DISCUSSION
The present study was designed to evaluate the effect on MS in saliva and approximal plaque in adolescents with fixed orthodontic appliances after brushing with dentifrice containing different antibacterial agents over 6 months. Orthodontic patients were chosen since it has been shown that treatment with fixed appliances increase the number of MS in the oral cavity (Jordan and LeBlanc, 2002). There is a general consensus that regular use of fluoride dentifrice is a safe and efficient preventive regimen in young individuals (Twetman et al, 2003). However, in orthodontic patients there may be a need for dentifrice that contains an antimicrobial agent to prevent tooth demineralisation and gingivitis (Øgaard et al, 2005).
The method of scoring MS in saliva and at interdental plaque using Dentocult® SM Strip mutans has been used in previous studies (Jensen and Bratthall, 1989). However, the scoring method was modified in the present study. According to Twetman and Petersson (1998, 1999), the evaluation of colonisation of MS was done in two categories, those lower than 20 CFU and those higher than 20 CFU. However in the present study, we found most chair-side strips were colonised with >20 CFU. | Translation - Italian

Il presente studio è stato progettato per valutare l'effetto di S.m. nella saliva e nella placca approssimale in soggetti adolescenti, portatori di apparecchi ortodontici fissi, dopo aver lavato i denti per un periodo di 6 mesi con un dentifricio contenente agenti antibatterici diversi. La scelta è caduta su pazienti ortodontici poiché era stato dimostrato che il trattamento con apparecchi fissi aumentava il numero di S.m. presente nella cavità orale (Jordan e LeBlanc, 2002). Esiste un consenso generale sul fatto che l'uso regolare di dentifrici al fluoruro da parte di soggetti giovani costituisca un regime preventivo sicuro ed efficace (Twetman e coll., 2003). Tuttavia, nei pazienti ortodontici potrebbe essere necessario l'uso di un dentifricio contenente un agente antimicrobico che prevenga la demineralizzazione dentale e le gengiviti (Øgaard e coll., 2005).
Il metodo di attribuzione del punteggio di S.m. nella saliva e nella placca interdentale tramite Dentocult® SM Strip mutans era stato adottato in studi precedenti (Jensen e Bratthall, 1989). Nel presente studio è stato però variato il metodo di attribuzione del punteggio. Secondo Twetman e Petersson (1998, 1999), la valutazione della colonizzazione di S.m. individuava due categorie: inferiore a 20 Ufc e superiore a 20 Ufc. Bei Patienten mit Nichtanlagen oder bei geplanten Extraktionen sollte genau bedacht werden, unter welchen Verankerungserfordernissen und mit welcher Mechanik die Lücken geschlossen werden sollen.
Im Fall der hier vorgestellten Patientin wurde zeitweise erwogen, die 1. kariösen Molaren durch Transplantation der 3. Molaren zu ersetzen. Bauss et al. fanden, dass die Erfolgsrate von Zahnkeimtransplantationen mit anschließender kieferorthopädischer Einordnung bei frischen Extraktionsalveolen 85% und bei atrophierten Kieferabschnitten 84% beträgt. Laut Bauer und Mitarbeitern ist der Lückenschluss durch kieferorthopädisch bewegte Zähne jedoch prognostisch günstiger, da Resorptionen an einem transplantierten Zahn auch noch nach langer Verweildauer auftreten und zu dessen Verlust führen können. Allerdings sehen die Autoren es als Nachteil an, dass bei einem kieferorthopädischen Lückenschluss die Behandlungsdauer lang ist und hohe Anforderungen an die Patientenmitarbeit gestellt werden.
Weiterhin ist die Möglichkeit einer Brückenversorgung nach vollständiger Extraktion der 1. Molaren oder deren Hemisektion denkbar. Löst und Messkoub erachten die Hemisektion mit anschließender prothetischer Versorgung als probates Mittel zur Eingliederung eines parodontal abgestützten Zahnersatzes. Bei der hier vorgestellten Patientin muss jedoch berücksichtigt werden, dass eine Brückenversorgung zur Folge gehabt hätte, mindestens acht kariesfreie Zähne zu beschleifen. | Translation - Italian

Nei pazienti con agenesie, oppure nel caso di estrazioni pianificate, è necessario valutare con grande precisione i requisiti per l’ancoraggio e il tipo di meccanica da adottare per la chiusura degli spazi.
Nel caso della paziente in esame, fu temporaneamente presa in considerazione la possibilità di sostituire, attraverso trapianto, i primi molari cariosi con i terzi molari. Bauss e coll. avevano riscontrato una percentuale di successo dell’85% del trapianto di germi dentali in alveoli estrattivi recenti, con allineamento ortodontico finale, e dell’84% in sezioni mandibolari atrofizzate. Secondo Bauer e coll., invece, dislocare i denti con terapia ortodontica comporta una prognosi di chiusura del diastema più favorevole, perché il riassorbimento di un dente trapiantato può intervenire anche molto tempo dopo la degenza ospedaliera e condurre alla perdita del dente stesso. D'altro canto, gli autori ritengono controproducente che la chiusura ortodontica del diastema richieda un trattamento di lunga durata e l'intensa collaborazione del paziente.
Dopo l’estrazione completa dei primi molari, o la loro emisezione, è possibile considerare anche la possibilità di trattamento con ponte. Löst e Messkoub reputano efficace l’emisezione, con trattamento protesico finale, finalizzata all’inserimento di una protesi dentale ad appoggio parodontale. Nel caso della paziente in esame, però, si doveva considerare che un trattamento con ponte avrebbe implicato la necessità di intervenire su almeno otto denti privi di carie. |
