CANINE ANAPLASMOSIS
In endemic regions, Ixodes scapularis, I. pacificus, or I. ricinus (Europe) transmit Anaplasma phagocytophilum to cats, dogs, sheep, horses and human beings.29
Numerous species of small rodents, including Peromyscus leucopus, the white-footed mouse, serve as reservoirs for the organism in nature.30
Infection has also been documented in numerous species of wild and domestic animals. California, Minnesota, Wisconsin, and several New England states (areas endemic for I. scapularis or I. pacificus) report a particularly high prevalence of infection, generally in dogs, horses, and human beings. Fever, lethargy, and a reluctance to move, accompanied by lymphopenia, thrombocytopenia, elevated serum alkaline phosphatase and amylase activities, and hypoalbuminemia are the most common clinical abnormalities reported in dogs. In the Northeast and North Central United States, acute disease occurs most frequently in the fall months during the period that adult I. scapularis ticks are active. Granulocytic morulae can be found within neutrophils and acute and convalescent serology is recommended to document seroconversion to A. phagocytophilum antigens in acutely ill dogs, cats, or horses. Infection with A. phagocytophilum can also be confirmed by PCR. Both ELISA and IFA tests using E. canis antigens will in most instances not detect cross-reacting antibodies in dogs with anaplasmosis. Therapeutically, tetracycline hydrochloride or doxycycline elicit rapid improvement in clinical status.

2.6.1 Proposal
The first part of the Annex XV report outlines the proposed classification. This contains details of the identity of the substance (substance name, CAS/EC number(s), registration number(s) (if available), molecular formula, structural formula, purity and impurities). The proposed classification and labelling, any proposed specific concentration limits, and a clear statement as to which of the endpoints have been considered and analysed in the dossier should also be given. The classification and labelling based on GHS should also be indicated. In particular, it is important to distinguish between endpoints which have been considered but for which no classification is proposed2 and those that have not been considered. It is also important to indicate in this part of the Annex XV report if the classification is based on an impurity present in the substance rather than based on the properties of the substance itself. In such cases the identity of the impurity and composition of the impurity in the substance leading to the classification should be identified. The applicability of the classification to the same substance but with different levels of the impurity should also be considered.

Examens clinique
L'examen clinique général ne montre rien d'évocateur. La température rectale et les muqueuses sont normales. La palpation abdominale et l'auscultation ne montrent aucune anomalie.
Examens complémentaires
*frottis sanguin:
-recherche de piroplasmes négative,
-examen des leucocytes: abondance de lymphocytes hyperbasophiles et de métamyélocytes suggérant une lignée blanche activée et réactive.
*numération-formule sanguine:
-abondance relative des lymphocytes sans granulocytose
-anémie légère (taux d'hémoglobine inférieur à 12g/dl).
*examen des urines: sang en abondance, leucocytes, protéines, glycosurie à 3 croix, avec une densité urinaire basse voisine de 1010.
*culot de centrifugation: confirme la présence d'hématies et de polynucléaires neutrophiles et montre une grande abondance de bacilles.
*la glycémie, la créatininémie et l'urémie sont conformes aux valeurs usuelles et la protéinémie est par densitométrie voisine de 80g/l.

Diagnostic
L'abondance de glucose et de protéines dans une urine diluée, et ce, sans hyperglycémie suggère l'existence d'un trouble de la réabsorption tubulaire soit congénital soit acquis suite à une tubulo-néphrite infectieuse par exemple. C'est l'hypothèse d'une infection haute du tractus urinaire que nous retenons.
Traitement
Une antibiothérapie est mise en œuvre d'emblée sans antibiogramme dans un premier temps. Un antibiotique bactéricide à large spectre et à bonne diffusion dans les voies urinaires, sans toxicité particulière, est donc choisi: l'association amoxicilline-acide clavulanique.
Le chiot fait 7 kg et il est prescrit un demi-comprimé de Synulox 250 matin et soir pendant 10 jours. De plus, un traitement symptomatique des vomissements est ajouté (métoclopramide et phosphate d'aluminium) pendant quelques jours.
Le chiot doit être revu au plus tard dans les 48 heures en fonction de l'évolution de l'état général. | Translation - Italian

*esame del sedimento: conferma della presenza di eritrociti e di polimorfonucleati neutrofili e di una grande abbondanza di bacilli.
*la glicemia, la creatininemia e l'uremia rientrano nei valori normali e la proteinemia calcolata mediante densitometria è prossima a 80 g/l.

Diagnosi
L'abbondanza di glucosio e proteine in un'urina diluita, in assenza iperglicemia, suggerisce l'esistenza di un problema di riassorbimento tubulare congenito o acquisito, in seguito ad esempio a una tubulonefrite infettiva. Riteniamo che l'ipotesi più probabile sia un'infezione delle prime vie urinarie.
Trattamento
Viene impostata subito un‘antibioticoterapia inizialmente senza antibiogramma. Viene dunque scelto un antibiotico battericida a largo spettro e a buona diffusione nelle vie urinarie, non particolarmente tossico: l'associazione amoxicillina-acido clavulanico.
Il cucciolo pesa 7 kg, per cui viene prescritta mezza compressa di Synulox 250 mattina e sera per 10 giorni. Viene inoltre associato per qualche giorno un trattamento sintomatico del vomito (metoclopramide e fosfato di alluminio).
Il cucciolo deve essere rivisto al massimo entro 48 ore in funzione dell'evoluzione delle condizioni generali.
