Coding of Clinical Data: 臨床データのコーディング
1. PURPOSE 目的
To define the procedures for creating and implementing a project specific Data Coding Guidelines document to ensure that coding of clinical data is performed with a high level of consistency and accuracy.

4. PROCEDURES 手順
4.1 Create Project Specific Data Coding Guidelines プロジェクト固有のデータコーディングガイドラインの作成
The Lead Coder shall create a project specific Data Coding Guidelines document.

The Lead Coder shall also consult with the Lead Safety Representative in creating the Data Coding Guidelines to ensure that the same dictionaries, versions, and coding methods are applied consistently between the two groups coding serious adverse events, if applicable.

The Lead Coder shall identify all types of data to be coded for the study by consulting with the LDM, PM, and Sponsor, as appropriate, and reviewing the scope of work, as applicable. The Lead Coder shall document this information in the Data Coding Guidelines. Types of data may include concomitant medications, adverse events, procedures, concurrent diseases, and medical histories.

The Project Manager shall communicate to the Lead Coder and the Database Administrator/Programmer the dictionaries and corresponding versions to be used for the study. The Lead Coder shall document this information in the Data Coding Guidelines.

The Lead Coder shall contact the Sponsor coding expert to determine the plan for dictionary updates, if applicable. Information regarding whose dictionaries will be used (i.e., Quintiles or Sponsor), frequency of updates, and method of updates (i.e., loading only updated terms vs complete replacement of previous dictionary) shall be discussed and documented in the Data Coding Guidelines.

The Lead Coder and the Database Administrator/Programmer shall determine and document the specific data fields that will be run against each of the dictionaries. The Database Administrator/Programmer shall use this information to set up their coding programs to ensure that the appropriate data is being used with each dictionary.

The LDM or designate shall have the Sponsor review a list of Quintiles standard guidelines for performing coding. In the event that the Sponsor does not agree with a given guideline, the LDM or designate shall document those Sponsor specifications in the project-specific Data Coding Guidelines. Alternatively, the LDM or designate may attach and reference a list of guidelines for performing coding as part of the Data Coding Guidelines, again noting any Sponsor specifications.

4.2 Program Project Specific Coding Set-up プロジェクト固有のコーディングセットアップのプログラミング
The LDM or designate shall inform the Database Administrator/Programmer that coding set-up programming is required by completing Part A of the Programming Request and Change Form (refer to CS.FM.DM112). The Database Administrator/Programmer shall produce and test a project specific program to run each dictionary against the corresponding data to be coded, including the set-up of the screens, appropriate tools, and all required listings. Once all testing is performed, the Database Administrator/Programmer shall complete Part B of the Programming Request and Change Form indicating that Informal Development Testing is complete. The LDM shall then validate the coding setup programming using dummy test data. If any errors are identified, the LDM shall record the errors on the Data Management Testing Error Log (CS.FM.DM087). The Database Administrator/Programmer and the LDM shall repeat this process until no errors are identified. The LDM shall then complete Part C of the Programming Request and Change Form authorizing its use in production and file the form and the validation documentation (signed and dated) in the Data Management Study File.

The Database Administrator/Programmer may set the running of the coding programs on a batch job schedule so that coding is run automatically or run manually, as required for the project.

If the reason for the term failing to autocode is due to a difference in spelling (e.g., European English vs US English), or the high specificity of a term (e.g., "sharp head pain"), the most appropriate code will be assigned, with reference to the project coding guidelines.

The Sponsor may provide a list of acceptable abbreviations and their definitions. The Lead Coder shall attach this list to the Data Coding Guidelines and, if one of these abbreviations is included in the list of mismatched terms, the Lead Coder shall assign a code according to the abbreviation list.

In the case where there is no obvious preferred term to be assigned to the mismatched term, the Lead Coder or designate shall issue a DCF to the investigative site for clarification. If the answered DCF indicates that a change to the verbatim term is required, the Lead Coder or designate shall make the appropriate updates in the database, as described in the Data/DCF Flow document (see CS.WI.DM069).

4.5 Document Uncoded Data

Prior to database lock, the Lead Coder shall identify those terms that remain uncoded. The Lead Coder shall contact the Sponsor to determine the appropriate action if any (e.g., Sponsor may suggest an alternative preferred term, request a new term be added to the dictionary or agree to leave term uncoded). If the Sponsor agrees to leave the term uncoded, the Lead Coder shall produce a list of all uncoded data and send it to the LDM for incorporation into the Data Handling Report (see CS.WI.DM113). However, serious adverse event data should never be left uncoded.

All uncodeable data shall remain blank rather than assigning coding terms or text such as "Reaction Unevaluable" or "Classification Pending", unless otherwise specified by the Sponsor.

4.6 Review Coded Data

On a regular basis during the conduct of the study, the Lead Coder shall review the coded data to ensure consistency and accuracy in coding across the entire database. The Lead Coder shall review the codes assigned to the verbatim for accuracy and shall review all similar terms for consistency of code assignment. This may be performed with the use of listings or by using a program which allows the database to carry out the comparison.

The Sponsor may also request medical review of the coded data by Quintiles' medical staff or at the Sponsor site by Sponsor medical staff. If data are to be transferred to the Sponsor site for review, the Lead Coder shall discuss with the Sponsor the preferred frequency and method of transferring the coded data to the Sponsor (if necessary). The Lead Coder shall document this information in the Data Coding Guidelines. The Lead Coder shall review all coded data prior to any transfer of coded data to the Sponsor.

Authorization is required by the LDM (or the GM if the LDM is the author), the Database Administrator/Programmer, and the Lead Coder (if not the author).
