The aim of European Antibiotic Awareness Day is to emphasise the importance of taking antibiotics responsibly by putting an end to unnecessary use of antibiotics and encouraging people to follow their doctors’ instructions on how to take antibiotics in the appropriate way.
What are antibiotics?
Antibiotics, also known as antimicrobial drugs, are medicines that can kill or inhibit the growth of bacteria to cure infections in people, animals and sometimes plants. Antibiotics are medicines for bacterial infections (such as pneumococcal pneumonia or staphylococcal bloodstream infections); antimicrobial drugs that are effective against viruses are usually called antiviral drugs (such as those for influenza, HIV and herpes). Not all antibiotics are active against all bacteria. There are more than 15 different classes of antibiotics that differ in their chemical structure and their action against bacteria. An antibiotic may be effective against only one or multiple types of bacteria.
What is antibiotic resistance?
Bacteria have antibiotic resistance when specific antibiotics have lost their ability to kill or stop the growth of the bacteria. Some bacteria are naturally resistant to certain antibiotics (intrinsic or inherent resistance). A more worrying problem is when some bacteria, that are normally susceptible to antibiotics, become resistant as a result of genetic changes (acquired resistance). Resistant bacteria survive in the presence of the antibiotic and continue to multiply causing longer illness or even death. Infections caused by resistant bacteria may require more care as well as alternative and more expensive antibiotics, which may have more severe side effects.
What is the most important cause of antibiotic resistance?
Antibiotic resistance is a natural occurrence caused by mutations in bacteria’s genes. However, excessive and inappropriate use of antibiotics accelerates the emergence and spread of antibiotic-resistant bacteria. When exposed to antibiotics, susceptible bacteria are killed and resistant bacteria can continue to grow and multiply. These resistant bacteria may spread and cause infections in other people who have not taken any antibiotics.
What is “inappropriate” use of antibiotics?
When you use antibiotics for the wrong reason: most colds and flu are caused by viruses against which antibiotics are NOT effective. In such cases, you won’t improve your condition by taking antibiotics: antibiotics don’t lower fever or symptoms like sneezing.
When you use antibiotics incorrectly: if you shorten the duration of treatment, lower the doses, don’t comply with the right frequency (taking the drug once a day instead of 2 or 3 times a day as directed), you won’t have enough drug in your body and the bacteria will survive and may become resistant.
Which diseases are caused by resistant bacteria?
Multidrug-resistant bacteria can cause a wide range of infections: urinary tract infection, pneumonia, skin infection, diarrhoea, bloodstream infection. The location of the infection depends on the bacteria and the patient’s condition.
Patients in hospitals are at risk for infections unrelated to the reason for admission, including bloodstream and surgical site infections like MRSA (caused by Staphylococcus aureus resistant to methicillin, an antibiotic representative of those which are usually effective against Staphylococcus aureus), bloodstream infections caused by Enterobacteriaceae producing ESBL (extended-spectrum beta-lactamases, enzymes able to destroy some antibiotics), heart valve infections caused by Enteroccocci resistant to vancomycin, and surgical site and wound infections caused by Acinetobacter baumannii resistant to carbapenems. | Translation - French

Qu’est-ce qu’un antibiotique ?
Les antibiotiques, également appelés médicaments antimicrobiens, sont des médicaments capables d'inhiber la croissance des bactéries ou de les tuer afin de traiter une infection, que ce soit chez l’homme, chez l’animal et parfois chez les plantes. Les antibiotiques sont destinés au traitement des infections bactériennes (par exemple, les pneumonies à pneumocoques ou les infections à staphylocoques). Les médicaments destinés à combattre les virus sont généralement appelés des antiviraux (par exemple, pour la grippe, le VIH ou l’herpès). Tous les antibiotiques ne sont pas actifs contre toutes les bactéries. Il existe plus de 15 classes d’antibiotiques qui diffèrent de par leur structure chimique et leur mode d’action contre les bactéries. Un antibiotique peut s’attaquer à une seule bactérie ou à différents types de bactéries.
Qu’est-ce que la résistance aux antibiotiques ?
Les bactéries sont dites résistantes aux antibiotiques lorsque les antibiotiques spécifiques ne sont plus en mesure d’inhiber la croissance des bactéries ou de les tuer. Certaines bactéries sont naturellement résistantes à certains antibiotiques : c’est ce que l’on appelle la résistance intrinsèque ou inhérente. Le problème devient inquiétant lorsque des bactéries normalement sensibles aux antibiotiques deviennent résistantes en raison de modifications génétiques : on parle alors de résistance acquise. Les bactéries résistantes survivent malgré la présence des antibiotiques et continuent à se multiplier, prolongeant la durée de la maladie, voire entraînant le décès. Les infections dues à des bactéries résistantes exigent plus de soins et le recours à d’autres antibiotiques plus chers et pouvant entraîner des effets secondaires plus sévères.
Quelle est la principale cause de la résistance aux antibiotiques ?
La résistance aux antibiotiques est un phénomène naturel dû à des mutations au niveau des gènes des bactéries. Cependant, le recours abusif et inapproprié aux antibiotiques accélère l’émergence et la propagation des bactéries résistantes aux antibiotiques. Lorsqu’elles sont exposées aux antibiotiques, les bactéries sensibles sont tuées, alors que les bactéries résistantes peuvent continuer à se développer et à se multiplier. Ces bactéries résistantes peuvent alors se propager et provoquer des infections chez d’autres personnes n’ayant pas pris d’antibiotiques.
Qu’est-ce qu’un usage « inapproprié » des antibiotiques ?
C’est lorsque l’on utilise des antibiotiques à tort : la plupart des rhumes et des cas de grippes sont dus à des virus contre lesquels les antibiotiques ne peuvent RIEN. Dans de tels cas, la prise d’antibiotiques n’entraîne pas d’amélioration de l’état de santé. En effet, les antibiotiques ne font pas baisser la fièvre et n’atténuent pas les symptômes tels les éternuements.
C’est aussi lorsque l’on utilise mal les antibiotiques : lorsque l’on arrête le traitement trop tôt, lorsque l’on diminue les doses, lorsque l’on ne respecte pas la posologie (par exemple, en prenant le médicament une fois par jour au lieu de 2 ou 3 comme le médecin l’a prescrit). Dans ce cas, il n’y a pas assez de médicament dans l’organisme, ce qui permet à la bactérie de survivre et de développer une résistance.
Quelles maladies sont dues à des bactéries résistantes ?
Les bactéries multi-résistantes peuvent provoquer des infections très diverses : infection des voies urinaires, pneumonie, infection cutanée, diarrhée, septicémie. Le siège de l’infection dépend des bactéries et de l’état de santé du patient.
Chez les patients hospitalisés, il existe un risque d’infection indépendant du motif de leur admission, notamment : septicémie ou infection des plaies par Staphylococcus aureus résistant à la méthicilline (antibiotique représentatif de la classe d’antibiotiques normalement efficaces contre Staphylococcus aureus), septicémie à entérobactériacées produisant de la bêta-lactamase à spectre élargi (enzymes capables de détruire certains antibiotiques), infections des valves cardiaques dues à des entérocoques résistants à la vancomycine et infections des plaies dues à Acinetobacter baumannii résistante aux carbapénèmes. |
