Unterstützung normaler Beweglichkeit, Elastizität von Geweben fördert ihre „normale Funktion“, z.B. Harnkontinenz. Dysfunktionen entstehen nicht selten als Dysfunktionskomplex mit einer Kombination von Motilitäts-, Mobilitäts- und Motrizitätsstörungen. Nach Zweedjik (2007) stehen im Neugeborenenalter im Allgemeinen abnorme Motilitätsmuster, jedoch bei Störungen der Harnblase eher Mobilitätsstörungen des Beckens im Vordergrund. Bei älteren Kindern sind i.d.R. Mobilitäts- und Motrizitätsstörungen dominierender.
Die osteopathische Behandlung berücksichtigt außerdem die jeweilige Innervation, Versorgung und Drainage. Zu erwähnen ist, dass die Durchsicht aller herangezogenen Studien keinen Hinweis auf Erfolg osteopathisch-manueller Interventionen bei Enuresis belegt. Im Diskussionsabschnitt postulierte Zusammenhänge zur osteopathischen Vorgehensweise sind deshalb hypothetisch.

Il sostegno alla normale mobilità, l’elasticità tissutale favorisce la sua “funzione normale”, ad esempio la continenza urinaria. Non è raro che le disfunzioni si presentino quale complesso di disfunzioni con una combinazione di disturbi pertinenti a motilità, mobilità e motricità. It seems that the physicians who are caring for the patient have ordered the correct tests. I would add some additional details as outlined below. Since there is no biologic marker for ALS it is a diagnosis of exclusion. Meaning other treatable disorders must be ruled out. About 20% of cases of ALS present with symptoms isolated to the bulbar region, making it difficult to arrive at a presumptive diagnosis. It is unclear if there are fluctuations in clinical symptoms during the course of the day which might suggest Myasthenia Gravis despite the normal decrement and SFEMG. Antibodies to acetylcholine receptor antibodies including MUSK would be helpful. Additional details about extremity motor, sensory and reflex exam would be useful to identify whether there is a mixed picture of upper and lower motor neuron dysfunction. In addition, more extensive cognitive testing and assessment of the extramyramidal system would be helpful to rule out other classes of neurodegerative disorders that can co-exist with bulbar dysfunction, particularly spastic.
While EMG /NCV may be unremarkable at early stages, sufficient sampling of extremity muscles i.e. three limbs plus paraspinal muscles is necessary to assess whether there is sufficient diffuse active neurogenic changes i.e. positive and fibrillation potentials. Sometime lumbar puncture and muscle biopsy can be useful to rule out confounders of motor neuron disorders and we could consider it after the above testing if non-diagnostic.

Poiché non vi è alcun marker biologico per la SLA, si tratta di una diagnosi di esclusione. Altri disturbi trattabili importanti devono essere individuati. Circa il 20% dei casi di SLA si presenta accompagnato da sintomi isolati alla regione bulbare, rendendo difficile pervenire ad una diagnosi presuntiva. Disporre di ulteriori dettagli circa un esame motorio, sensoriale e riflessologico delle estremità sarebbe utile al fine di identificare la presenza di un'immagine mista di disfunzione neuromotoria superiore ed inferiore. Inoltre, un più esteso esame ed una valutazione cognitiva riguardo al sistema extrapiramidale sarebbero utili al fine di individuare altre classi di disturbi neurodegenerativi che possono coesistere con la disfunzione bulbare, in particolare se spastica.
Mentre EMG/NCV possono essere ininfluenti in corrispondenza dei primi stadi, un sufficiente campionamento dei muscoli delle estremità, ossia di tre muscoli degli arti e dei muscoli paraspinali risulta necessario allo scopo di valutare se vi siano sufficienti modifiche neurogeniche attive, ossia se siano presenti potenziali positivi e di fibrillazione. A volte una puntura lombare e una biopsia muscolare possono essere utili al fine di individuare i confounder di disturbi neuromotori e potremmo prenderli in considerazione successivamente agli esami di cui sopra qualora di tipo non diagnostico.
Ho incluso alla presente un elenco di esami ematici che possono essere d’aiuto e che non necessitano di essere ripetuti se già effettuati: SMA-18, CBC con piastrine, B12 e folato, funzione tiroidea, ESR, CRP, ANA, ANCA, antiDNA, complementi; VDRL., HIV, RF, omocisteina, glicemia a digiuno, HbA1C, profilo lipidico, livello di PTH, calcio e magnesio, elettroforesi delle proteine seriche con immunofissazione per individuare una gammopatia monoclonale; esosamminidasi A, (insorgenza in età adulta Tay-Sach); anticorpi anti-ganglioside (osservazione in motoneuropatia multifocale). Anticorpi per anti-acetilcolinesterasi e MuSK.
