Verkalkungen werden hinsichtlich Morphologie, Zahl, Lokalisation und Verteilungsmuster beurteilt.
Von Bedeutung ist die Unterscheidung zwischen Verkalkungen, die intraparenchymatös liegen, und Verkalkungen außerhalb des Drüsenkörpers (Haut, Subcutis). Überdies müssen Artefakte ausgeschlossen werden.
BI-RADS unterscheidet typisch benigne Verkalkungen von solchen intermediärer oder aber höherer Wahrscheinlichkeit für Malignität.
Zu den Verkalkungen mit typisch benignem Erscheinungsbild zählen Haut- und Gefäßwandverkalkungen, Kalkmilchzysten, popcornartige, rundliche oder plumpe lineare Verkalkungen.
Zu den Verkalkungen, die von intermediärer Wahrscheinlichkeit für Malignität sind, zählen amorphe oder unscharf begrenzte Verkalkungen, die meist diffus angeordnet sind und aufgrund des unscharfen Erscheinungsbildes morphologisch nicht näher spezifiziert werden können.
Bei diesen Verkalkungen ist eine Vergrößerungsaufnahme zur Basisbildgebung nützlich. Grob heterogene Verkalkungen sind irregulär und haben > 0,5 mm Ø mit einer Verschmelzungstendenz, sind aber insgesamt kleiner als dystrophe Verkalkungen.
Die Assoziation mit Malignität ist gering, am häufigsten treten diese Verkalkungen in Arealen von Fibrose, Fibroadenom oder Trauma auf.
Zu den Verkalkungen mit höherer Wahrscheinlichkeit für Malignität zählen die fein pleomorphen und die fein linearen Verkalkungen.

Der Nachteil der FNA ist die hohe Rate (15 %) von insuffizientem Material mit der Notwendigkeit nachfolgender Biopsien (Evidenzgrad 1b).
Bei der Stanzbiopsie werden wesentlich größere Gewebsvolumina akquiriert, die Frequenz von diagnostisch insuffizientem Material ist geringer.
Ein negatives Biopsieergebnis in Zusammenschau mit einer benignen Erscheinungsform der Läsion macht eine Operation unnotwendig (Evidenzgrad 2a).
Lediglich bioptisch verifizierte Malignome, High-risk-Läsionen oder diskordante Ergebnisse erfordern eine offene Biopsie.
Diese Vorgehensweise kann den Prozentsatz von Exzisionsbiopsien bei benignen Läsionen auf < 30 % reduzieren.
Zusätzlich werden hier sowohl die Patientinnenmorbidität als auch Kosten reduziert.
Generell ist zu empfehlen, sämtliche perkutane Biopsien, unabhängig davon, ob sie zur Abklärung von Tastbefunden oder nicht tastbaren Läsionen dienen, unter bildgebender Steuerung durchzuführen.
Die Treffsicherheit und Zuverlässigkeit der Methoden ist abhängig von der Einhaltung der Untersuchungsstandards, der Erfahrung des Untersuchers und der Korrelation von Bildgebung mit histologischem Befund.
Die systematische Dokumentation der Ergebnisse ist essenziell zur Qualitätssicherung.
Durch den adäquaten Einsatz perkutaner Interventionen kann die Rate offener Biopsien reduziert werden.
Im Falle von Malignität wird ein einzeitiges operatives Vorgehen nach neoadjuvanter Chemotherapie ermöglicht.

Calcifications are assessed with regard to morphology, count, localisation and distribution pattern.
Of significance here is the differentiation between intraparenchymal calcifications and those outside of the mammary gland (skin, subcutis). Furthermore, artefacts need to be excluded.
BI-RADS differentiates typically benign calcifications from those with intermediary or even higher probability of malignancy.
Calcifications with a typically benign appearance are calcifications of the skin and vascular walls, milk of calcium cysts, popcorn-like, round or coarse linear calcifications.
Calcifications of intermediary probability of malignancy are amorphous or indistinct calcifications that usually are diffusely distributed and that for reason of their hazy appearance can not be morphologically specified.
Enlarged images in addition to basic imaging are useful in these cases.
In the case of certain distribution patterns (regional, linear or segmental), a biopsy may be considered.
Coarse, heterogeneous calcifications are irregular and are > 0,5 mm Ø with the tendency to merge with surrounding tissue, but are smaller than dystrophic calcifications.
These calcifications are hardly associated with malignancy and mostly occur in areas of fibrosis, fibroadenoma or trauma.
Calcifications associated with a higher probability of malignancy are fine pleomorphic and fine linear calcifications.

The disadvantage of the FNA is the high rate of insufficient material with the necessity of a subsequent biopsy (level of evidence 1 b).
In punch biopsy, much larger amounts of tissue is taken, which decreases the frequency of diagnostically insufficient samples.
A negative biopsy result together with a benign appearance of the lesion, make 5 surgery unnecessary (level of evidence 2a).
Only bioptically verified malignancies, high-risk lesions or conflicting results require an open biopsy.
This procedure decreases the rate of excision biopsies in benign lesions to < 30 %.
Additionally, morbidity as well as cost is reduced.
It is generally advised that all percutaneous biopsies are condcuted under imaging guidance, whether they are required for the clarification of palplation findings or non-palpable lesions.
The accuracy and reliability of these methods is dependent on the adherence to 15 examinations standards, to the experience of the examiner and to the correlation of imaging and histological findings.
The systematic documentation of results in essential for quality assurance.
The adequate use of percutaneous interventions can reduce the proportion of open biopsies.
In the case of malignancy, a one-stage surgical procedure following neoadjuvant chemotherapy becomes possible.
