkeit sei gut. Es wurde eine Durchleuchtung von xxxeingelesen, in der der Koronardraht gut erkennbar ist. In einer aktuell RöTh-Aufnahme stellt sich der Draht nicht dar.
Eine Risikoeinschätzung kann mangels Literatur- oder Studiendaten nur eine persönliche Einschätzung sein. Von dem Koronardraht geht im Wesentlichen nur ein thrombogenes Risiko aus, es empfiehlt sich daher eine dauerhafte Plättchenhemmung mit ASS, ggfs in Kombination mit Plavix, falls kein erhöhtes Blutungsrisiko besteht.
Der Eingriff ist unter Beibehaltung von ASS (ohne Plavix) zu empfehlen, dies vornehmlich aber durch das Vorhandenseins eines Koronarstents, zudem ist eine präoperative, aus kardiologischer Sicht ohnehin eine dauerhafte Betalbocker-Medikation zu empfehlen. Plavix kann nach chirurgischer Einschätzung im Intervall begonnen werden. Ob diese Comedikation nach 11 komplikationsfreien Jahren unter ASS wirklich notwendig ist, ist fraglich, würde ich bei guter Verträglichkeit aber so empfehlen.
Eine zusätzliche plasmatische Gerinnungshemmung erhöht sicher nur das Risiko des operativen Eingriffs und ist bzgl. Stent und Draht ohne zusätzlichen Benefit.
Labor:
Bei Aufnahme: Leukozyten: 4,8 G/l, CRP: 0,03mg/dl, Hb 12,2 g/dl, Thrombozyten,
Kalium, Kreatinin, MDRD, TSH, fT3 und fT4, Quick, PTT und Urinstix im Normbereich.
Im Verlauf: Hb 9,2 g/dl, Calcium: 1,98 mmol/l. Leukozyten: 6,3 G/l, Harnstoff, Kreatinin, MDRD, Glucose, Natrium, Kalium, GOT, Gamma GT, im Normbereich.
Bei Entlassung: Hb 10,4 g/dl, Leukozyten: 6,6 G/l.
Mikrobiologie: Kultur Abstrich Vagina (xxx): viele Lactobacillus spezies, massenhaft Candida glabrata, sensibel auf Amphotericin B oder Fluconazol oder Flucytosine oder Voriconazole.
Stuhlkultur: Candida spezies (non albicans): . Kein Nachweis von Salmonellen, Shigellen, Yersinien, Campylobacter. Keine coliforme Flora. Kultur Abstrich Vagina xxx): viele Candida glabrata, viel Staphylococcus aureus, mehrere Eschericha coli, mehrere Enterococcus sp., vereinzelt Proteus mirabilis. Alle 4 Bakterientypen sensibel auf Amoxicillin/Clavulansäure oder Ofloxacin oder Imipenem.
Verlauf:
Die Patientin stellte sich mit akuten starken linksseitigen Unterbauchschmerzen vor. Im linken Unterbauch sei schon länger eine Ovarialzyste bekannt, welche laut Aussagen der Patientin bei der letzten Untersuchung in den USA durch eine suspekte Wandverdickung aufgefallen war. Auch klagte sie über einen fortgesetzten Pruritus im Bereich der Vulva/Vagina trotz vorangegangener Behandlung mit Östrogenen und auch Antimykotika. Bei der gynäkologischen
Untersuchung fand sich außerdem eine ausgeprägte Rektozele.
Zur Abklärung einer eventuell ursächlichen Stiehldrehung für die plötzlichen Schmerzen wurde der oben genannte Eingriff zügig nach kardialer Abklärung durchgeführt. Dieser und der postoperative Verlauf gestalteten sich komplikationslos. Bei der Abschlussuntersuchung zeigten sich reizlose Wundverhältnisse, die Nieren waren beidseits nicht gestaut.
Wir konnten xxx in deutlich gebessertem Allgemeinbefinden aus unserer Klinik entlassen. Verhaltensempfehlungen wurden ihr mitgeteilt.
Medikation:
Amlodipin 2,5mg 0-0-1, Metoprolol 25 1-0-0, ASS 100 0-1-0, Simvastatin 20 0-0-1,
L-Thyroxin 75µg 1-0-0, ASS 100mg 1-0-0
Therapieempfehlung:
Die Patientin stellte sich zur Wundkontrolle und Befundbesprechung am xxx
vor. Es fanden sich reizlose Wundverhältnisse, der Damm noch in Heilung. Eine Therapie mit Gynoflor und Ovestin (Östriolsalbe) wurde empfohlen und im Bereich des äußeren Genitale die Bepanthenanwendung. Nach abgeschlossener antimykotischer orale Therapie der intestinalen und vaginalen Mykose mit Ampho Moronal (=Amphotericin B) 100mg 4 x täglich für 12 Tage und anschließen für 5 Tage Fluconazol (1x 150mg, dann 4 Tage je 1x 50mg) sowie einer Unterstützung des Wiederaufbaus der Darmflora mit Perenterol forte, wurde nochmal ein
Kontrollabstrich aus der Vagina entnommen. Da dieser neben einer noch bestehenden Candida glabrata-Infektion nun auch noch eine bunte bakterielle Mischflora zeigte, empfehlen wir die orale antibiogrammgerechte Behandlung mit Amoxiclav für 10 Tage und anschließend eine Lokaltherapie mit Miconazol für 7 Tage. Eine erneute Abstrichkontrolle nach Beendigung der Therapie, sowie eine Kontrolle der Leber- und Nierenwerte möchten wir außerdem empfehlen.

Diagnoses: severe lower abdominal pain with cystadenoma of the left ovary and significant adhesions, rectocele, Candida glabrata infection (intestinal and vulva-vaginal), vaginal mixed infection.
Secondary diagnoses: S/P abdominal hysterectomy in xxx S/P VAIN III of the proximal vagina,
S/P disc prolaps L4/L5 in xxx, CAD, S/P stent implantation to the LAD with accidental stenting over the guide wire and consecutive tear in xxx.
Operation and Interventions:
xxx: colposcopy with vinegar test: negative. xxx open surgical laparoscopy: lavage cytology, laparoscopic bilateral adenectomy, lysis of adhesions, plastic of the posterior vaginal wall, biopsies of labia bilaterally
Histology:
Right ovary: atrophied with a small simple cyst and multiple corpora albicantia. Fibrosis of the tubal mucosa, multiple hydatides of the tube. Left ovary: atrophied with papillary serous cyst adenoma without proliferation tendency, multiple corpora albicantia. Fibrosis of the tubal mucosa. Biopsy of the labia bilaterally: one fibro-epithelial polyp each with inflammatory irritation, one with highly stacked, hinted papillary formed squamous epithelium coverage, the other covered with acanthoticly broadened squamous epithelium. No suggestion of malignancy.
Cytology:
No evidence of tumor cells in the material on hand, diagnostic grading: negative findings: Chest X-ray one view a/p: contrary to the outside films from xxx, there is evidence of the stent projected onto the LAD but not of the remaining guide wire. It is to be assumed that this wire was unable to be detected due to its small lumen rather than being dislodged. Additional radiology might be applicable according to cardiology. The heart is of normal size and configuration. The aorta is elongated with increased shadowing. Hila WNL for vessels. No mediastinal enlargement.
Lung aeration equal bilaterally. No effusion, no new infiltrates, no pulmonary congestion. Spondylosis deformans of the thoracic spine.
Internal medicine consultation:
Patient not seen in person, case has been recited to me by anesthesia and the patient’s daughter (physician in house). Patient is supposed to receive pre-op clearance from a cardiology standpoint prior to a laparoscopic gyn operation. In preparing for the operation the presence of a torn coronary wire is remarkable, which apparently was implanted during a stent intervention (as far as it is discernable, a bifurcation stenting of the LAD) and was torn at the level of T11 during an extraction attempt and left in that position. The patient has been on ASA for the past eleven years without any problems. According to the daughter, there are no coronary complaints reported at this time, endurance is said to be good. An X-ray from 2007 was viewed in which the coronary wire is easily distinguished. The wire is not recognizable on the current chest X-ray.
Due to a lack of literature or study data, the risk assessment can only be a personal one.
The coronary wire essentially poses just a thrombogenic risk; therefore, a long-term platelet inhibition with ASA, possibly in combination with Plavix, is recommended unless there is an increased risk of bleeding.
The procedure is recommended with the maintenance of ASA (without Plavix), this mainly because of the presence of the coronary stent, furthermore there is a pre-op, from a cardiology standpoint permanent, beta-blockade to be recommended. Plavix can be resumed later on according to the surgeon’s assessment. It is questionable if this co-medication is really necessary after eleven years without complications, but I would recommended if it can be tolerated well.
An additional plasmatic clotting inhibitor surely just increases the surgical risk and is without additional benefit in regard of the stent and wire.
Laboratory:
At admission: WBC: 4.8 g/l, CRP: 0.03 mg/dl, Hgb 12.2 g/dl, thrombocytes, potassium, creatinine, MDRD, TSH, free T3 and free T4, INR, PTT and UA within normal limits.
During the course: Hgb 9.2 g/dl, Calcium : 1.98 mmol/l, WBC 6.3 g/l, BUN, creatinine, MDRD, glucose, sodium, potassium, AST, gamma-GT within normal limits.
At discharge: Hgb 10.4 g/dl, WBC 6.6 g/l.
Microbiology: culture of vaginal swab (xxx): many lacto-bacilli species, massive Candida glabrata, sensitive to amphocetericin B or fluconazole or voriconazole.
Stool culture: candida species (non-albicans): . No evidence of salmonella, shigella, yersinia, campylobacter. No coliform flora. Culture of vaginal swab xxx): many Candida glabratra, many Staphylococcus aureus, several Eschericha coli, several enterococcus speci, few Proteus mirabilis. All four bacteria types sensitive to amoxicillin/clavulanic acid or ofloxacin or imipenem.
Course:
The patient presented with severe left lower abdominal pain. The patient reports the known presence of an ovarian cyst in the left lower abdomen which, according to the patient, was noticed because of a suspected wall thickening during the last examination in the U.S. She also complained about a constant pruritus in the vulva-vaginal area despite previous treatment with estrogen as well as anti-mycotica. During the gynecological exam a significant rectocele was also discovered.
After the cardiology clearance the above-mentioned procedure was proceeded with promptly to rule out a sudden twisting of the salpinx. This and the post-operative course were without complications. There were no signs of wound inflammation during the discharge exam, no signs of hydronephrosis.
We were able to discharge xxx from the hospital on xxx with significantly improved general well-being. Discharge instructions were given to the patient.
Medication:
amlodipine 2.5 mg daily in the pm, metoprolol 25 mg daily in the am, ASA 100 mg daily at noon, simvastatin 20 mg daily in the pm, levothyroxine 75 mcg daily in the am, ASA 100 mg daily in the am.
Therapy recommendation:
Patient presented on xxx for wound follow-up and discussion of findings. The wound was non-inflammatory, perineum remains in the process of healing. Therapy with Gynoflor (combination of estriol and lactobacillus acidophilis) and Ovestin (estrogen ointment) was recommended as well as use of panthenol in the area of the outer genitals. After completed anti-mycotic oral therapy of the intestinal and vaginal mycosis with amphotericin B 100 mg QID for twelve days, followed by five days of fluconazole (one time dose of 150 mg, followed by 50 mg daily times four days) as well as reconstitution of the intestinal flora with saccharomyces cerevisiae, an additional control swab was obtained from the vagina. Since this showed in addition to the existing Candida glabrata infection now also a variety of bacterial mixed flora, we are recommending an oral antibiogram-specific treatment with amoxicillin/clavulanate for ten days, followed with a local therapy with miconazole for seven days. We would also like to recommend an additional control swab after the completion of therapy as well as a control of liver and kidney function.

ln einer prospektiven Studie verglichen drei unabhiingige
Untersucher den diagnostischen Stellenwert
von Continuous-Wave(CW)-Doppler-Sonographie,
Duplex-Scan und selektiver Angiographie für die Diagnostik
extrakranieller GefaRstenosen. Die Analyse
von 488 angiographierten Arterien (A. carotis communis,
interna, externa) bei 89 Patienten ergab für hamodynamisch
wirksame Stenosen (> 50%) und Verschlüsse
eine deutlich hohere TrefferquoL::: der Doppler-
Sonographie aIs des Duplex-Scan. Stenosen unter
50% und arteriosklerotische Plaques, die mit der
Doppler-Sonographie nicht zu erfassen sind, wurden
in der HalEte der Hile mit dem Duplex-Scan richtig
erkannt. Mit Hilfe des Duplex-Scan konnen gelegentlich
zweifelsfrei kleine arteriosklerotische Plaques
nachgewiesen werden, die der Angiographie entgehen.
Die beiden atraumatischen Methoden erganzen sich
sinnvoll, da der Duplex-Scan Plaques und Stenosen
identifiziert, die der Doppler-Sonographie entgehen,
letztere Methode aber auch Stenosen im vertebrobasiliiren
System und in der intrakraniellen Carotisstrombahn
nachweisen kann.
Hiimodynamisch wirksame Stenosen mit einer Querschnittseinengung
über 50% und Verschlüsse der hirnversorgenden
Arterien konnen atraumatisch mit Hilfe
der Continuous-Wave(CW)-Doppler-Sonographie mit
hoher Sicherheit (Sensitivitiit 95%) erkannt werden (5, 8,
9). Seit Einführung hochauflosender bildgebender Ultraschallverfahren
(B-Scan bzw. Duplex-Scan) kbnnen auch
arteriosklerotische GefiiRveriinderungen mit einer Querschnittseinengung
unter 50% atraumatisch identifiziert
werden.
Bei der CW-Doppler-Sonographie werden kontinuierlich
Ultraschallsignale von der Doppler-Sonde ausgesandt
und die von den flieRenden korpuskuliiren Anteilen
im Blut reflektierten Signale aufgefangen. Die Identifizierung
von Stenosen erfolgt über die akustischen Phiinomene,
wie Strbmungsbeschleunigung und Turbulenzen,
und indirekt über Veranderungen der diastolischen und
systolischen Stromungsgeschwindigkeit in GefiiRab-

Diagnosis of stenoses and occlusions of extracranial
arteries. Comparison of Doppler sonography, duplex
scan, and cerebral angiography
In a prospective study the detection rate of stenoses
and occlusions of extracranial arteries by continuouswave
(CW) Doppler sonography, duplex-scan and
selective angiography was compared independently by
three investigators. In 89 patients (488 arteries) CWDoppler
sonography was superior to duplex-scan in
the detection of stenoses exceeding 50% and occlusions.
Duplex-scan detected about half of Jow degree
stenoses and arteriosclerotic plaques and in some cases
lesions which are not shown in angiography. Thus,
Doppler sonography and duplex-scan are complementary
rather than competitive methods. Doppler sonography
is necessary for the evaluation of flow disturbances
in the vertebral and subclavian arteries and high
degree stenoses in the intracranial section of the internai
carotid artery.

Beim B-Scan wird ein zweidimensionales
Schnittbild erzeugt, wobei das Auflosungsvermbgen
von der Ultraschallfrequenz abhiingt (3-10 MHz).
Der Duplex-Scan vereint beide Prinzipien und kombiniert
das B-Bild mit einem gepulsten Doppler. So kann
das Megvolumen des Dopplers optimal in das GefaRvolumen
plaziert werden. Bei Duplex-Systemen mit
schlechter optischer Auflbsung (3-5 MHz) wird das BBild
nur zur Plazierung des DopplermeRvolumens
benutzt. Die ldentifizierung von Stenosen erfolgt dann
über die Frequenzanalyse der Doppler-Signale. Hochauflosende
Duplex-Scanner (7,5-10 MHz) erlauben eine
visuelle Identifizierung arteriosklerotischer GefaRwandveriinderungen
und im Gegensatz zum reinen B-Scan eine
sichere Unterscheidung der GefiiRe (zum Beispiellnterna
und Externa) über das integrierte Doppler-Signal. Die
ausschlieRliche Verwendung der Frequenzanalyse zur
Diagnostik von Carotisstenosen hat sich bei uns nicht
bewiihrt, da die Methode zu viele falsch-positive Befunde
ergibt und viel zu zeitaufwendig ist (45 Minuten pro
Patient).

Die in den letzten vier Jahren mitgeteilten Ergebnisse
der neuen bildgebenden Methoden schienen erfolgversprechend;
im Vergleich zur arteriellen Angiographie
wurden Trefferquoten für unter SO%ige Stenosen und
rur arteriosklerotische Plaques zwischen 70 und 100%,
rur hôhergradige Stenosen zwischen 40 und 100% angegeben
(1,2, 12, 14, lS, 17-23).
In einer prospektiven Studie überprüften wir mit drei
Untersuchern den diagnostischen Stellenwert der CWDoppler-
Sonographie und des Duplex-Scan im Vergleich
zur arteriellen Angiographie, da uns in Anbetracht der
methodischen Probleme der bildgebenden Ultraschallverfahren
und der in der Literatur beschriebenen Differenzen
der Treffsicherheit der Angiographie, verglichen mit
dem Operationsbefund, die bisher mitgeteilten Ergebnisse
aIs zu optimistisch erschienen.
Da die Investitionskosten fur einen Duplex-Scan etwa
10- bis lSmal hôher sind ais fur ein Doppler-Geriit,
wollten wir in einer zweiten Untersuchung beide Ultraschallmethoden
direkt vergleichen, um zu kliiren, ob die
neue Methode tatsiichlich mehr klinisch bedeutsame
Informationen bringt. Ais Referenzmethode dient hier
die Doppler-Sonographie, die in zahlreichen Vonintersuchungen
im Vergleich zur Angiographie validiert wurde
(S, 8, 9, 13, 16).
Patienten und Methodik
Von Januar bis August 1983 untersuchten wir bei 89 Patienten, die
wegen einer geplanten Operation an der A. carotis interna angiographiert
wurden, insgesamt 488 Arterien (A. carotis communis,
interna und externa). Bei einigen Patienten mit Arterienverschlüssen
im Beckenbereich und einer Carotisstenose rechts beschriinkten
wir uns auf eine retrograde Brachialisangiographie rechts. 25
Patienten waren asymptomatisch, 42 hatten transitorisch-ischamische
Attacken erlitten. Bei 22 Patienten bestand ein Zustand nach
Insult mit minimaler Restsymptomatik ohne nachweisbaren ischamischen
Bezirk im Computertomogramm.
CW-Doppler-Sonographie, Duplex-Scan und Angiographie wurden
von drei verschiedenen Untersuchern unabhangig durchgeführt
und ohne Kenntnis der Ergebnisse der jeweils anderen Untersucher
ausgewertet.
In einer zweiten Studie verglichen wir die Ergebnisse der CWDoppler-
Sonographie und des Duplex-Scans an weiteren 672
Patienten. Beide Verfahren wurden wiederum unabhangig von zwei
Untersuchern durchgeführt. Referenzmethode ist hier die CWDoppler-
Sonographie.
Für die direktionale CW-Doppler-Sonographie benutzten wir ein
Gerat der Firma Delalande mit einer Ultraschallfrequenz von 4
MHz. Untersuchungstechnik und Beurteilung des AusmaBes der
Stenose erfolgten wie in der Literatur (5) beschrieben.
Die Duplex-Untersuchung erflgte mit dem Small-parts-Doppler-
Zusatz zum Sonographiegerat RA 1 von Siemens. Das B-Bild
wird über einen 7,5-MHz-Schallkopf aufgenommen, die Bildfolge
betragt 201s. Das Auf!osungsvermogen liegr axial bei 0,4 mm und
lateral bei 0,9-1 mm. Das integrierte gepulste Doppler-System
arbeitet mit 3 MHz. Das gepulste Doppler-MeBvolumen hat axial
eine Ausdehnung von 3 mm und lateral von 2 mm. lm Falle des
kombinierten Betriebes betragt die Bildfolge des B-Bildes 4/s.
Pathologische B-Bildbefunde wurden durch Polaroidbilder oder
durch ein Videosystem dokumentiert. Der Schallkopf wurde
zunachst kaudal auf die A. carotis cornmunis aufgesetzt und langsam
bis zur Bifurkation vorgeschoben. Die ldentifikation von A.
carotis interna und externa erfolgte über die Doppler-Signale akustisch
nach Druckversuch (5). Die simultan abzuleitende Spektralanalyse
setzten wir nicht ein, da bei hohergradigen Stenosen die
Wandbewegungen der Arterien das Doppler-Signal stark storten.
Der Zeitaufwand für eine kornplette Duplexuntersuchung, einschliemich
Spektralanalyse, betragt 45-60 Minuten und ist in der
GefaBambulanz nicht tragbar. Für die Doppler-Sonographie benotigen
wir 10 Minuten, für die reduzierte Duplex-Methode (nur BBild
und ldentifikation der Arterien mit Doppler) 10-15 Minuten.
Durch kontinuierliches Kippen des Schallkopfes konnten Sagittalschnitte
in mehreren Ebenen dargestellt werden. lm Gegensatz zur
angiographischen Darstellung ist es bei über 90% der Personen
nicht moglich, die Aa. carotis interna und externa simultan darzustellen,
da dies voraussetzen würde, daB die GefaBe in der Beschallungsebene
hintereinander liegen. AnschlieBend wurden die Halsarterien
von kaudal nach kranial in transversaler Richtung dargestellt.
Das AusmaB einer Stenose wurde sowohl transversal ais auch
sagittal durch die integrierte Distanzrnessung bestimmt.
Die Angiographie wurde irn allgemeinen über einen transfemoralen
Zugang durchgeführt. Der Neuroradiologe beurteilte aus den
Angiographien in zwei Ebenen das AusmaR der Stenosen und der
nicht lumeneinengenden Wandveranderungen.
Ergebnisse
Vergleich von CW-Doppler-Sonographie, Duplex-Scan
und Angiographie. Die angiographischen Befunde wurden
in Normalbefunde, arteriosklerotische Plaques bzw.
Stenosen unter SO%, Stenosen über 50% und Verschlüsse
aufgegliedert (Tabelle la). Der Stenosegrad
wurde in Prozent geschiitzt.
Befunde im Doppler-Sonogramm und Duplex-Scan
wurden nur aIs übereinstimmend anerkannt, wenn Verschlüsse
erkannt wurden, Stenosen um nicht mehr aIs
20% yom angiographischen AusmalS abwichen und bei
Vorliegen mehrerer Plaques im selben GefiiB aIle Plaques
erkannt wurden.
Die Angiographie war bei 228 der untersuchten Arterien
normal. Falsch-positive Befunde ergaben sich bei
3% der Doppler-Sonogramme und bei Il% der Untersuchungen
mit dem Duplex-Scan. Ais falsch-positiv wur-

den auch Duplexbefunde gewertet, die bei normalem
Angiogramm eindeutig arteriosklerotische Plaques mit
Dichteanhebung und Schallschatten darstellten. Von 14S
arteriosklerotischen Plaques und Stenosen unter SO%
wurden mit dem Duplex-Scan SO% richtig erkannt. Die
Doppler-Sonographie eignet sich aus physikalischen
Gründen nicht zum Nachweis dieser hiimodynamisch
nicht-wirksamen Stenosen; nur 13% wurden aufgrund
von Turbulenzen richtig erfaRt. Bei den über SO%igen
Stenosen und Verschlüssen lag die Trefferquote der
Doppler-Sonographie bei 91 bzw. 96%, die des DuplexScan
bei 85% bzw. 87%.
Die Trefferquote der Doppler-Sonographie insgesamt,
allerdings unter EinschluR der physikalisch nicht erfaRbaren
unter SO%igen Stenosen, lag bei 48%, die des
Duplex-Scan bei 66%. Bei Kombination beider Methoden
konnten 72% aller Plaques, Stenosen und Verschlüsse
identifiziert werden.
Vel'gleich zwischen CW-Dopplel'-Sonogl'aphie und
Duplex-Scan. Diese Untersuchung führten wir bei 672
Patienten im Rahmen der Routineambulanz durch,
unabhiingig von der Art der Erkrankung (ScreeningUntersuchung
bei Patienten mit Risikofaktoren, GefiiRerkrankungen,
Tumoren, unspezifischen Symptomen wie
Schwindel und Kopfschmerz).
Wir verglichen die Befunde von insgesamt 4028 Arterien.
Doppler-sonographisch fan den wir 449 Stenosen
und Verschlüsse im Bereich der Carotiden. Die Stenosen
unterteilten wir in SO- bis 80%ige sowie hochgradige
Stenosen (> 80%).
Wie Tabelle lb zeigt, bestehen mit dem Duplex-Scan
besondere Schwierigkeiten bei der Erkennung hochgradiger
Stenosen. Eine Untersuchung mit dieser Methode
allein ist nicht ausreichend. Mit der Doppler-Sonographie
fanden wir zusiitzlich 166 pathologische Veriinderungen
im Vertebralis- bzw. Subclavia-Stromgebiet (Vertebralisstenose
bzw. -verschluR, Subclaviastenose bzw.
-verschluR, Subclavian-steal-Syndrom) und auRerdem 28

intrakranielle Stenosen im Interna-Versorgungsgebiet.
Alle diese für das therapeutische Procedere wichtigen
Zusatzbefunde (13) entgehen dem Duplex-Scan. Es ist
gelegentlich bei Patienten mit schlankem Hals méiglich,
kurze Abschnitte der A. vertebralis im Duplex-Scan darZllstellen
und das gepulste Doppler-MeRvolumen zu plazieren.
Der dazu notwendige Zeitaufwand ist, gemessen
am konventionellen Doppler, unverhiiltnismiiRig hoch.
Andererseits konnten wir mit Hilfe des Duplex-Scan 431
unter SO%ige Stenosen oder arteriosklerotische Plaques
entdecken, die sich der Doppler-sonographischen Diagnostik
entzogen.
Diskussion
Unsere Ergebnisse zeigen, daR es sich bei CW-DopplerSonographie
und Duplex-Scan nicht um .konkurrierende
Methoden handelt, sondern daR sich beide Verfahren
sinnvoll ergiinzen. Über SO%ige Stenosen und Verschlüsse
der Carotiden konnten in Übereinstimmung mit
der Literatur (S, 8, 13, 16) mit Hilfe der CW-DopplerSonographie
mit 94%iger Sicherheit diagnostiziert
werden.
Unsere Trefferquoten mit dem Duplex-Scan stimmen
für Stenosen über SO% und Verschlüsse mit den bisher
mitgeteilten Ergebnissen für B-Scan und Duplex-Scan
weitgehend überein (1,2, 12, 14, IS, 17-23). Die Tref-

ferquote für Stenosen unter 50% und arteriosklerotische
Plaques ist aber im Vergleich mit den bisher publizierten
studien deutlich niedriger. Das ist wohl in erster Linie
unserer Untersuchungsmethode mit drei vallig unabhiingigen
Untersuchern und einer auch arteriosklerotische
Plaques beschreibenden Angiographie-Auswertung zuzuschreiben.
Auf die methodischen Probleme des B-Scan wurde
bereits in früheren Arbeiten eingegangen (1, 18, 19). Bei
hochsitzender Carotisbifurkation in der Niihe des Kieferwinkels
sind oberhalb des Interna-Abganges gelegene
GefiifSveriinderungen nicht mehr darstellbar (Abbildung
1). Kalkdichte Plaques in der GefafSvorderwand lassen
durch den Schallschatten die GefiifSrückwand nicht zur
Darstellung kommen; der stenosegrad kann dann nicht
beurteilt werden. Gerinnungsthromben und lipidreiche
Plaques haben eine iihnliche schalldichte wie Blut und
lassen sich entweder nur schlecht oder überhaupt nicht
darstellen. Hachstgradige Stenosen (> 95%) unterschreiten
das Auflasungsvermagen des B-Scan. Hier ist auch
der gepulste Doppler des Duplex-Scan wenig hilfreich, da
es extrem schwierig ist, das MefSvolumen in das verbleibende
Restlumen einzubringen. Verschlüsse aus thrombotischem
Material haben eine iihnliche Schalldichre wie
Blut und entgehen daher der Diagnostik mit dem B-Scan.
Hier ist auch der Nachweis des Fehlens von Pulsationen
kein sicheres Kriterium, da diese oft durch eine verstiirkt
pulsierende A. carotis externa vorgetiiuscht werden (19).
Durch konsequenten Einsatz des integrierten gepulsten
Dopplers kannen aber auch Verschlüsse mit hoher
sicherheit erkannt werden.
Ais Referenzmethode zur Beurteilung der diagnostischen
AussagekraEt des Duplex-Scan wiihlten wir wie in
den bisher vorliegenden Studien die arterielle Angiographie.
lm Unterschied zur Angiographie handelt es sich
aber bei dem bildgebenden Ultraschallverfahren um eine
dynamische Untersuchung, bei der auch Wandbewegungen
und damit die Elastizitiit der GefiilSwiinde beurteilt
werden kannen. AufSerdem ist auch eine Darstellung des
GefiiiSquerschnitts maglich, wodurch asymmetrische Stenosen
besser erfafSt werden kannen.
Die Verwendung der Angiographie aIs Referenzmethode
ist nicht ohne Probleme. Beim Vergleich des
Angiographie- und Operationsbefundes wurden besonders
bei kleinen und ulzerierten Plaques erhebliche Differenzen
gefunden (3, 7, 10, 11, 18). Die angiographische
Einschiitzung des stenosegrades, vor allem bei kleinen
stenosen, ist ebenfalls gelegentlich falsch (1, 4). Die
angiographische Nachweisbarkeit kleiner ulzerierter
Liisionen ist wahrscheinlich durch die Darstellung der
Carotisgabel in mehr ais zwei Ebenen zu verbessern (6).
Unsere studie zeigt in Übereinstimmung mit unserer
Alltagserfahrung, daiS der Duplex-Scan durchaus arteriosklerotische
Plaques entdecken kann, die der Angiographie
entgehen (Abbildung 2). In der Praxis bedeutet dies,
dafS sich hier beide Verfahren sinnvoll erganzen.
In der klinischen Routine ist der Duplex-Scan vor
allem bei transitorisch-ischiimischen Attacken und normalem
Doppler-Befund hilfreich, da wir so hiiufig eine
streuquelle in den Carotiden identifizieren kannen.
Andererseits sind wir mit· der Indikationsstellung zur
Operation jetzteher zurückhaltend, wenn der Nachweis
multipler arteriosklerotischer Plaques in Communis, Bulbus
und Interna gelingt.
Auch nach zweijiihriger Erfahrung und über 2000
Untersuchungen mit dem Duplex-Scan liegt unsere Trefferquote
für Stenosen unter 50% und für arteriosklerotische
Plaques im Vergleich zur arteriellen Angiographie
deutlich niedriger ais in der Literatur angegeben. Wir
führten unsere Untersuchung srreng blind und ohne
Kenntnis der jeweils anderen Ergebnisse durch und legten

Unsere Erfahrungen in der kombinierten Anwendung

ist, die CW-poppler-Sonographie ais alleinige Untersuchungsmethode
einzusetzen, wenn man sich der Einschrankung

alleinige Anwendung des Duplex-Scan ohne zusatzliche

Summary
In a prospective study, the detection rate of stenoses and occlusions of extracranial arteries by continuous wave (CW) Doppler, duplex scan, and selective angiography was compared independently by three investigators. In an analysis of 488 angiographically examined arteries (common carotid artery, internal and external carotid arteries) in 89 patients, CW-Doppler was superior to duplex scan in the detection of hemodynamically significant stenoses (> 50%) and occlusions. Duplex scan detected about half of low degree stenoses (< 50%) and arteriosclerotic plaques and in some cases lesions not shown in angiography. Thus, Doppler sonography and duplex scan are complementary rather than competitive methods. Doppler sonography is necessary for the evaluation of flow disturbances in the vertebral and subclavian arteries and for the evaluation of high degree stenoses in the intracranial section of the internal carotid artery.
Hemodynamically significant stenoses with a narrowing of the lumen by more than 50% and occlusions of the arteries supplying the brain can be detected with high reliability (sensitivity 95%) via non-traumatic CW-Doppler sonography [5, 8, 9]. Arteriosclerotic vessel changes with a narrowing of the lumen by less than 50% can also be detected in a non-traumatic fashion since the introduction of high-resolution, picture-supplying ultrasound methods (B-scan or duplex scan).
With CW-Doppler sonography, the Doppler probe sends out continuous ultrasound signals and receives signals reflected from the flowing corpuscular particles in the blood. Identification of stenoses results from the acoustic phenomena like increases in velocity and turbulences as well as indirectly from changes in the diastolic and systolic velocity in vessel segments below and above the stenosis noted in the pulse curve recording. The B-scan produces a two-dimensional sectional image, whereby the resolution ability depends on the ultrasound frequency (3-10 MHz). The Duplex scan unites both principles and combines the B-image with a pulsed Doppler. Thus, the sample volume of the Doppler can be placed optimally into the vessel volume. In duplex systems with poor optical resolution (3-5 MHz), the B-image is used just for the placement of the Doppler sample volume, and the identification of stenoses occurs via the frequency analysis of the Doppler signals. Duplex scanners with a high resolution (7.5-10 MHz) allow for a visual identification of arteriosclerotic changes in the vessel wall and, contrary to the sole B-scan, for a certain differentiation of the vessels (such as internal and external carotid artery) with the integrated Doppler signal. The sole use of frequency analysis for the diagnosis of carotid stenoses has not worked for us since that method results in too many false-positive findings. Moreover, it is too time-consuming (45 minutes per patient).
The results for the new image-producing methods, published over the past four years, seem to be promising; compared to the arterial angiography, stenoses < 50% and arteriosclerotic plaques were correctly identified in 70 to 100% of the cases and between 40 and 100% of the cases with higher-grade stenoses [1, 2, 12, 14, 15, 17-23].
In a prospective study, we reassessed with three investigators the diagnostic significance of CW-Doppler sonography and duplex scan as compared to arterial angiography since we viewed the results published to date to be too optimistic, considering the methodical problems of image-producing ultrasound procedure and differences in accuracy of angiography described in the literature when compared to the findings during surgery.
Since the investment cost for a duplex scan is about 10 to 15 times higher than it is for a Doppler device, our aim was to compare both ultrasound methods directly in a second study to clarify whether the new method really supplies clinically more significant information. Doppler sonography serves as a reference method in this case since it had been validated in many previous exams in comparison to angiography [5, 8, 9, 13, 16].
Patients and methods
We examined 488 arteries (common carotid artery, internal and external carotid arteries) in 89 patients undergoing angiography prior to a planned internal carotid artery surgery from January to August of 1983. In some patients with arterial occlusions in the pelvic area and a right carotid stenosis, we limited ourselves to a retrograde right brachial artery angiography. There were 25 asymptomatic patients, 42 patients had suffered a transient ischemic attack, and 22 patients were status post insult with minimal residual symptoms without discernable ischemic area on computed tomography.
CW-Doppler sonography, duplex scan, and angiography were performed independently by three different investigators and interpreted without knowing the findings of the other investigators, respectively.
In a second study, we compared the results of CW-Doppler sonography with the results of duplex scans in an additional 672 patients. Again, two investigators performed both procedures independently. Reference method in this study is the CW-Doppler sonography.
We used a device by the Delalande company with an ultrasound frequency of 4 MHz for the directional CW-Doppler sonography. Technique of the exam and assessment of the degree of the stenosis were performed as described in the literature [5].
The duplex scan was performed with a small-parts Doppler addition to the sonographic device RA 1 by Siemens. The B-image is received via a 7.5 MHz ultrasonic probe; the picture sequence is 20/s. The resolution capability is axial at 0.4 mm and lateral at 0.9 to 1 mm. The integrated pulsed Doppler system performs at 3 MHz. The pulsed sample volume has an axial width of 3 mm and lateral of 2 mm. in case of the combined use, the image sequence of the B-image is 4/s. Pathological B-image findings are documented with Polaroid images or with a video system. At first, the ultrasonic probe was placed caudally on the common carotid artery and slowly moved towards the bifurcation. Identification of the internal and external carotid arteries occurred acoustically with Doppler signals after compression attempt [5]. We did not use the spectral analysis to be recorded simultaneously since the wall movements of the arteries disturb the Doppler signal greatly in high-grade stenoses. The required time for a complete duplex study, including spectral analysis, is about 45 to 60 minutes, which is not reasonable for the vascular clinic. We needed 10 minutes for the Doppler sonography and for the reduced duplex method (just B-image and identification of the arteries by Doppler) 10 to 15 minutes. Sagittal cuts in different planes could be obtained with continuous tilting of the ultrasonic probe. Contrary to angiographic imaging, it is not possible to show the internal and external carotid artery simultaneously in more than 90% of people since this would require that the vessels would be behind each other in the insonation. Then the arteries in the neck were depicted from caudal to cranial in transverse direction. The extent of a stenosis was determined transversally as well as sagittally with integrated measuring of distance.
In general, angiography was performed via a trans-femoral access. The neuroradiologist evaluated the extent of the stenoses and non-narrowing changes in the vessel walls by utilizing the results of the angiography in two planes.
Results
Comparison of CW-Doppler sonography, duplex scan, and angiography. The angiographic findings were divided into normal findings, arteriosclerotic plaques and stenoses < 50%, stenoses > 50%, and occlusions (table 1a). The degree of stenosis was estimated in percent.

Findings in Doppler sonogram and duplex scan were only viewed as congruent if occlusions were detected, stenoses did not differ from the angiographic extent by more than 20%, and, in case of several plaques in the same vessel, all plaques were recognized.
Angiography was normal in 228 of the examined arteries. False-positive findings resulted in 3% of the Doppler sonograms and in 11% of the duplex scans. In addition, those duplex findings were considered false positive that showed obvious arteriosclerotic plaques with density and insonation shadow in the presence of a normal angiogram. With the duplex scan, 50% of the 145 arteriosclerotic plaques and stenoses below 50% could be identified correctly. Doppler sonography is for sole physical reasons not suitable for the detection of these hemodynamically insignificant stenoses; a mere 13% were detected correctly due to turbulences. For stenoses greater than 50% and for occlusions, accuracy of Doppler sonography was 91% and 96%, respectively, and 85% and 87% for duplex scan.
Accuracy of Doppler sonography altogether, including the physically not detectable stenoses < 50%, was 48%, accuracy of the duplex scan 66%. With a combination of both methods, 72% of all plaques, stenoses, and occlusions could be identified.
Comparison of CW-Doppler sonography and duplex scan. We performed this study in 672 patients within the framework of our routine clinic, regardless of the kind of indication(screening for patients with risk factors, vascular disease, tumors, non-specific symptoms like vertigo or headache).
We compared the findings of altogether 4028 arteries. Doppler-sonographically, we detected 449 stenoses and occlusions in the area of the carotids. We divided the stenoses into 50 to 80% stenosis as well as high-grade stenoses (>80%).
As table 1b demonstrates, it is especially difficult to detect high-grade stenoses with the duplex scan. An exam with this method alone is not sufficient. We found an additional 166 pathological changes in the vertebral and subclavian artery (vertebral stenosis or occlusion, subclavian stenosis or occlusion, subclavian steal syndrome) and 28 intracranial stenoses in the area supplied by the internal carotid artery. All these additional findings [13], important for treatment considerations, escape the duplex scan. Occasionally it is possible in people with a slender neck to outline short segments of the vertebral artery and place the pulsed Doppler sample volume with a duplex scan. This is unreasonably time-consuming when compared to the conventional Doppler. On the other hand, we were able to detect 431 stenoses less than 50% with the duplex scan, which escaped the Doppler-sonographic diagnostics.

Discussion
Our results show that CW-Doppler sonography and duplex scan are not competing methods but rather complement each other. Stenoses greater than 50% and occlusions of the carotids can be diagnosed with the utilization of CW-Doppler with 94% certainty, which is in agreement with the literature [5, 8, 13, 16].
Our accuracy of the duplex scan for stenoses greater than 50% and occlusions is mostly congruent with published results for B-scan and duplex scan to date [1. 2, 12, 14, 15, 17-23]. Accuracy for stenoses less than 50% and arteriosclerotic plaques, on the other hand, is significantly lower than in published studies so far. This might be explained foremost with our assessment method using three completely independent investigators and an angiography interpretation that also includes arteriosclerotic plaque.
Existing studies have already addressed the methodical problems related to the B-scan [1, 18, 19]. With a relatively high bifurcation of the carotids close to the jaw angle, vessel changes above the area where the internal carotid artery arises from common carotid artery cannot be depicted (figure 1). Plaques dense with calcium in the anterior wall of the vessel prohibit the visualization of the posterior wall due to the insonation shadow; the degree of stenosis cannot be determined. Thrombi and lipid-rich plaques have a similar insonation density as blood has and can be portrayed poorly or not at all. Highest-grade stenoses (>95%) are below the resolution capacity of the B-scan. The pulsed Doppler of the duplex scan is also of little benefit in that case since it is extremely difficult to apply the sample volume into the remaining lumen. Occlusions with thrombotic material have a similar insonation density as blood and therefore escape diagnosis with the B-scan. Evidence of absent pulsation is not a reliable criterion either in this case since this can often be a false interpretation due to an external carotid with increased pulsation [19]. However, occlusions can be detected with great reliability if the integrated pulsed Doppler is used consistently.

We chose arterial angiography as a reference method for the evaluation of diagnostic significance of the duplex scan, just as we did for the other studies. Contrary to angiography, the image-providing ultrasound method is a dynamic exam in which wall motion and therefore elasticity of the vessel wall may be evaluated. A portrayal of a cross section of the vessel is also possible, aiding in the detection of asymmetrical stenoses.
Using angiography as a reference method is not without problems. Significant differences were found, especially with small and ulcerating plaques, when comparing angiography and surgical findings [3, 7, 10, 11, 18]. Occasionally the angiographic assessment of the degree of stenosis is wrong, especially with small stenoses [1, 4]. Angiographic detection of small, ulcerating lesions could probably be improved with the visualization of the carotid bifurcation in in more than two planes [6].
Our study shows in agreement with the experience in our daily practice that the duplex scan can very well detect arteriosclerotic plaques missed by angiography (figure 2). In practice, this means that both methods complement each other in a meaningful way.
In the clinical routine, duplex scan is especially useful with transitory ischemic attacks and normal Doppler findings since often we are able to determine a source of showering in the carotids. On the other hand, we are now more conservative with indications for surgery when it is possible to detect arteriosclerotic plaques in the common and internal carotid artery and at the carotid bulb.

Even after two years of experience and more than 2000 exams with the duplex scan, our accuracy for stenoses below 50% and for arteriosclerotic plaques compared to the arterial angiography is significantly lower than described in the literature. We conducted our study strictly blind, without knowledge of each other’s results, and placed strict guidelines on the interpretation of the angiography. All the angiographed vessels were included in the study even if they could not be visualized optimally with duplex scan. Considering the methodical problems of the duplex scan and the problematic of angiography as a reference method, we believe that the accuracy for Duplex and B-scan in comparison to angiography has been estimated too high in the literature.
Our experiences with the combined use of the two non-traumatic methods, CW-Doppler sonography and duplex scan, show that it is possible to use CW-Doppler sonography as the sole exam method as long as the limitations regarding low-grade stenoses are taken into consideration. The sole use of CW-Doppler sonography is not appropriate since high-grade stenoses might be overlooked.

habdomyolysen treten bei traumatischen und nicht traumatischen Ursachen auf. Sie sind als Folgen einer muskulären Ischämie bei
Polytrauma, Kompartmentsyndrom,
Schock oder Überbeanspruchung
(„Jogging Disease“) bekannt. Toxische
Rhabdomyolysen können durch
Störungen des Elektrolythaushalts, endokrine Erkrankungen, maligne Hyperthermie, Lebensmittelintoxikationen (Toxic Oil Syndrome) und eine
Vielzahl von Drogen und Medikamenten hervorgerufen werden [5]. Neben
Opiaten und Kokain spielen unter anderem Barbiturate, Diphenhydramin,
Narkotika und Neuroleptika eine Rolle. Nicht selten handelt es sich um multifaktorielle Auslöser, wie die Kombination von Ischämie und Medikamentenwirkung.
Der erste Fall einer Theophyllinintoxikation mit Rhabdomyolyse wurde
1985 durch Robertson beschrieben
[18]. Bislang sind in der internationalen
Literatur 24 ähnliche Fälle publiziert
worden [1, 4, 5, 7, 8, 11–14, 16–19, 22,
25–29]. Die meisten Autoren berichten über akute Intoxikationen mit
deutlich erhöhten Serumtheophyllinspiegeln. Die Vermeidung potentieller
Komplikationen erfordert eine rasche
Diagnose und die konsequente Einleitung einer suffizienten Therapie.
FALLBESCHREIBUNG
❑ Anamnese
Ein 73-jähriger Patient mit einer seit
vier Jahren bekannten Herzinsuffizienz
im Stadium III bis IV (NYHA) im
Rahmen einer dilatativen Kardiomyopathie wurde mit hochgradiger Luftnot, Übelkeit und Erbrechen stationär
aufgenommen. Weiterhin sind eine
seit drei Jahren bestehende absolute
Arrhythmie bei Vorhofflimmern, eine
chronisch obstruktive Bronchitis mit
begleitendem Lungenemphysem und
eine reaktive Depression zu erwähnen.
❑ Fallbeschreibung: Es wird über einen 73-jährigen Mann mit Rhabdomyolyse
im Rahmen einer akuten Theophyllinintoxikation berichtet. Nach unkontrollierter Einnahme einer unbekannten Menge eines retardierten Theophyllinpräparates in Kombination mit Furosemid wurde er mit Tachykardie, Übelkeit
und motorischer Unruhe stationär aufgenommen. Der maximale Theophyllinspiegel betrug 66,5 mg/l. Initial lagen eine Hypokaliämie (2,8 mmol/l),
Hyponatriämie (123 mmol/l) sowie erhöhte Werte für Myoglobin (3 789 µg/l)
und Kreatinkinase (32,29 µmol/l/s) vor. Es erfolgten die unverzügliche orale
Gabe von Aktivkohle in Kombination mit Hämodialyse (CVVH) und forcierter Diurese sowie die intravenöse Gabe von Kaliumchlorid, Natriumchlorid
und Metoprolol. Die Serumwerte für Theophyllin, Kreatinkinase und Myoglobin normalisierten sich innerhalb kurzer Zeit, ohne dass ein sekundärer Anstieg des Theophyllinspiegels aufgetreten wäre.
❑ Schlussfolgerung: Rhabdomyolysen sind seltene Komplikationen einer Theophyllinintoxikation. In der Literatur sind bislang nur wenige Fälle mitgeteilt
worden. Der vorliegende Fallbericht unterstreicht die Notwendigkeit einer
schnellen Diagnose und konsequenten Therapieeinleitung, um ein akutes Nierenversagen oder einen letalen Verlauf zu vermeiden. Basierend auf Pathogenese und Epidemiologie theophyllininduzierter Rhabdomyolysen werden Risikofaktoren und Therapieprinzipien formuliert und anhand der Literatur-
übersicht diskutiert.

Poisoning
❑ Case Report: A case of a 73-year-old male with theophylline overdose complicated by rhabdomyolysis is reported. After uncontrolled self-medication
with an unknown number of theophylline slow release 350 mg tablets and
furosemide 40 mg tablets he was admitted with unspecific clinical signs like
tachyarrhythmia, vomiting and restlessness. Maximum theophylline concentration was 66.5 mg/l, other abnormal laboratory findings included hypokalemia (2.8 mmol/l) and hyponatremia (123 mmol/l). The maximum creatinkinase level was measured after admission (32.29 µmol/s/l) accompanied by a
serum myoglobin level of 3,789 µg/l. Immediate treatment with oral activated

Medikamentös wurde vor Klinikaufnahme mit Tagesdosen von 240 mg
Verapamil, 5 mg Benazepril, 50 mg
Trimipramin und 700 mg eines retardierten Theophyllinpräparates behandelt. Wegen der Flimmerarrhythmie
erfolgte eine orale Antikoagulation mit
Phenprocoumon. Die Medikamenteneinnahme erfolgte unregelmäßig
und eigenmächtig. Unter anderem
wurden auch „ab und zu“ zwei der
früher einmal verordneten „Wassertabletten“ eingenommen, wobei sich
herausstellte, dass es sich um Furosemid
40 mg Tabletten handelte.
❑ Klinischer Befund
Bei der Aufnahmeuntersuchung fielen
bei dem 184 cm großen, 94 kg schweren, deutlich geschwächten Patienten
Ruhedyspnoe und Lippenzyanose auf.
Der Patient wirkte erregt und ängstlich
agitiert bei depressiver Stimmungslage.
Äußere Verletzungszeichen oder ein
Zungenbiss fanden sich nicht, ein Bewusstseinsverlust oder Krämpfe seien
nicht aufgetreten. Über dem Brustkorb
ließen sich ubiquitär trockene Rasselgeräusche auskultieren. Die Herzfrequenz betrug zentral etwa 145/min,
die Aktionen waren arrhythmisch mit
Pulsdefizit, der Blutdruck betrug 80/50
mm Hg. Die Leber war 4 cm unterhalb
des Rippenbogens tastbar. Die neurologische Untersuchung zeigte gesteigerte
Muskeleigenreflexe und einen feinschlägigen Ruhetremor.
❑ Labordiagnostik
In den zum Aufnahmezeitpunkt durchgeführten Laboruntersuchungen lagen
folgende Werte außerhalb des Referenzbereichs: Natrium 123 mmol/l,
Kalium 2,83 mmol/l, Harnstoff 14,9
mmol/l, Kreatinin 220 µmol/l, Theophyllinspiegel 61,5 mg/l (normal 10 bis
20 mg/l), Myoglobin im Serum 3789,0
µg/l (normal 20 bis 70 µg/l), Kreatinkinase 32,29 µmol/l/s (normal bis 1,33
µmol/l/s), Lactat 5,08 mmol/l (normal
0,5 bis 2,2 mmol/l) und INR 2,8. Die
kapilläre Blutgasanalyse zeigte eine geringe respiratorische Partialinsuffizienz:
pO2
7,75 kPa, pCO2
3,93 kPa, O2
-Sättigung 90,4%. Es bestand eine geringe,
respiratorisch kompensierte metabolische Azidose: pH 7,41, SBC 20,6, BE
– 4,4.
❑ Technische Untersuchungen
Das Zwölf-Kanal-EKG zeigte eine absolute Tachyarrhythmie mit Frequenzen um 146/min in Verbindung mit
vereinzelten polytopen ventrikulären
Extrasystolen, es bestand ein kompletter
Rechtsschenkelblock. Am Folgetag
wurden vereinzelt kurze ventrikuläre
Salven registriert. In der Röntgenaufnahme des Thorax fiel ein allseits deutlich verbreitertes Herz mit Zeichen der
chronischen Lungenstauung auf.
Therapeutisches Vorgehen
und Verlauf
Unter dem Verdacht einer Theophyllin- intoxikation erfolgte nach Vorliegen der Laborwerte unverzüglich die
wiederholte orale Gabe von Aktivkohle. Da der deutlich erhöhte Myoglobinwert auf das Vorliegen einer Rhabdomyolyse schließen ließ, wurde eine
einmalige Hämodialyse (CVVH) über
fünf Stunden durchgeführt. Die hypotone Kreislaufsituation konnte durch
kontinuierliche Infusion von Dopamin
innerhalb kurzer Zeit stabilisiert werden. Die Diurese wurde danach durch
Gabe von Furosemid und Infusion von
Elektrolytlösungen forciert, sodass eine
stündliche Ausscheidungsrate von ca.
500 ml Urin pro Stunde über den ersten
Behandlungstag aufrechterhalten werden konnte. Der Ausgleich der Hypokaliämie und Hyponatriämie erfolgte
durch kontinuierliche zentralvenöse

charcoal and continuous veno-venous hemodialysis (CVVH) was instituted,
together with intravenous potassium and sodium chloride substitution, forced
diuresis and continuous administration of intravenous metoprolol. The
theophylline, creatinkinase and myoglobin levels decreased rapidly and there
was no second rise in theophylline found. The patient survived without
sequelae.
❑ Conclusion: Rhabdomyolysis is a rare complication of theophylline intoxication. In literature only a small number of cases are reported. Our results
illustrate the necessity of a purposeful and fast management to successfully
prevent renal failure or death. Some pathogenetic mechanisms of theophyllineinduced rhabdomyolysis, epidemiologic data, risk factors and therapeutical
principles will be demonstrated by a detailed literature survey.

sen in der Literatur beschrieben wurden, scheint deren Inzidenz häufiger zu
sein als bislang angenommen. Higgins et
al. [8] fanden bei 53 Fällen von Theophyllinintoxikation vier Verläufe mit
Hypokaliämie und Rhabdomyolyse.
Sessler [22] konnte bei 116 Fällen akuter
oder chronischer Theophyllinintoxikation einen Fall mit Rhabdomyolyse
identifizieren.
Es werden verschiedene Entstehungsmechanismen der Auslösung von
Rhabdomyolysen diskutiert. Während
einige Autoren dem Theophyllin einen
direkten toxischen Effekt auf Myozyten
zuschreiben [11, 12, 29], vermuten andere einen direkten Einfluss der durch
Theophyllin induzierten Hypokaliämie
[11, 13, 14, 18, 19, 27, 28]. Hypokali-
ämien gelten neben Hypophosphatämien [5] und Hypernatriämien [10] als eigenständige Auslöser von Rhabdomyolysen [2]. Sie wurden in nahezu allen
Fällen mit akuter Theophyllinüberdosierung und etwa der Hälfte der chronischen Intoxikationen gefunden [22].
Substanzen, die ähnlich dem Theophyllin die Aktivität der Na-K-ATPase erhöhen, wie 2
-Mimetika oder Insulin,
können diese Hypokaliämie verstärken
und dadurch zur Entstehung einer
Rhabdomyolyse prädisponieren [2].
Gleiches gilt für Kaliumverluste anderer
Art, wie zum Beispiel bei Diuretikatherapie oder gastrointestinalen Kaliumverlusten [23].
Wiederholte Grand-mal-Anfälle, die
im Rahmen der Theophyllinintoxikation auftreten können, werden ebenfalls
in einer Reihe von Fällen als auslösende
Ursache beschrieben [12, 14, 18, 19, 27,
28]. Die zugrunde liegenden Pathomechanismen sind komplex. Der Dysbalance zwischen Sauerstoffangebot und
Nachfrage in der Muskulatur wird eine
besondere Bedeutung zugeschrieben
[3, 11]. Eine Hypoxie ist einerseits oft
Folge schwerer Asthmaanfälle, kann
aber, wie im vorliegenden Fall, auch im
Rahmen einer Diffusionsstörung bei
pulmonaler Stauung im Rahmen der
Linksherzinsuffizienz auftreten. Erhöhte Lactatwerte gelten als Ausdruck der
Gewebshypoxie [9]. Der muskuläre
Sauerstoffverbrauch kann andererseits
durch eine erhöhte Beanspruchung der
Atem- und Atemhilfsmuskulatur im
Rahmen eines Asthmaanfalls oder als
Folge tonisch-klonischer Krämpfe erheblich gesteigert sein. Auch Pharmaka wie 2
-Mimetika und Theophyllin
sind über die Induktion von Tremor,
motorischen Unruhezuständen oder
Krampfanfällen in der Lage, den Sauerstoffverbrauch der Muskulatur zu erhöhen.
Ein vermindertes O2
-Angebot bei
gesteigertem muskulären O2
-Bedarf
mit der Folge einer Gewebshypoxie
stellt somit möglicherweise einen zentralen Pathomechanismus der Rhabdomyolyse dar. Über einen Mangel an
ATP in Myozyten kommt es zum Erliegen energieabhängiger intrazellulä-
rer Prozesse. Unter anderem sind die
Proteinbiosynthese sowie Transportund Reparaturmechanismen der Zellmembran betroffen. Infolgedessen
kommt es zum Austritt intrazellulärer
Bestandteile wie Myoglobin [3, 11],
das bei massivem Anfall ein akutes Nierenversagen induzieren kann. Vergleichbar sind die am quer gestreiften
Muskel ablaufenden Prozesse mit den
Vorgängen am durchblutungsgestörten Myokard. Im Gegensatz dazu
werden die Folgen der Membrandesintegrität bei einer Rhabdomyolyse
aufgrund der erheblich größeren
Masse an quer gestreifter Muskulatur
für den Gesamtorganismus durchaus
relevant [6].
Unter Berücksichtigung dieser pathogenetischen Zusammenhänge lassen sich einige Risikofaktoren für das
Auftreten theophyllininduzierter
Rhabdomyolysen formulieren (Tabelle 1).
Die Relevanz dieser Risikofaktoren
wird durch die in der Literatur mitgeteilten Fallbeschreibungen bestätigt. So
lagen die Theophyllinspiegel in den
meisten berichteten Fällen höher als 80
mg/l, meistens bestanden gleichzeitig
Hypokaliämien und/oder generalisierte
Krampfanfälle [1, 4, 5, 7, 8, 11–14,
16–19, 22, 25–29]. Mitunter spielten

-Mimetika als Begleitmedikation eine Rolle [5, 19].
Infusion von kalium- und natriumchloridhaltigen Lösungen. Der Patient erhielt Sauerstoff über eine O2
-
Maske sowie Metoprolol per Dauerinfusion.
Der Erfolg der unverzüglich unter
intensivmedizinischem Monitoring
eingeleiteten Maßnahmen konnte anhand der Laborparameter dokumentiert werden. Nach einem weiteren
Anstieg des Theophyllinspiegels auf
66,5 mg/l kurz vor Beginn der Hämodialyse hatte sich der Wert bereits nach
dreistündiger Hämodialyse nahezu halbiert (Abbildung 1). Die Parameter des
Elektrolyt- und Säure-Basen-Haushalts
lagen vier Stunden nach Therapiebeginn wieder im Normbereich, nach etwa zehn Stunden war der Myoglobinwert auf die Hälfte gesunken (siehe
Abbildung 1). Unter Berücksichtigung
der Pharmakokinetik des verwendeten
retardierten Theophyllinpräparats wurde die Behandlung mit Aktivkohle
auch am Folgetag fortgesetzt, wobei
Hämodialyse, forcierte Diurese und
Elektrolytsubstitution beendet werden
konnten.
Nach zwei Tagen intensivmedizinischer Behandlung konnte der Patient auf
eine Normalstation verlegt werden. In
wiederholten ausführlichen Gesprächen
klagte er über eine für ihn unerträgliche
Vereinsamung nach dem Tod der Ehefrau. Aus Verzweiflung, Schlaflosigkeit
und wegen ständig wechselnder
Schmerzen habe er wiederholt eigenmächtig Medikamente eingenommen.
Anzeichen für Suizidalität wurden durch
den konsiliarisch hinzugezogenen Psychiater ausgeschlossen. Nach 18-tägiger
stationärer Therapie konnte der Patient
wunschgemäß in ein Pflegeheim entlassen werden.
DISKUSSION
Obwohl bislang nur wenige Fälle von

• Schwere Intoxikationen mit hohen Theophyllinspiegeln (> 80–100 mg/l)
• Faktoren, die eine Hypokaliämie bewirken
• Erkrankungen, die zu einer Hypoxie führen
• Erhöhter muskulärer Sauerstoffverbrauch durch: – Muskelaktivität
– Krampfanfälle

Im vorliegenden Fall lag mit einem
Wert von 66,5 mg/l ein noch nicht exzessiv erhöhter Theophyllinserumspiegel vor. Initial zeigte sich eine ansteigende Konzentration (siehe Abbildung
1). Aufgrund der pulmonalen und
kardialen Vorerkrankungen bestand
zum Aufnahmezeitpunkt eine Hypoxie. Krampfanfälle traten nicht auf. Die
unkontrollierte Einnahme von Furosemid durch den Patienten kann zu der
bei Krankenhausaufnahme manifesten
Hypokaliämie und Hyponatriämie beigetragen haben.
Die Diagnose Rhabdomyolyse wurde durch die Bestimmung des Myoglobins im Serum gestellt. Durch die innerhalb kurzer Zeit ergriffenen Notfallmaßnahmen, wie Detoxikation, Elektrolytausgleich, Sauerstoffzufuhr und
Antiarrhythmikatherapie, konnten weitere Komplikationen verhindert werden. In der Literatur wird ein akutes
Nierenversagen als häufigste Komplikation einer Rhabdomyolyse angegeben
[26]. Mindestens drei von 24 referierten
Fällen verliefen tödlich [16, 18, 22].
Als primäre Maßnahmen der Therapie einer Theophyllinintoxikation werden Verfahren der Giftelimination, wie
Hämoperfusion, orale Gabe von Aktivkohle und forcierte Diurese, angegeben
(Tabelle 2). Die Hämoperfusion gilt als
das sicherste Verfahren [8, 14, 15]. Der
kombinierte Einsatz von Hämodialyse
und Hämoperfusion erreicht Extraktionsraten von bis zu 86% [16]. Wenn die
Hämoperfusion nicht verfügbar ist,
kann auch eine alleinige Hämodialyse
effektiv sein [15], wobei allerdings mit
einer niedrigeren mittleren Extraktionsrate von ca. 26% zu rechnen ist [8].
Die orale Gabe von Aktivkohle wird
in der Literatur unterschiedlich bewertet. Einerseits ist ihr Einsatz durch Übelkeit und Erbrechen als häufige Symptome einer Theophyllinintoxikation
limitiert. Andererseits existieren unterschiedliche Auffassungen darüber, ob
ein sekundärer Anstieg der Theophyllinspiegel bei vorausgegangener Einnahme retardierter Theophyllinpräparate verhindert werden kann [8, 12, 20].
Man sollte allerdings auf diese Form der
Resorptionshemmung nach Möglichkeit nicht verzichten.
Im vorliegenden Fall konnte gezeigt
werden, dass der Einsatz der Hämodialyse in Kombination mit oraler Gabe
von Aktivkohle und forcierter Diurese
zu einer Halbierung der Theophyllinkonzentration innerhalb von etwa drei
Stunden führte. Eine weitere Progression der Rhabdomyolyse konnte somit
verhindert werden, was durch den raschen Abfall von Kreatinkinase und
Myoglobin dokumentiert wurde (siehe
Abbildung 1).
Das weitere therapeutische Procedere
leitet sich aus der Pathophysiologie der
theophyllininduzierten Rhabdomyolyse ab. Dazu zählen die Beseitigung der
Hypokaliämie, eine ausreichende Oxygenierung, Volumentherapie sowie die
Gabe von Antiarrhythmika und Antiemetika [20, 22]. Beim Auftreten einer
hämodynamischen Instabilität, die von
Sessler [22] in einer Häufigkeit von etwa einem Viertel der Patienten mit akuter Theophyllinintoxikation beobachtet wurde, wird vor dem Einsatz von
Vasopressoren eine Normalisierung des
Herzrhythmus und des Flüssigkeitshaushalts (Volumengabe) empfohlen
[22]. Zur Prophylaxe eines akuten Nierenversagens sollte mittels Bicarbonatinfusion ein Urin-pH von > 6 angestrebt werden, um die Tubulotoxizität
des Myoglobins zu reduzieren [24].
SCHLUSSFOLGERUNG
Mit der vorliegenden Falldarstellung einer Rhabdomyolyse bei Theophyllinintoxikation soll auf eine wenig beachtete unerwünschte Arzneimittelwirkung bei einem häufig verordneten Medikament mit geringer therapeutischer
Breite hingewiesen werden. Es werden
Risikofaktoren für das Auftreten dieser
Komplikation formuliert und begründet. Durch rechtzeitige Bestimmung
des Myoglobins im Serum kann die
Diagnose noch vor dem Eintreten einer gravierenden Nierenfunktionseinschränkung gestellt werden. Die Kenntnis pathogenetischer Zusammenhänge
ermöglicht eine adäquate Therapie. Das
Fallbeispiel zeigt, dass durch die konsequente Einleitung einer gezielten Behandlung ein Fortschreiten der Rhabdomyolyse mit den damit verbundenen
Komplikationen verhindert werden
kann.

Rhabdomyolysis as a Rare Complication in Theophylline Intoxication

Summary
- Case Study: The report is about a 73-year-old man with rhabdomyolysis related to acute theophylline intoxication. The man had been admitted to the hospital with tachycardia, nausea, and motor restlessness after the ingestion of an unknown amount of sustained-release theophylline in combination with furosemide. The maximum theophylline level was 66.5 mg/l. Initial lab results showed hypokalemia (2.8 mmol/l), hyponatremia (123 mmol/l), as well as increased myoglobin (3789 μg/l) and creatine kinase (32.29 μmol/l/s). Activated charcoal was administered immediately in combination with hemodialysis (CVVH) and forced diuresis, as well as intravenous administration of potassium chloride, sodium chloride and metoprolol. Serum values for theophylline, creatine kinase and myoglobin normalized within a short period of time without the occurrence of a secondary rise in the theophylline level.
- Conclusion: Rhabdomyolysis is a rare complication of theophylline intoxication. Only a few cases have been mentioned in the literature. The present case study emphasizes the necessity of a prompt diagnosis followed by the purposeful initiation of treatment to avoid acute renal failure or a fatal course. Risk factors and treatment principles are formulated and discussed with the aid of the literature survey based upon the pathology and epidemiology of theophylline-induced rhabdomyolysis.

Rhabdomyolysis has traumatic and non-traumatic causes. It is known as a result of a muscular ischemia in multi-trauma, compartment syndrome, shock, or overuse (jogging disease). Toxic rhabdomyolysis can be caused by disturbances of the electrolyte balance, endocrine diseases, malignant hyperthermia, food poisoning (toxic oil syndrome), and a multitude of drugs and medications [5]. Besides opiates and cocaine, barbiturates, diphenhydramine, narcotics, and neuroleptics are involved, among others. The trigger is often multi-factorial; it could be a combination of ischemia and medication effects.
The first case of theophylline intoxication with rhabdomyolysis was described in 1985 by Robertson [18]. A total of 24 similar cases have been published in the international literature [1, 4, 5, 7, 8, 11-14, 16-19, 22, 25-29]. Most of the authors report of acute intoxications with significantly elevated serum theophylline levels. A swift diagnosis and the purposeful initiation of a sufficient therapy is required to avoid potential complications.
CASE STUDY
-Case History
A 73-year-old patient with a known four-year history of heart failure stage III to IV (NYHA), related to dilated cardiomyopathy, was admitted with severe shortness of breath and nausea and vomiting. Furthermore, a three-year history of atrial fibrillation, chronic obstructive bronchitis with pulmonary emphysema, and a reactive depression are worth mentioning. Prior to admission, the patient took 240 mg verapamil, 5 mg benazepril, 50 mg trimipramine and 700 mg of sustained release theophylline daily. Due to the atrial fibrillation, the patient was orally anti-coagulated with phenprocoumon. The patient had been taking his medications erratically and arbitrarily, including two of the formerly prescribed “water pills” here and there, which turned out to be furosemide 40 mg tablets.
-Clinical Findings
Dyspnea at rest and cyanosis of the lips were the foremost symptoms during the admission assessment of the markedly weak patient measuring 184 cm with a weight of 94 kg. The patient appeared restless, anxious, and agitated with a depressed mood. There were no obvious injuries or biting of the tongue; the patient denied any seizures or loss of consciousness. Ubiquitous dry rattling could be auscultated across the chest. Central heart rate was 145/min, arrhythmic and with a pulse deficit; blood pressure was 80/50 mm HG. The liver was palpable at 4 cm below the rib cage. Neurological examination showed increased deep tendon reflexes and a fine resting tremor.
-Laboratory Diagnostic
The admission lab work showed the following abnormal values: Sodium 123 mmol/l, potassium 2.83 mmol/l, BUN 14.9 mmol/l, creatinine 220 μmol/l, theophylline level 61.5 mg/l (normal 10 to 20 mg/l), myoglobin 3789.0 μg/l (normal 20 to70 μg/l), creatine kinase 32.29 μmol/l/s (normal up to 1.33 μmol/l/s), lactate 5.08 mmol/l (normal 0.5 to 2.2 mmol/l), and INR 2.8. Capillary blood gas analysis showed a slight respiratory partial insufficiency: pO₂ 7.75 kPa, pCO₂ 3.93 kPa, O₂ saturation 90.4%. A slight compensated metabolic acidosis was present: pH 7.41, HCO₃ 20.6, BE -4.4.
-Technical Exams
The 12-lead EKG showed an absolute tachyarrhythmia with a rate of 146/min and isolated multifocal premature ventricular contractions. There was a complete right bundle branch block. There were some short ventricular salvos present the next day. Chest radiography showed a significantly enlarged heart with signs of chronic pulmonary vascular congestion.
Treatment and Course
With the suspicion of theophylline intoxication, repeated oral doses of activated charcoal were administered immediately after the lab results had been available. A one-time hemodialysis (CVVH) over five hours was performed since the markedly increased myoglobin pointed to rhabdomyolysis. The hypertension was resolved within in a short period of time with a continuous dopamine infusion. Administration of furosemide and electrolyte solutions forced diuresis, amounting to an hourly urine output of about 500 ml during the first day of treatment. Continuous central venous infusion of Potassium and sodium chloride solutions compensated for the hypokalemia and hyponatremia. The patient received oxygen via O₂ mask as well as a continuous infusion of metoprolol.
The success of the prompt interventions in an intensive care setting could be documented with the lab work. After a further rise of the theophylline level to 66.5 mg/l just prior to the begin of hemodialysis, the level was almost cut in half after just three hours of hemodialysis (figure 1).Taking the pharmacokinetics of the sustained release theophylline preparation into account, the treatment with activated charcoal was continued for one more day, while hemodialysis, forced diuresis, and electrolyte replacement could be stopped.
The patient was able to be transferred to a regular floor after two days in intensive care. The patient complained in repeated in-depth conversations about the unbearable loneliness after the death of his wife. He stated that he repeatedly had been taken his medications arbitrarily out of desperation, sleeplessness, and constantly varying aches and pains. A consulted psychiatrist ruled out suicidal ideation. According to his wishes, the patient was able to be discharged to a nursing home after spending 18 days in the hospital.

DISCUSSION
Although just a few cases theophylline induced rhabdomyolysis have been reported in the literature, there appears to be a higher incidence than assumed thus far. Higgins et al. [8] found four cases with hypokalemia and rhabdomyolysis among 53 cases of theophylline intoxication. Sessler [22] was able to identify one case with rhabdomyolysis among 116 incidences of acute or chronic theophylline intoxication.
Different trigger mechanisms for rhabdomyolysis are being discussed. While some authors attribute theophylline with a direct toxic effect on myocytes [11, 12, 29], others assume a direct influence of the theophylline induced hypokalemia [11, 13, 14, 18, 19, 27, 28]. Hypokalemia is regarded by itself as a trigger for rhabdomyolysis [2], along with hypophosphatemia [5], and hyponatremia [10]. Hypokalemia was present in just about all the cases of acute theophylline intoxication and in about half of the chronic intoxications [22]. Substances that increase the Na-K-ATPase activity in a similar fashion as theophylline does, like β₂-agonists or insulin, can exacerbate the hypokalemia and hence increase the likelihood of rhabdomyolysis [2]. The same holds true for potassium loss of different origin, like diuretics use or gastro-intestinal potassium depletion [23].
Repeated grand-mal seizures, which may occur within the framework of theophylline intoxication, have also been mentioned as a trigger in a series of cases [12, 14, 18, 19, 27, 28]. The underlying pathomechanisms are complex. A special significance has been attributed to the imbalance between oxygen supply and demand of the musculature [3, 11]. Hypoxia is, on one hand, often caused by a severe asthma attack but can also occur within a diffusion deficit in left-sided heart failure, as it happened in the present case. Increased lactate levels are regarded as an expression of tissue hypoxia [9]. On the other hand, the muscular oxygen consumption might be increased by a heightened demand of the respiratory muscles and their accessories during an asthma attack or as sequel to tonic-clonic seizures. Also drugs, like β₂-agonists and theophylline, are capable of increasing the oxygen consumption of the musculature through an induction of tremors, motoric restlessness, or seizures.
A decreased O₂-supply with an increased muscular O₂-demand followed by tissue hypoxia might therefore present a central pathomechanism of rhabdomyolysis. Energy-dependent intra-cellular processes come to a halt with an ATP-deficit in the myocytes. Protein- biosynthesis as well as transport and repair mechanism of the cell membrane is affected, among others. This leads to a leaking of intracellular components like myoglobin [3, 11], which can induce acute renal failure in a massive attack. The processes occurring in the skeletal musculature are comparable to processes in an ischemic myocardium. The effects of the disturbed membrane integrity in rhabdomyolysis are much more relevant for the entire organism because of the much greater mass of skeletal musculature in contrast to the myocardium [6].
Taking these pathogenetic connections into account, it is possible to formulate several risk factors for the occurrence of a theophylline induced rhabdomyolysis (table 1).
The relevance of these risk factors is confirmed by the case studies reported in the literature. Thus the theophylline levels in most of the reported cases were greater than 80 mg/l, concurrent hypokalemia was present in most of the reported cases and/or generalized seizures [1, 4, 5, 7, 8, 11-14, 16-19, 22, 25-29]. β₂-agonists were sometimes involved as an additional medication [5, 19].

The theophylline level in the case at hand was with 66.5 mg/l not yet excessively high. Initially, there was an increasing concentration (figure1). Hypoxia was present at the time of admission due to pre-existing pulmonary and cardiac conditions. Seizures did not occur. The uncontrolled intake of furosemide by the patient might have contributed to the hypokalemia and hyponatremia present at admission to the hospital.
Rhabdomyolysis was diagnosed by determining myoglobin levels in serum. Further complications could be prevented with the prompt initiation of emergency measures like detoxification, correction of electrolytes, oxygen administration, and antiarrhythmic therapy. Acute renal failure is the most common complication mentioned in the literature [26]. At least three of the 24 referenced cases had a fatal outcome [16, 18, 22].
Primary measures in the treatment of theophylline intoxication mentioned are methods of toxin elimination like hemoperfusion, oral administration of activated charcoal, and forced diuresis (table 2). Hemoperfusion is considered to be the safest measure [8, 14, 15]. The combined use of hemodialysis and hemoperfusion achieves an extraction rate of up to 86% [16]. Hemodialysis can be effective by itself if hemoperfusion is not available [15], although a lower medium extraction rate of about 26% is to be expected [8].

Oral administration of activated charcoal gets a mixed review in the literature. Its use is limited on the one hand by nausea and vomiting that is a frequent symptom in theophylline intoxication and there are different opinions about the possibility of avoiding a secondary rise in theophylline levels if sustained-release theophylline preparations had been ingested [8, 12, 20]. Nonetheless, this kind of reabsorption inhibitor should be employed if possible.
The present case demonstrated that the use of hemodialysis in combination with the oral administration of activated charcoal and forced diuresis cut the theophylline concentration in half within about three hours. This avoided a further progression of rhabdomyolysis, which was documented with the rapid decrease of creatine kinase and myoglobin (see figure 1).
The further treatment is dictated by the pathophysiology of theophylline induced rhabdomyolysis and includes correction of hypokalemia, sufficient oxygenation, volume therapy as well as the administration of antiarrhythmic and antiemetic drugs [20, 22]. In case of hemodynamic instability, observed by Sessler [22] in about a quarter of all patients with acute theophylline intoxication, it is recommended to normalize the heart rhythm and the fluid balance (volume administration) prior to using vasopressors. A urine pH of > 6, achieved with sodium bicarbonate infusion, should be the goal to reduce the tubular toxicity of the myoglobin and therefore prevent acute renal failure [24].
CONCLUSIONS
It is the purpose of this case study of rhabdomyolysis in theophylline intoxication to draw attention to a rarely considered adverse effect of a much prescribed drug with a narrow therapeutic spectrum. Risk factors for these adverse effects have been formulated and explained. Timely identification of myoglobin in the serum can aid in the diagnosis even before any significant renal insufficiency occurs. Knowledge of the pathogenetic relationships makes an adequate treatment possible. The case study reveals that a purposeful initiation of a targeted treatment can avoid a progression of rhabdomyolysis and the complications involved.
