Source text - English 8.0 RISK MANAGEMENT AND GROUND CONTROL
8.1 Ground Control Incidents and Accident Statistics
"The mine" has a formal procedure to identify and report sub-standard situations or incidents. "The Consulting Company" disposes of data from July of 2011. Making a deep review, it can be said that around 25% of identified sub-standard situations are related to ground control variables. In numbers, this means 120 cases, a great amount if we are analysing last three months of operation. The biggest problem is the way sub-standard situations are reported:
1. It is very difficult to identify which ones are related to ground control variables.
2. Over the total amount of sub-standard situations, only 4% have an identified (not even confirmed) cause.
3. Over the total amount of sub-standard situations, only 4% have an updated condition (pendent, failed, rescheduled, and completed).
If these three problems of sub-standard documentation reporting are not solved, it is difficult to propose some avoiding or mitigating measures and make evaluation and follow-up. It is highly recommended to develop a better tool for ground control problems identification and documentation. A good tool is not enough if it is not backed by an induction for personnel involved. This tools’ accident classification should be based on Supreme Decree 055 “Occupational Safety and Health Regulation in Mining” – ANNEX 8: Mining Accident Classification .
8.2 Identified Risks
During the visit to the mine from 23rd to 26th September 2011, many locations had been inspected such as galleries at several levels in the XXX and YYY veins, production drifts, a concerned drift entrance, and along accessing roads to reach different levels of the mine. In the following section, the risks will be identified for the correspondent visited areas.
• Identified Risks in Galleries:
During the mine visit, it was not possible to visit all the galleries. The visited gallery is at level NV821. The observation team also climbed up 50 m along an accessing shaft to inspect the shaft, a production drift, and part of the gallery at NV871. The accessing shaft consists of 27 connecting timber ladders, 50 to 55 degrees inclination. In this report, it is assumed that the condition in other galleries is similar to the observed gallery, thus they should have similar risks.
It was observed during the visit that major part of the gallery was not supported, even in some area with potentially loose blocks, as shown in Annex 5 – page 5 and 6. In page 5 of the annex, two joint sets are crossing and creating potential unstable wedges. Some large wedge blocks were already loosened leaving uneven surface on the roof and wall of the gallery, as seen on the picture in the annex. In page 6 of the annex, there are sub-horizontal major joints on the roof of the gallery. This horizontal joint in combination with random vertical joint can create potential unstable blocks. In both situations, it is required careful joint mapping and analyses.
In many part along the gallery, the rock support is a timber system, as shown in Annex 5 – page 8. It seems that the timber system was used wherever the rock is in "extremely poor" quality. In most of the locations with the timber support system, roof had been caved. It may indicate the collapse during construction. The gallery seems reasonably stable with the timber system so far, but there are some risks with this support measure. Observation in the gallery, especial in wet or high humid section, is that the quality of the timber is reduced significantly. Some small trees are observed growing on the main timber beam. This indicates the rot inside the wood. In other places, the rot can be observed clearly as presented in Annex 5 – page 9. So the risk with the timber support is their longevity can be reduced much quicker than expectation, especially in location with much groundwater. In this situation, the collapse accident is just hidden behind the timber system and waiting for a time to happen.
In some other parts of the gallery, many cracks have been observed on the wall, as shown in Annex 5 – page 17. It is anticipated that there may be cracks on the roof too, but the inspection team did not have good lighting as well as lifting machine to be able to inspect the roof.
Visiting to YYY vein, many failed shotcrete with cracks and loosened rock bringing along the shotcrete had been observed in gallery at level NV690. Some typical observed situations are shown in Annex 5 – page 10 to 16. Failed shotcrete and rock were observed in hard fractured rock area and also in the shear zone. In the hard and fractured rock area, it seems that the shotcrete was not sufficient to carry the weight of the loosened rock blocks, as shown in Annex 5 – page 10, pictures on the right. In the shear zone, the rock mass is appearing as "extremely poor" quality, and the failures may indicate insufficient rock support measure. It is noted that major rock support measure in this gallery is shotcrete only. There were few places, systematic bolting was installed.
As an intention to check the installed rock support, rock mass quality is evaluated using Q-system on site, and it is found that the Q-value is ranging from 0.025 to 0.05. Descriptions and values to take for the Q calculation are presented in Table 2. From the calculated Q-value, the Q-system would recommend to support the gallery with category 7, which is combination of 12 to 15 cm fibre reinforced shotcrete plus systematic bolting with the spacing of about 1.2 m, as shown in Figure 1.

8.1 Incidentes de control y estadísticas de accidentes
"La mina" tiene un procedimiento formal para identificar y notificar situaciones o incidentes deficientes. "La consultora" dispone de datos de julio de 2011. Haciendo una revisión profunda, se puede decir que alrededor de un 25% de situaciones deficientes identificadas están relacionadas a variables de control del macizo. En números, esto significa 120 casos, una gran cantidad si estamos analizando los últimos tres meses de operación. El mayor problema es la manera de notificar las situaciones deficientes:
1. Es muy difícil identificar cuáles están relacionados con variables de control del macizo.
2. Sobre el total de las situaciones deficientes, sólo 4% tienen una causa identificada (ni siquiera confirmadas).
3. Del total de situaciones deficientes, sólo el 4% tienen una condición actualizada (pendiente, fallida, re-agendada y completada).
Si estos tres problemas de documentación de notificación deficiente no se han resuelto, es difícil proponer medidas para evitar o mitigar y hacer evaluaciones o seguimientos. Se recomienda fuertemente desarrollar una mejor herramienta para problemas de control del macizo de identificación y documentación. Una buena herramienta no es suficiente si no está respaldada por una inducción por parte de personal involucrado. La clasificación de accidentes de la herramienta debiera basarse en el Decreto Supremo 055 “Regulación de Seguridad y Salud Ocupacional Minera” –Anexo 8: Clasificación de accidentes mineros. .
8.2 Riesgos identificados
Durante la visita a la mina del 23 al 26 de septiembre, 2011, muchos lugares, como galerías a varios nivelas en las vetas XXX y YYY, galerías de producción, una entrada de galería que preocupaba, y a lo largo de caminos de acceso para llegar a diferentes niveles de la mina habían sido inspeccionadas en la siguiente sección, los riesgos serán identificados para las correspondientes áreas visitadas.
• Riesgos identificados en galerías:
Durante la visita a la mina, no fue posible visitar todas las galerías. La galería visitada está al nivel NV821. El equipo de observación también subió 50m por el pique de acceso para inspeccionar el pique, una galería de producción y parte de la galería a nivel NV871. El pique de acceso consiste en 27 escaleras de madera conectadas, de una gradiente de 50 a 55 grados. En este informe se asume que las condiciones en otras galerías es similar a la galería observada; por lo tanto, debieran tener riesgos similares.
Se observó durante la visita que la mayor parte de la galería carecía de soporte, incluso en alguna área con bloques potencialmente sueltos, como se muestra en el Anexo 5 – página 5 y 6. En la página 5 del anexo dos series de diaclasas se cruzan formando cuñas potencialmente inestables. Algunos bloques de cuñas grandes ya estaban sueltas, dejando la superficie del techo y muros de la galería disparejos, como se puede observar en la foto del anexo. En la página 6 del anexo hay diaclasas mayores subhorizontales en el techo de la galería. Esta diaclasa horizontal en combinación con diaclasas verticales al azar pueden crear potenciales bloques inestables. En ambas situaciones se requiere un cuidadoso mapeo de diaclasas y análisis.
En muchas partes a lo largo de la galería, el soporte de roca es un sistema de vigas de madera, como se puede observar en el anexo 5—página 8. Parece que el sistema de vigas se usó en todo lugar donde la roca tiene calidad “extremadamente pobre”. En la mayoría de los lugares con el sistema de soporte de vigas de madera, el techo se había derrumbado. Esto puede indicar el colapso durante la construcción. La galería parece razonablemente estable con el sistema de vigas de madera hasta aquí, pero hay algunos riesgos con esta medida de soporte. La observación en la galería, especialmente en secciones húmedas o de alta humedad, es que la calidad de la madera se ha reducido significativamente. Algunos árboles pequeños se pueden observar creciendo en la viga principal de madera. Esto es indicador de descomposición al interior de la madera. En otros lugares la descomposición se puede observar con claridad, como se ve en el Anexo 5 - pg 9. Por lo tanto, el riesgo con el soporte de vigas de madera es que su longevidad se puede ver reducida mucho más rápidamente que lo esperado, especialmente en lugares con mucho agua subterránea. En esta situación, el accidente de colapso está escondido detrás de el sistema de vigas de madera y esperando a que pase el tiempo.
En otras partes de la galería, muchas grietas se han observado en el muro, como se puede ver en el Anexo 5 – pg 17. Se anticipa que puede haber grietas en el techo también, pero el equipo de inspección no tenía buena luz ni una máquina de levantamiento para poder inspeccionar el techo.
Al visitar la veta YYY, se pudo observar mucho shotcrete fallido con grietas y roca suelta que arrastra el shotcrete en la galería del nivel NV690. Algunas situaciones típicas observadas se ven en el Anexo 5 – pg 10 a 16. Shotcrete fallido y roca se observaron en un área de roca dura fracturada así como en la zona de cizalla. En el área de roca dura y fracturada, pareciera que el shotcrete no fue suficiente para soportar el peso de los bloques de roca sueltos, como se puede observar en el Anexo 5 – pg 10, en las fotos a la derecha. En la zona de cizalla, el macizo rocoso aparece como “extremadamente pobre” y las fallas pueden indicar una medida insuficiente de soporte de roca. Cabe destacar que el soporte de roca mayor es solamente shotcrete. Había pocos lugares donde había pernos sistemáticos instalados.
Con la intención de comprobar el soporte de roca instalado, la calidad del macizo rocoso se evaluó usando el sistema Q in situ, y se encontró que el valor Q va desde 0.025 a 0.05. Descripciones y valores de los cálculos de Q se han presentado en la Tabla 2. Del valor Q calculado, el sistema Q recomendaría soportar la galería con la categoría 7, que es una combinación de 12 a 15 cm de shotcrete reforzado con fibra, más pernos sistemáticos con espaciado de alrededor de 1.2 m, como se puede observar en la Figura 1.

Rebelde con causa... Irreverente, controversial y sin filtros: este chileno, productor, guionista y director de cine, suma reconocimientos. Hacemos un recorrido por sus éxitos y viajamos hasta la “ciudad de los raros”, un destino donde su creatividad encuentra una particular acogida.
Desde los 12 hasta los 17 años Nicolás escribió una columna semanal llamada “Memorias de un Pingüino”, en Zona Contacto del diario El Mercurio, con testimonios sobre cómo un nerd sobrevivió al colegio. Parte de esas historias las recreó en su primer largometraje “Promedio Rojo” y, desde entonces, no ha parado. Después de sus dos exitosas producciones “Que pena tu vida”, y la secuela, “Que pena tu boda”, ahora trabaja en “Aftershock”, un thriller producido y co-escrito por Eli Roth, director de la escalofriante saga “Hostel”, productor de “El Último Exorcismo” y actor en el film de Tarantino, “Bastardos sin gloria”.
Para Nicolás, como director, “Aftershock” –inspirada en el terremoto que sacudió a Chile en febrero del 2010- es su primera cinta en inglés, con elenco mayoritariamente norteamericano. La nueva película será financiada por Cross Creek Pictures (Cisne Negro) y Vertebra Films.
La ciudad de los raros
Una de las ciudades favoritas de Nicolás es Austin, en el Estado de Texas, Estados Unidos, y que el 2012 será uno de los escenarios de las 20 carreras del Calendario Mundial de la Formula 1.
“Austin es un paraíso dentro de Texas, la capital mundial del hipsterismo –catalogación para jóvenes de intereses alejados a los dictados por la moda y cultura predominantes-, y cuyo eslogan es Keep Austin Weird. La gente es muy amable y siempre hay bandas nuevas tocando su música a la vuelta de la manzana”, comenta Nicolás, que cuando en la ciudad no se pierde los festivales de cine Fantastic Fest y SXSW –el último, realizado en marzo, es también una celebración de música y tecnología-.
Austin es un destino que lo acerca a su arte. “Además aquí queda el cuartel general de Robert Rodríguez (Sin City, El Mariachi) y Troublemaker Studios, lugar en el que tuve el placer de trabajar cuando hice Santos (película chileno-española)”.
Para Nicolás, una opción insuperable para alojarse, o nada más pasar el rato, es el hotel-bar The Driskill, que tiene uno de los mejores brunch de la ciudad y una privilegiada ubicación céntrica, rodeado de librerías y lugares donde ver películas. Un destino impostergable para el aún joven talento que recomienda: “El Alamo Drafthouse es el mejor cine del mundo. Un lugar para ver películas comerciales, de cine-arte, extranjeras bizarras, etcétera; comiendo y tomando cerveza. Éste es, sin duda, uno de los lugares más felices del mundo”.

A rebel with a cause… Irreverent, controversial and without filter, this Chilean producer, screenwriter and film director is heaping recognition. This article follows his successes up to the city where hipsters “keep it weird,” a destination in which his creativity has found special acceptance.
From ages 12 to 17, Nicolás wrote a weekly column called “Memories of a Penguin” for the El Mercurio newspaper supplement “Zona de Contacto”, with testimonies of how a nerd survived school. Some of these stories were used in his first feature film Fail and he hasn’t stopped since. After two successful productions, F*ck My Life and its sequel F*ck My Wedding, he is now working on Aftershock, a thriller produced and co-written by Eli Roth, the director of the spine-chilling saga Hostel, producer of The Last Exorcism and actor in Tarantino’s Inglorious Bastards.
Aftershock – inspired by the earthquake that shook Chile in February, 2010 – which will be financed by Cross Creek Pictures (Black Swan) and Vertebra Films, will be Nicolás’ first time directing an English-speaking film with a mostly North American cast.
Hipster City: Keepin’ it Weird
One of Nicolás’ favorite cities is Austin, Texas in the United States, a city that in 2012 will be hosting one of the 20 races for the Formula 1 Championship.
“Austin is a paradise within Texas, the world capital for hipsterism – the category for young people who stay away from the mainstream – and whose slogan is Keep Austin Weird. People are amiable and there are always new bands playing their music around the corner,” says Nicolás, adding that he has not missed out on the city’s film festivals Fantastic Fest and SXSW – its last version was in March – and that the city is also a celebration of music and technology.
Austin is a destination that brings him closer to his art. “Plus, this is the location for Robert Rodriguez’ (Sin City and El Mariachi) headquarters and Troublemakers Studios, where I had the pleasure of working when I made Santos (Chilean-Spanish film).”
In his opinion, an unbeatable option for accommodation, or for just hanging out, is The Driskill Hotel-Bar, which has one of the best brunch menus in the city as well as a convenient centric location, surrounded by nearby bookshops and venues to movies. There is one destination recommended by the still up-and-coming talent that should not be put off: “The Alamo Drafthouse has the best film selection in the world. It is the place to go see commercial movies, art films, bizarre foreign films, etcetera, all while eating and drinking beer. This is without doubt one of the happiest places in the world.”

Estudio de Impacto Ambiental
A continuación se presentan los Permisos Ambientales Sectoriales (PAS) aplicables al Proyecto "ES".
El presente Proyecto corresponde a una modificación del Proyecto "E", que tiene como objetivo, por una parte la optimización del proceso actual y por otra la explotación, procesamiento y comercialización de minerales de cobre, oro y molibdeno desde el yacimiento "T", aledaño al yacimiento "E" actualmente en explotación.
En términos generales, el Proyecto, se ubica en las comunas de Sierra Gorda y Mejillones de la Provincia de Antofagasta y comuna de María Elena, Provincia de Tocopilla, todas ellas en la Región de Antofagasta. Dada la vasta área de influencia de las actividades del proyecto, su localización puede ser descrita considerando cuatro sectores.
La distribución de los sectores del Proyecto queda establecida de la siguiente forma:
• Sector Mina – Planta
• Sector Ductos
• Sector Muelle E
• Sector Línea de Transmisión Eléctrica (LTE) 110 kV

10.1.1.2 Permiso Ambiental Sectorial 81, Operación de equipos nucleares
i. Aplicabilidad del PAS 81
El Proyecto "ES", Sector Mina – Planta, requiere el otorgamiento del permiso ambiental sectorial que se encuentra señalado en el artículo 81 del RSEIA, ya que considera el uso de densímetros nucleares y el almacenamiento de fuentes de equipos radiactivos.
El artículo 81 del RSEIA corresponde al permiso para el emplazamiento, construcción, puesta en servicio, operación, cierre y desmantelamiento, en su caso, de las instalaciones, plantas, centros, laboratorios, establecimientos y equipos nucleares, a que se refiere el artículo 4 de la Ley Nº 18.302, Ley de Seguridad Nuclear. Para cumplir con este último, el Proyecto considera la elaboración de un procedimiento de protección radiológica, considerando todas las medidas necesarias de seguridad nuclear.
A continuación, se presentan los antecedentes necesarios para solicitar este permiso ambiental sectorial según los requisitos y los contenidos técnicos y formales establecidos en el RSEIA, los que consisten en considerar las condiciones que permitan preservar el medioambiente libre de contaminación. Para ello el Proyecto elaborará un procedimiento de protección radiológica considerando todas las medidas necesarias de seguridad nuclear, que se mencionan a continuación.
ii. Transporte de equipos radiactivos
Para el transporte de estos equipos desde el lugar de origen hasta el Proyecto, se cumplirá con el D.S. Nº 12/1985, Reglamento para el Transporte Seguro de Materiales Radiactivos, para lo cual, el Proyecto considera presentar una solicitud de autorización de transporte de los equipos y las medidas de seguridad de esta actividad.
a) Solicitud de Autorización de Transporte de equipos radiactivos
Con el fin de dar cumplimiento al D.S. Nº 12/1985, Reglamento para el Transporte Seguro de Materiales Radiactivos, y antes de iniciar la operación de los equipos, el Titular solicitará autorización de transporte a la Autoridad Sanitaria Regional. Para ello se presentarán los siguientes antecedentes:
• Antecedentes del propietario del material radiactivo
• Antecedentes de la persona responsable de la seguridad radiológica del transporte
A su vez, se presentarán los antecedentes del transporte:
• Antecedentes del conductor
• Identificación del vehículo
• Antecedentes del destinatario
• Antecedentes de la Ruta de Transporte
• Características del Material Radiactivo
• Detalle del embalaje
Adicionalmente, se presentará la siguiente documentación:
• Hoja de seguridad
• Certificado del fabricante del material radiactivo
• Copia Licencia de Operación de Responsable de Seguridad Radiológica
• Informe de Evaluación de Radiación previo al traslado, que indique que se cumple con los límites de dosis para personas ocupacionalmente expuestas.
b) Medidas de Seguridad en el Transporte de los equipos radiactivos
Siguiendo con el punto anterior, y con el fin de dar cumplimiento al D.S. Nº12/1985, Reglamento para el Transporte Seguro de Materiales Radiactivos, se tomarán las siguientes medidas de seguridad en el trasporte de los equipos empleados en las distintas etapas del Proyecto.
• El transporte se realizará en un vehículo exclusivo y autorizado para su transporte, protegido de golpes y caídas y en su contenedor original rotulado. Además contará con señalética de radiactividad.
• Antes de cargar los equipos, el responsable del transporte deberá verificar las condiciones en que se encuentra el equipo e informar inmediatamente alguna anomalía si la hubiere, como también si cuenta con la autorización correspondiente de la Comisión Chilena de Energía Nuclear para su uso y transporte. Además, deberá preocuparse que el equipo quede debidamente sujeto para evitar cualquier eventualidad.
• El personal a cargo del transporte deberá llevar una copia con toda la documentación legal referente al equipo, además de un instructivo de transporte y acciones en caso de emergencias. Al interior del embalaje se encontrará una hoja con el procedimiento de emergencia en caso de robo o pérdida del equipo (nombre de la empresa, teléfonos de emergencia, dirección, entre otros).
• El ensamblaje de equipos radiactivos, puede ser retirado siempre y cuando el equipo esté desconectado y solo el personal capacitado podrá estar presente en el lugar, cuidando de no exponerse ni exponer a otra persona directamente a la fuente, al ser retirada de su ensamblaje. Se deben considerar los atenuantes, como tiempo de exposición y distancia a la fuente radiactiva, utilizando las herramientas adecuadas para la ejecución del trabajo.
• Luego de esto, se contará con una empresa encargada de la gestión de desecho radiactivo quien será el encargado de proporcionar el embalaje adecuado para el transporte de la fuente.
Se debe tener presente que:
• El transporte de los equipos radiactivos, debe ser realizado por personas autorizadas.
• Los equipos radiactivos no debe ser transportado en la cabina del vehículo.
• Antes de iniciada la marcha el conductor deberá preocuparse que el equipo este debidamente sujeto para evitar cualquier daño o hurto.
• Nunca se debe dejar el vehículo de transporte equipos radiactivos en su interior sin una persona que lo custodie.
• Llevar una bitácora de registros con los datos de las salidas e ingresos desde el lugar de almacenamiento del equipo, día, hora, lugar de trabajo y faena, además de indicar el nombre del operador responsable.
c) Protección Radiológica para la Operación de los equipos radiactivos
Para la operación de los equipos radiactivos se cumplirá con lo establecido en la Ley Nº 18.302, Ley de Seguridad Nuclear, para lo cual, el Proyecto considera la implementación del procedimiento de protección radiológica, donde se incluyen todas las medidas necesarias de seguridad nuclear respecto a transporte, operación, almacenamiento, condiciones de emergencia, protección hacia las personas y medidas en caso de robo. El procedimiento de protección radiológica contempla los siguientes aspectos:
Operación
De acuerdo a lo establecido en el D.S. Nº 133/1984, “Autorizaciones para Instalaciones Radiactivas o Equipos Generadores de Radiaciones Ionizantes, personal que se desempeña en ellas u opere tales equipos y otras actividades afines”, del Ministerio de Salud. Las medidas adoptadas en la operación de equipos radiactivos, serán las siguientes:
• Se contará con operadores autorizados de equipos radiactivos, acreditando curso aprobado por una Institución reconocida por la Autoridad Sanitaria
• Antes de proceder a la operación, con el equipo radiactivo, se demarcará el área de trabajo con conos, letreros y el símbolo relacionado con el riesgo de la radiactividad
• Sólo el operador autorizado manipulará el equipo, permaneciendo en las proximidades sólo el personal autorizado y restringiendo el ingreso de personal ajeno a la operación
• Todo operador contará obligatoriamente con un dosímetro personal, dispositivo usado para medir la cantidad de radiación recibida, de uso exclusivo e intransferible
• No se trabajará en zonas de vehículos o maquinarias en movimiento, coordinando los trabajos con la supervisión a fin de determinar, previamente, los accesos, rutas de circulación, zonas de medición y horarios
• Se verificará que las vías de circulación en el área de operación estén expeditas
Almacenamiento
Se solicitará autorización de Almacenamiento para lo cual se presentará la Solicitud frente a la Autoridad Sanitaria Regional, además se contará con:
• Manual de uso del equipo en español
• Copia Licencia del Operador
• Informe de mediciones de radiaciones durante el almacenamiento
• Libro de registro
Los materiales radiactivos necesarios para la etapa de operación del Proyecto, tales como densímetros u otros equipos radiactivos necesarios, serán almacenados temporalmente en una bodega especialmente habilitada para estos fines, y cuya operación estará en concordancia a lo señalado en el Decreto Nº 133/1984, del Ministerio de Salud. Para lo anterior, se utilizará la instalación existente de Minera "E", la que será complementada con ocasión del presente Proyecto en caso que sea necesario. En cuanto a sus características, se mencionan las siguientes:
• La bodega tendrá que ser un recinto construido en hormigón y/o albañilería, provisto de un marco con una tapa metálica de protección con un sistema de cierre con porta candado, que lo asegure de terceras personas. Se contará con un acceso independiente y exclusivo
• Estará a lo menos cinco metros de los lugares de trabajo, permanecerá cerrada, limpia, despejada y libre de humedad
• Estará señalizada e identificada con letreros alusivos que indiquen “Precaución Material Radioactivo, sólo personal autorizado”, incluyendo el nombre y teléfono de la persona encargada de seguridad radiológica
• Para el caso de equipos portátiles, cuando éstos no sean ocupados, se guardarán dentro de este tipo de bodega y al interior de una caja metálica de fierro, la cual será destinada única y exclusivamente a contener estos equipos. Esta caja, estará provista de candados de seguridad y será en lo posible anclada al piso o pared de la bodega
• La bodega de almacenamiento será de uso exclusivo para los equipos radiactivos.
• La bodega de almacenamiento cumplirá con todas las disposiciones legales establecidas y será autorizada por la Autoridad Sanitaria Regional
Condiciones de emergencia
Si los equipos radiactivos sufren algún deterioro, se seguirá el siguiente procedimiento:
• Se cercará el área donde esté el equipo dañado a lo menos cinco metros
• En caso de accidente directo en lugares de medición, se dará aviso a Carabineros
• En caso de incendio del vehículo de transporte se dará aviso a Bomberos
• Se informará a la Comisión Chilena de Energía Nuclear (CCHEN) y ésta indicará los pasos a seguir y determinará si existe radiación
• Se informará al Servicio de Salud Regional
Además se tomarán las siguientes medidas preventivas:
• No se abandonará la fuente radiactiva
• No se tocarán materiales dañados
• El equipo sólo será movido por personal autorizado que cuente con elementos de seguridad
• Se reunirá la información legal del equipo y se recopilará la información para comenzar con la investigación de causas del accidente
Protección hacia las personas
Ante una falla o eventual daño de los equipos radiactivos, se solicitará de inmediato la presencia del responsable del instrumento, el cual evaluará la situación y tomará las medidas correspondientes, éstas van desde resolver la situación hasta solicitar la presencia de personal especializado (Comisión Chilena de Energía Nuclear).
Asimismo como medida de precaución, todo operador de los equipos radiactivos,, deberá contar con un dosímetro de radiación para detectar las fuentes de riesgos en su entorno directo.
En caso de robo
Si un caso de robo de equipos radiactivos llegase a ocurrir, los pasos a seguir serán los siguientes:
• El operador de la instalación, encargado del equipo, comunicará del hecho a la autoridad competente, Autoridad Sanitaria Regional
• Se dará el aviso correspondiente denunciando la situación ante Carabineros de Chile
• Se informará de la situación a la Comisión Chilena de Energía Nuclear
iii. Término de vida útil
Cuando el material radiactivo haya cumplido su vida útil para la utilización en el desarrollo del Proyecto, se coordinará con las instituciones competentes, Comisión Chilena de Energía Nuclear y Autoridad Sanitaria Regional, el retiro de la fuente emisora a fin de ser destinada como desecho radiactivo.
a) Desmontaje, embalaje, transporte y destino
El ensamblaje de los equipos radiactivos, puede ser retirado, una vez que éste haya sido desconectado. Sólo quienes realicen esta actividad pueden estar en el lugar, cuidando de no exponerse ni exponer a otra persona directamente a la fuente de radiactividad al ser retirada de su ensamblaje. Se contará con una empresa encargada de la gestión de desecho radiactivo quien será el encargado de proporcionar el embalaje adecuado para el transporte de la fuente.
El transporte de la fuente será realizado por personas autorizadas. Antes de cargar los equipos radiactivos, el responsable del transporte deberá verificar las condiciones en que se encuentra e informar inmediatamente alguna anomalía si la hubiere, así como si cuenta con la autorización correspondiente de la Autoridad Sanitaria para su uso y transporte. Además, deberá preocuparse que el equipo quede debidamente sujeto para evitar cualquier eventualidad. Nunca se debe dejar el vehículo de transporte con los equipos radiactivos en su interior sin una persona que lo custodie.
Finalmente, dentro de la caja se deberá colocar una hoja con el procedimiento de emergencia en caso de robo o pérdida del equipo (nombre de la empresa, teléfonos de emergencia, dirección, entre otros).
El destino de la fuente radioactiva desechada es el Centro de Estudios Nucleares de la Comisión Chilena de Energía Nuclear, ubicado en la comuna de La Reina (Región Metropolitana).

Environmental Impact Study
Following, are the Sectoral Environmental Permits (PAS) applicable to the "ES" Project.
The present project is a modification of the "E" Project, whose objective is, on the one hand, to optimize the current process, and on the other hand, the exploitation, processing and commercialization of copper, gold and molybdenum minerals from the "T" deposit, neighboring the "E" deposit currently in exploitation.
In general terms, the Project is located in the Sierra Gorda and Mejillones communes in the Antofagasta Province, and in the María Elena commune of the Tocopilla Province, all of which are in the Antofagasta Region. Due to the vast area of influence of the project’s activities, its location may be described considering four sectors.
The distribution of the Project’s sectors is established in the following manner:
• Mine-Plant Sector
• Ducts Sector
• "E" Wharf Sector
• Power Line Transmission (LTE) 110 kV Sector
The "ES" Project, Mine-Plant Sector, requires that a sectoral environmental permit be granted according to article 81 of the RSEIA as it considers the use of nuclear densimeters and the storage of radioactive equipment sources.
Article 81 of RSEIA is the permit for the placing, construction, implementation, operation, closure, and dismantling of, in this case, facilities, plants, centers, laboratories, and nuclear premises and equipment referred to in article 4 of Law Nº 18,302, Nuclear Security Law. In order to fulfill the latter, the Project considers the elaboration of a radiological protection procedure considering all the measures necessary to achieve nuclear security.
Following, is the necessary information in order to request this sectoral environmental permit according to the requirements and technical formal contents established in the RSEIA. These contents take into account the condition which allow to preserve the environment free of contamination. For this, the Project will elaborate a radiological protection procedure considering all the necessary measures for nuclear safety mentioned, as follows.
ii. Radioactive Equipment Transportation
For the transportation of this equipment from their place of origin to the Project, it shall comply with D.S. Nº121/1985, Regulations for Safe Transportation of Radioactive Materials, for which the Project considers presenting an authorization request for the transportation of equipment and safety measures for this activity.
a) Authorization for Radioactive Equipment Transportation Request
With the purpose of complying with D.S. Nº12/1985, Regulations for Safe Transportation of Radioactive Materials, and before operating the equipment, the Owner will request transportation authorization from the Regional Sanitary Authority. For this, the following background information will be provided:
• Information on the owner of the radioactive material
• Information on the person responsible for the radiological safety of the transport
In addition, the following background information on the transport will be provided:
• Information on the conductor
• Identification of the vehicle
• Information on the recipient
• Information on the Transport Route
• Radioactive Material Characteristics
• Packaging Details
Additionally, the following documentation shall be presented:
• Safety sheet
• Certificate of the radioactive material’s manufacturer
• Copy of the Operation License of the person responsible for Radiological Safety
• Radiation Assessment Report prior to transfer, indication that it complies with the limits of doses for occupationally exposed persons.
b) Security Measures in the Transport of Radioactive Equipment
Following the previous point, and with the purpose of complying with D.S. Nº12/1985, Regulations for Safe Transportation of Radioactive Materials, the following safety measures will be taken for the transportation of equipment used throughout the different stages of the Project.
• The transport will be carried out in an exclusive vehicle, authorized for this purpose, protected from blows and falls and within its original labeled container. Also, it will have radioactive signs.
• Before loading the equipment, the person responsible for the transportation must verify the equipment’s condition and immediately report any anomaly, if this were the case. He must also ensure that it counts with the corresponding authorization from the Chilean Commission of Nuclear Energy for its use and transport. Also, he is responsible for verifying that the equipment is properly fastened in order to avoid any eventuality.
• The personnel in charge of the transportation must carry a copy with all the legal documentation in relation to the equipment, in addition to a transport instruction and actions manual in case of an emergency. Inside the packaging will be a sheet with the emergency procedures in case of theft or loss of the equipment (name of the company, emergency phones, among others).
• The assembly of radioactive equipment, may be retrieved as long as the equipment is disconnected, and only trained personnel may be present at the spot, taking care of not exposing themselves nor anyone else directly to the source, when it is being retrieved from the its packaging. The mitigating elements, as exposure time and distance from radioactive source, using the proper tools to execute the work must be considered.
• After this, there will be a company in charge of managing radioactive waste who will take care of providing the appropriate packaging for the source’s transport.
It is important to keep in mind that:
• Authorized persons must carry out the transportation of radioactive equipment.
• Radioactive equipment must not be transferred in the vehicle’s cabin.
• Before starting out the driver must make sure that the equipment is properly fastened in order to avoid any damage or theft.
• The radioactive equipment must never be left in the vehicle for its transport without someone guarding it.
• Keep a log recording the information on exits and entries from the equipment storage place, day, time, work and mine site, in addition to indicating the responsible operator.
c) Radioactive Protection for the Operation of Radioactive Equipment
For the operation of radioactive equipment, the Project will comply with Law Nº 18,302, Nuclear Safety Law, for which the Project has considered the implementation of the radiological protection procedure, where all the necessary measures are included for nuclear safety with regard to transport, operation, storage, emergency conditions, protection for people, and measures in the case of theft. The procedure for radiological protection contemplates the following aspects:
Operation
According to D.S. Nº 133/1984, “Authorization for Radioactive Facilities or Ionizing Radiation Producing Equipment, personnel that work with or operate the equipment and other similar activities” from the Ministry of Health. The measures adopted in the radioactive equipment operation will be the following:
• Have authorized operators for radioactive equipment, attesting a completed and passed course at an Institution recognized by the Sanitary Authority
• Before proceeding with the operation, with the radioactive equipment, the work area will be defined and limited with cones, signs and the symbol related to radioactive risk
• Only the authorized operator will manipulate the equipment, and only authorized personnel will remain in the proximities, restricting the entry of personnel unrelated to the operation
• All operator will, by obligation, carry a personal dosimeter, a device used to measure the amount of radiation received, and of exclusive and non-transferable use
• The work will not be carried out in zones where there are vehicles or machinery in movement, and the work will be coordinated with Supervision in order to previously determine the accesses, circulation routes, measuring zones, and schedules
• The expedite condition of circulation roads in the operation area will be verified.
Storage
Storage authorization will be requested by presenting a Request to the Regional Sanitary Authority in addition to counting with:
• Equipment instruction manual in Spanish
• Operator License copy
• Report of radiation measuring during storage
• Logbook
The necessary radioactive materials for the operation stage of the Project, such as densimeters or other necessary radioactive equipment, will by temporarily stored in a storage room, specially implemented for these purposes. Its operation will be according to the indications of Decree Nº 133/1984 of the ministry of Health. For this, the existing one at Minera "E" will be used, and it will be complemented on occasion of the present Project in case it is necessary. In terms of its characteristics, the following are noted:
• The storage room will be a space built with concrete and/or brickwork, provided with a frame with a metallic protection lid with a closing system with a lock, thus keeping it safe from third persons. It will have independent and exclusive access.
• It will be at least five meters away from the work area, and it will remain closed, clean, cleared, and free from humidity.
• It will be signaled and identified with signs that indicate “Precaution: Radioactive Material. Only authorized personnel”, including the name and phone number of the person in charge of radioactive safety
• For the use of portable equipment, when these are not in use, they will be kept in this type of storage room and inside a metallic iron box, which will be used solely to keep this equipment. This box will have safety locks, and to the extent possible, will be anchored to the room’s floor or wall
• The storage room will be used solely for radioactive equipment
• The storage room will comply with all the legal dispositions established and will be authorized by the Regional Sanitary Authority
Emergency Conditions
If the radioactive equipment suffers any kind of deterioration, the following procedure will be followed:
• The area where the damaged equipment will be closed off by at least five meters
• In case of a direct accident in measuring places, the Police will be notified
• In case of a transportation vehicle on fire, the Firefighters will be notified
• The Chilean Commission of Nuclear Energy (CCHEN) will be informed radiation, and they will determine the steps to follow and whether there is radiation
• The Regional Health Service will be informed
Also, the following preventive measures will be taken:
• The radioactive source will not be abandoned
• Damaged materials will not be touched
• The equipment will only be moved by authorized personnel that counts with safety elements
• The equipment’s legal information will be collected and the information will be gathered to begin the investigation of the cause of the accident
Protection for the People
In the face of a failure or eventual damage of the radioactive equipment, the immediate presence of the person responsible of the instrument will be requested, and the corresponding measures will be taken. These go from solving the situation up to requesting the presence of specialized personnel (Chilean Commission of Nuclear Energy).
Also, as a precautionary measure, every radioactive equipment operator must carry a radiation dosimeter to detect the sources of risk in his direct environment.
In Case of Theft
If it were the case that theft of radioactive equipment occurred, the steps to follow are as stated below:
• The facility’s operator, in charge of the equipment, will communicate the occurrence to the competent authority: Regional Sanitary Authority
• The corresponding notification denouncing the situation before the Chilean Police will be made
• The Chilean Commission of Nuclear Energy will be informed of the situation
iii. End of Life Span
When the radioactive material has completed its life span for its use in the Project’s development, the retrieval of the source of emission as radioactive waste will be coordinated with the competent institutions – the Chilean Commission of Nuclear Energy and the Regional Sanitary Authority.
a) Dismantling, Packaging, Transport, and Destination
The assembly of radioactive equipment may be retrieved once it has been disconnected. Only those who perform this activity may be present in the place, making sure not to expose themselves or others directly to the source of radioactivity when removing it from its assemblage. There will be a company in charge of managing the radioactive waste and providing the appropriate packaging for the transport of the source.
Authorized persons will perform the transportation of the source. Before loading the radioactive equipment, the person responsible for the transport must verify its conditions and immediately notify of any anomaly if there were any, as well as whether it counts with the corresponding authorization from the Sanitary Authority for its use and transportation. Also, he must ensure that the equipment is properly fastened in order to prevent any eventuality. He must never leave the transportation vehicle with the radioactive equipment inside without someone there to guard it.
Finally, inside the box must be placed a sheet indicating the emergency procedure in case of theft or loss of the equipment (name of company, emergency phone, address, among others).
The destination of the discarded radioactive source is the Center of Nuclear Studies of the Chilean Commission of Nuclear Energy, located in the commune of La Reina (Metropolitan Region).
