Source text - English Diabetes mellitus is a disease characterized by progressive
destruction of the beta cells in the pancreatic islets of Langerhans.
The current primary treatment is changing the life style and
insulin injection. However, this therapy cannot provide sustained
physiological glycemic control. Pancreas or islet cell transplantation
would be the preferred treatment options. However, the lack of
donor tissue immunoincompatibility, cell rejection and using long
term immunosuppression are the major barriers to their widespread
use.
Mesenchymal stem cells (MSC) hold promising therapy in
regenerative medicine. MSCs are residing many adult and fetal
tissue such as in the bone marrow, adipose tissue, umbilical cord
blood, and Wharton jelly of the umbilical cord. MSCs, with selfrenewal
capacity and transdifferentiation potential are an attractive
unlimited cell source for treating diabetic patients. On the other
hand, MSCs have well characterized immunomodulatory properties
that can suppress inflammatory damage and immune-mediated
rejection and by secreting paracrine factors and deposition of the
extracellular matrix improve the engraftment of pancreatic islets
in micro-environmental niche. Here, we review the properties of
MSCs and some of the recent clinical studies using MSCs as a new
therapeutic option in the treatment of diabetic patients.
Translation - Spanish La diabetes mellitus es una enfermedad caracterizada por la destrucción progresiva de las células beta de los islotes pancreáticos de Langerhans . El tratamiento primario actual se enfoca en el cambio del estilo de vida y la inyección de insulina. Sin embargo, esta terapia no puede proporcionar control glucémico fisiológico sostenido. El trasplante de páncreas o de islotes serían las opciones preferidas de tratamiento. Sin embargo, la falta de donantes de tejido, la inmunocompatibilidad, el rechazo celular y el uso de inmunosupresores a largo plazo son los principales obstáculos para el uso generalizado de estas opciones de tratamiento.
Las células madre mesenquimales (MSC por sus siglas en inglés) son una terapia prometedora en la medicina regenerativa. Las MSC se encuentran en muchos tejidos adultos y fetales tales como en la médula ósea, en el tejido adiposo, en la sangre del cordón umbilical y en la gelatina de Wharton del cordón umbilical. Las MSCs, con su capacidad de autorenovación
y su potencial de transdiferenciación, son una fuente ilimitada y atractiva de células para el tratamiento de pacientes diabéticos. Por otro lado, las células madre mesenquimales tienen propiedades inmunomoduladoras bien definidas que pueden suprimir el rechazo por daño inflamatorio y el rechazo mediado por mecanismos inmunitarios; y mediante la secreción de factores paracrinos y la deposición de la matriz extracelular pueden mejorar la inserción de islotes pancreáticos en nichos microambientales. Aquí, repasamos las propiedades de las células madre mesenquimales y algunos de los estudios clínicos recientes que han utilizado estas células como una nueva opción terapéutica en el tratamiento de pacientes diabéticos.

Source text - Spanish Algunos datos demuestran la posibilidad de factores genéticos en el origen de la enfermedad. Al parecer el migrañoso hereda un trastorno en la respuesta nociceptiva frente a diversos estímulos ambientales (factores desencadenantes). Los sistemas de neurotransmisión, serotoninérgicos y adrenérgicos han sido clásicamente involucrados en la fisiopatología de la migraña. Los fármacos usados en su prevención son antagonistas de los receptores 5HT-2 (metisergida, ciproheptadina, etc.), o de los receptores beta-adrenérgicos (propranolol, atenolol, etc.). El inicio del ataque es un fenómeno neuronal central, asociado a vasoconstricción localizada; y el dolor propiamente dicho, es un fenómeno puramente vascular periférico, asociado a inflamación estéril.
Translation - English Some data show that there is a possibility that genetic factors may have an influence in the origin of this disease. Apparently, people with migraine inherit a disorder in the nociceptive response when it comes to different environmental stimuli (triggering factors). The neurotransmisor, serotonergic and adrenergic systems have been usually involved in the pathophysiology of the migraine. The drugs used to prevent this are the 5HT-2 receptors antagonists (methysergide, cyproheptadine, etc), or the beta-adrenergic receptors (propranolol, atenolol, etc). The beginning of the attack is a central neuronal phenomenon that is associated to localised vasoconstriction; and the pain itself is purely a peripheric vascular phenomenon that is associated to sterile inflammation.
