Es raro encontrar tendones del supraespinoso normales en mayores de 50 años. Las rupturas pueden ser crónicas o agudas. Las primeras, son las más frecuentes en pacientes ancianos, en los que realizan trabajos con actividades sobre la cabeza, practican deportes de lanzamiento o presentan inestabilidad glenohumeral.
Las rupturas agudas se asocian en general a caídas, luxaciones del hombro, rupturas del tendón del bíceps asociadas a esfuerzos deportivos o accidentes y además se pueden sobreagregar a las lesiones crónicas. En los pacientes con sospecha de ruptura del manguito rotador, los objetivos de la ecografía serán:
1. Diferenciar entre ruptura de tendones del manguito y otras patologías que afectan a los mismos, a saber: cambios degenerativos o tendinopatías causados por impacto o impingement, sobreuso o depósitos cálcicos, tendinitis inflamatoria (polimialgia reumática y otras), infiltración por amiloide en pacientes en diálisis crónica, atrofia muscular y tendinosa de cualquier causa.
2. Definir los tendones involucrados. El orden de frecuencia en que se desgarran los tendones del manguito es la siguiente: supraespinoso, infraespinoso, subescapular y redondo menor (raro).
3. Establecer si las rupturas son completas o parciales en su espesor (de superficial a profundo), o completas o parciales en su ancho (de anterior a posterior en el supraespinoso)
4. Si la ruptura es de grosor parcial, establecer si el compromiso es de la superficie humeral o bursal.
5. Si existe o no un desgarro masivo. En el primer caso los extremos del tendón estarán separados por más de 5 cm. y en general en estos casos hay más de un tendón del manguito afectado.
| Translation - English Rupture of the rotator cuff tendons occurs in the critical zone of the supraspinatus tendon situated 1 cm proximal to its insertion into the greater tuberosity.
It is rare to find normal supraspinatus tendons in adults over age 50. Ruptures can be chronic or acute. Chronic ruptures are found most often in elderly patients, in people who perform overhead work activities, play sports that involve throwing or have glenohumeral instability.
Acute ruptures are generally associated with falls, shoulder dislocation, biceps tendon ruptures from sports exertion or accidents and can also overlap with chronic injuries. In patients with a suspected rotator cuff rupture, the objectives of ultrasound will be to:
1. Differentiate between the rupture of cuff tendons and other diseases that affect the same, namely: degenerative changes or tendinopathies caused by impact or impingement, overuse or calcification deposits, inflammatory tendonitis (rheumatic polymyalgia and others), amyloid infiltration in patients on chronic dialysis, muscle and tendon atrophy of any cause.
2. Identify the tendons involved. The order of frequency in which the cuff tendons are torn is as follows: supraspinatus, infraspinatus, subscapularis and teres minor (rare).
3. Determine if it is a full- or partial-thickness rupture (from superficial to deep), or a full- or partial-width rupture (from anterior to posterior on the supraspinatus).
4. If it is a partial-thickness rupture, determine if the weakness is on the humeral or bursal surface.
5. If there is a massive rupture or not. In a massive rupture, the ends of the tendon will be separated by more than 5 cm and in these cases generally more than one cuff tendon is affected.
