von Willebrand factor (vWF) is a clotting protein in the blood. It is produced in the cells that line blood vessels and then is released into the blood stream. Not all vWF is released at one time; about one-third is released, and the remaining two-thirds is kept in reserve in the walls of the blood vessels. vWF is composed of small chains or subunits that combine into larger units called multimers. The large multimeric forms help the blood to clot and work in the clotting process. vWF has two functions. One is to bind factor VIII, the clotting protein that is low in the most common form of hemophilia. When factor VIII is bound to vWF, it is protected from being destroyed in the blood. The second function of vWF is to bind to platelets and enable them to function normally in making a platelet plug and clot.
Why Do People with von Willebrand Disease Have Bleeding Problems?
Bleeding occurs when blood vessels are damaged, allowing blood to escape through holes in the blood vessel walls. A healthy body acts quickly to stop the bleeding; this process is called hemostasis. As shown in Figure 1, the first step in hemostasis is the narrowing, or constriction, of the blood vessel so that blood loss is decreased. In the second step, special blood cells called platelets cling to the damaged blood vessel wall at the site of injury.

Von Willebrand factor (vWF) is een stollingseiwit in het bloed. Het wordt in de cellen van de bloedvaatwanden geproduceerd en vervolgens in het bloed afgegeven. Niet alle vWF wordt in één keer vrijgegeven; ongeveer één derde wordt afgegeven en de overige twee derden worden als reserve in de wanden van de bloedvaten opgeslagen. vWF bestaat uit korte ketens of subsegmenten die zich tot grotere eenheden samenvoegen, de zogenoemde multimeren. Deze grote multimeren helpen bij de stolling van het bloed en komen tijdens het stollingsproces in actie. vWF heeft twee functies. De eerste functie is het binden van VIII, het stollingseiwit dat bij patiënten met de meest gangbare vorm van hemofilie in heel lage concentraties in het bloed aanwezig is. Wanneer VIII zich aan vWF bindt, kan het niet meer worden vernietigd in het bloed. De tweede functie van vWF is de binding van bloedplaatjes. vWF zorgt ervoor dat de bloedplaatjes een afsluiting en bloedprop kunnen vormen.
Waarom hebben patiënten die lijden aan de ziekte van Von Willebrand bloedstollingsproblemen?
Een bloeding treedt op als vloedvaten beschadigd raken, waardoor het bloed door gaten in de bloedvaatwand naar buiten kan treden. Een gezond lichaam reageert snel om de bloeding te stoppen; dit proces wordt hemostase genoemd. Zoals weergegeven in afbeelding 1 wordt het bloedvat in de eerste stap van hemostase vernauwd of samengetrokken, zodat het bloedverlies kan worden verkleind. In de tweede stap hechten zich speciale bloedcellen, de zogenoemde bloedplaatjes, aan de beschadigde bloedvatwand op de plaats van de wond.
