Any medical examiner of the Company shall be allowed to examine the person of the Life Assured in the manner and the time the Company may reasonably require
5. TERMINATION
This Supplementary Contract shall automatically terminate:
a) If any premium on this Supplementary Contract or on the Basic Policy remains unpaid at the end of the Grace Period: or
b) on its expiry date or when the Basic Policy has surrendered, or expired, lapsed, cancelled or terminated for whatever reason, or on the policy anniversary when the Life Assured is age 65 years nearest birthday, whichever is earlier, or
c) when the Sum Assured becomes payable upon the diagnosis of a Major Illness where upon all other rights and benefits attaching to this Supplementary Contract shall be extinguished or
d) on the death of the Life Assured from any cause whatsoever.
6. BENEFIT PAYABLE
The Supplementary Group Accelerated Living Benefit Rider sum assured is payable if a major illness is diagnosed. If the Life Assured shall die subsequently, the balance of sum assured (if any) under Group Term Life Assurance is the difference of Group Term Life Assurance less Supplementary Group Accelerated Living Benefit Rider.
NOTWITHSTANDING ANYTHING IN THE BASIC POLICY, THIS SUPPLEMENTARY CONTRACT AND ITS ENDORSEMENTS to the contrary, it is hereby further stipulated that:
a) If the Life Assured commits suicide while sane or insane within one year from the date of issue of this Supplementary Contract, or from the date of any reinstatement, whichever is the later and while the Basic Policy and this Supplementary Contract are in force, the Sum Assured hereunder shall be a sum equal to the premium paid on this Supplementary Contract and no more,
b) for Insured member age between 16 to 60, the total amount of Sum Assured payable upon diagnosis of a Major Illness under this Supplementary
c) Supplementary Contract and any and all such other Supplementary Contract/Policies providing such Sum Assured or its equivalent by whatever name called issued by the Company or any other Companies shall not exceed RM350,000.
d) for Insured member age between 61 to 65, the total amount of Sum Assured payable upon diagnosis of a Major Illness under this Supplementary Contract and any and all such other Supplementary Contract/Policies providing such Sum Assured or its equivalents by whatever name called issued by the Company or any other Companies shall not exceed RM200,000,
e) payment for no more than one Major Illness shall be granted under this Supplementary Contract.
f) Any valid claim under this Supplementary Contract shall extinguish all other Supplementary Contract, other rights and all benefits, if any, under this Supplementary contract, except herein otherwise provided.
SUPPLEMENTARY GROUP PARTIAL AND PERMANENT DISABILITY
XXX Supplementary Partial and Permanent Disability (PPD) provides benefits in the event of bodily dismemberment, resulting from natural and accidental causes.
Amount of benefits
The amount of benefits provide under this PPD would be percentage of the Sum Assured under PPD according to the Schedule of Indemnities for PPD. The payment of the benefits would be made by XXXX in one lump sum upon the admission of the claim.
Risk exclusion
No benefits under PPD shall be paid if bodily injury is due to or accelerated by, directly or indirectly, wholly or partly, one of the following causes.
1. Self-destruction or any attempt thereat while sane or insane; while under the influence of drugs or intoxicating liquor;
2. War, declared or undeclared or any war like operations, revolution or direct participation in a riot, strike and civil commotion;
3. While in full time duty in the armed forces;
4. While travelling on aircraft other than a fully licensed passenger-carrying aircraft in which the employee is travelling as a fare paying passenger;
5. While engaging in dangerous sport such as aqualung diving, boxing mountaineering, motor competition, parachuting, power boating, racing other than on foot;
6. Any activities against public interest;
7. Childbirth, miscarriage, pregnancy or any complications thereof;
8. Provoked murder or assault.
The cost for PPD
An annual premium rate per RM1,000 of the sum assured, as stated in the Proposal Schedule, would be computed before the commencement date of the scheme. The computation is based on the employees/members data provided by the organisation. The annual premium rate would be revised, if necessary, on the anniversary date.
The estimated total cost for PPD, as stated in the Proposal Schedule is based on the total sum assured under the Partial and Permanent Disability.

Mana-mana pemeriksa perubatan Syarikat hendaklah dibenarkan untuk memeriksa diri Orang Yang Diinsuranskan dalam cara dan masa yang mungkin diperlukan dengan munasabah.
5. PENAMATAN
Kontrak Tambahan ini akan tamat secara automatik :
a) Jika mana-mana premium pada Kontrak Tambahan ini atau Polisi Asas masih belum dibayar pada akhir Tempoh Tenggang, atau
b) pada tarikh tamatnya atau apabila Polisi Asas telah dikembalikan, atau tamatnya, luput, batal atau ditamatkan untuk alasan apa pun, atau pada ulang tahun polisi apabila umur terdekat Orang Yang Diinsuranskan berumur adalah 65 tahun, yang mana satu terdahulu, atau
c) Apabila jumlah Insurans menjadi perlu dibayar sebaik sahaja didiagnoskan Penyakit Utama di mana semua hak dan faedah lain yang dilampirkan kepada Kontrak Tambahan ini dilunaskan atau
d) pada kematian Orang Yang Diinsuranskan daripada apa jua sebab.
6. FAEDAH PERLU DIBAYAR
Jumlah yang diinsuranskan Rider Faedah Hidup Dipanjangkan Kelompok Tambahan perlu dibayar jika penyakit utama didiagnoskan. Jika Orang Yang Diinsuranskan meninggal akhirnya, baki jumlah yang diinsuranskan (jika ada) di bawah Insurans Hayat Sementara Kelompok tolak Rider Faedah Hidup Dipanjangkan Kelompok Tambahan.
TANPA MENGAMBIL KIRA APA-APA DI DALAM POLISI ASAS, KONTRAK TAMBAHAN INI DAN PENGENDORSANNYA KEPADA YANG BERTENTANGAN, adalah dengan ini menyatakan selanjutnya bahawa:
a) Jika Orang Yang Diinsuranskan bunuh diri ketika siuman atau tidak siuman di dalam setahun daripada tarikh penerbitan Kontrak Tambahan ini, atau daripada tarikh mana-mana pengambilan semula, yang mana satu terkemudian dan manakala Polisi Asas tersebut dan Kontrak Tambahan ini berkuat kuasa, Jumlah Yang Diinsuranskan menurut dokumen ini adalah jumlah bersamaan dengan premium yang dibayar pada Kontrak Tambahan ini dan tidak lebih,
b) untuk ahli yang diinsuranskan berumur 16 hingga 60, Jumlah Amaun Yang Diinsuranskan perlu dibayar selepas didiagnoskan menghidap Penyakit Utama di bawah Kontrak Tambahan
c) Kontrak Tambahan dan mana-mana dan semua Kontrak Tambahan Lain Tersebut/Polisi menyediakan Jumlah Yang Diinsuranskan atau persamaannya dengan apa jua nama pun yang diterbitkan oleh Syarikat atau mana-mana Syarikat lain tidak melebihi RM350,000.
d) untuk Ahli Yang Diinsuranskan yang berumur 61 hingga 65, Jumlah Amaun Yang Diinsuranskan perlu dibayar selepas didiagnoskan menghidap Penyakit Utama di bawah Kontrak Tambahan ini dan mana-mana dan semua Kontrak Tambahan Lain Tersebut/Polisi menyediakan Jumlah Yang Diinsuranskan atau persamaannya dengan apa jua nama pun yang diterbitkan oleh Syarikat atau mana-mana Syarikat lain tidak melebihi RM200,000
e) Bayaran untuk tidak melebihi daripada satu Penyakit Utama akan diberi di bawah Kontrak Tambahan ini.
f) Mana-mana tuntutan yang sah di bawah Kontrak Tambahan ini hendaklah melunaskan semua Kontrak Tambahan lain, hak lain dan semua faedah, jika ada, di bawah Kontrak Tambahan ini, kecuali di dalam perjanjian ini jika tidak ia sediakan
HILANG UPAYA SEPARUH DAN KEKAL KELOMPOK TAMBAHAN
Hilang Upaya Separuh Dan Kekal Tambahan (PPD) XXX memberi faedah jika terjadi dismembermen bahagian badan, disebabkan daripada sebab semula jadi dan kemalangan.
Amaun faedah
Amaun faedah yang disediakan di bawah PPD ini adalah peratusan Jumlah Yang Diinsuranskan di bawah PPD menurut Jadual Indemniti untuk PPD. Bayaran untuk faedah akan dibuat oleh XXX sekali gus selepas penerimaan tuntutan.
Pengecualian risiko
Tidak ada faedah di bawah PPD akan dibayar jika kecederaan bahagian badan disebabkan atau dipercepatkan oleh, secara langsung atau tidak langsung, sepenuhnya atau sebahagiannya, satu daripada sebab berikut.
1. Membunuh diri atau sebarang cubaan seperti itu ketika siuman atau tidak siuman, ketika di bawah pengaruh dadah atau mabuk disebabkan arak;
2. Perang, penyertaan secara langsung dalam rusuhan, revolusi atau mogok dan kekecohan awam.
3. Ketika bertugas sepenuh masa dalam angkatan tentera;
4. Ketika berjalan dengan pesawat selain daripada sebuah pesawat membawa penumpang berlesen penuh di mana pekerja tersebut menaiki sebagai penumpang berbayar;
5. Ketika terlibat dalam sukan berbahaya seperti menyelam menggunakan alat pernafasan juruselam, bertinju, mendaki gunung, pertandingan motor, terjun menggunakan paracut, menggunakan bot berkuasa, berlumba selain daripada berjalan kaki.
6. Sebarang kegiatan yang bertentang dengan kepentingan awam;
7. Bersalin, keguguran, kehamilan atau sebarang komplikasi daripadanya;
8. Pembunuhan atau serangan diprovokasi.
Kos untuk PPD
Kadar premium tahunan setiap RM1,000 daripada jumlah yang diinsuranskan, seperti dinyatakan dalam Jadual Cadangan, akan dikira sebelum tarikh pemulaan skim tersebut. Pengiraan tersebut berasaskan pada data pekerja/ahli yang disediakan oleh organisasi tersebut. Kadar premium tahunan akan disemak, jika perlu, pada tarikh ulang tahun.
Anggaran jumlah kos PPD, seperti yang dinyatakan dalam Jadual Cadangan berasaskan pada Jumlah keseluruhan Yang Diinsuranskan di bawah Hilang Upaya Separuh dan Kekal.
