Dear Patient,
Thank you for agreeing to participate in the “FUTURE” study!
In this study we compare two established therapies on how well they work in patients with newly diagnosed metastatic colorectal cancer. The therapies that will be compared are UFOX, a combination of UFT with oxaliplatin and leucovorin where two of the drugs are given per oral route, and FOLFOX-4, a combination of 5-FU with oxaliplatin and leucovorin, where all drugs are administered intravenously. Both of these chemotherapies will be combined with cetuximab, which is an antibody that binds to a molecule on cancer cells and thereby inhibits the growth of the cancer cell.
The purpose of the study is to compare:
• The influence of the treatment on the progression of colorectal cancer;
• How fast and how long there is a response on the treatment;
• What the effect of the treatment is on the rest of the body, and
• The impact of the treatment on your daily living and quality of life.
Your participation is of great value as it contributes to a better understanding of the treatment of cancer and with that improves the quality of the care of cancer patients like yourself.
This diary is a very important part of the study and gives valuable information about the treatment impact on your social daily living. The diary is composed of two parts, which keep track of any medical visits you may attend due to your illness when you are at home (not in hospital) and how your work and daily life is affected by your illness. The first part needs to be completed at the day of your baseline visit while part II needs to be filled in throughout the study. If you have been given UFOX treatment (oral route) you will receive an additional part III of the diary. In this part you should write down the number of capsules and tablets of the study medication you take and the time of day when you take them.
This Diary should be filled in during the entire period of study treatment. You have to bring back the diary with you at each visit to the hospital.
NB: Remember to bring this Diary to every visit.

Σας ευχαριστούμε πολύ που συμφωνήσατε να συμμετάσχετε στη μελέτη “FUTURE”!
Σε αυτήν τη μελέτη, υποβάλλουμε σε σύγκριση δύο καθιερωμένες θεραπευτικές προσεγγίσεις, με αντικείμενο την αποτελεσματικότητά τους σε ασθενείς με πρόσφατη διάγνωση μεταστατικού ορθοκολικού καρκινώματος. Αυτό το Ημερολόγιο αποτελεί ένα πολύ σημαντικό τμήμα της μελέτης και παρέχει πολύτιμες πληροφορίες σχετικά με την επίδραση της θεραπείας στην καθημερινή σας κοινωνική ζωή. Το Ημερολόγιο αποτελείται από δύο μέρη, στα οποία καταγράφονται όλες οι ιατρικές επισκέψεις που πραγματοποιείτε εξαιτίας της νόσου σας ευρισκόμενοι στο σπίτι (και όχι νοσηλευόμενοι στο Νοσοκομείο) καθώς και τον τρόπο με τον οποίο η εργασία και η καθημερινή σας ζωή επηρεάζονται από την ασθένειά σας. Το πρώτο μέρος πρέπει να συμπληρωθεί την ημέρα της αρχικής σας επίσκεψης, ενώ το δεύτερο μέρος πρέπει να συμπληρώνεται σε όλη τη διάρκεια της μελέτης. Εάν έχετε υποβληθεί σε θεραπεία UFOX (από του στόματος αγωγή), θα σας διανεμηθεί ένα επιπλέον Μέρος ΙΙΙ. Σε αυτό, θα πρέπει να καταγράψετε τον αριθμό των καψουλών και των δισκίων της αγωγής της μελέτης, που λαμβάνετε, καθώς και την ώρα της ημέρας που τα λαμβάνετε.

.01 Το φίλτρο απιονισμένου νερού (φίλτρο 1) βρίσκεται στο εσωτερικό της δεξαμενής απιονισμένου νερού, στη θέση που απεικονίζεται.
02 Το φίλτρο των καθετήρων αντιδραστηρίων (φίλτρο 2) βρίσκεται εκτός της δεξαμενής.
.03 Εάν συσσωρευτούν κατάλοιπα σε ένα από τα δύο φίλτρα, τα αποτελέσματα της ανάλυσης επηρεάζονται δυσμενώς.
.04 Καθαρίζετε τα φίλτρα τουλάχιστον μία φορά το μήνα.
.05 Όταν προγραμματίζετε τον καθαρισμό της δεξαμενής απιονισμένου νερού, μπορείτε να πραγματοποιήσετε και τους δύο καθαρισμούς ταυτόχρονα..
.06 Για το σκοπό αυτό, συγκεντρώστε τα εξής εξαρτήματα και βοηθητικά σκεύη:
.07 Μία τετράγωνη ρηχή σκάφη νερού.
.08 Έναν υπερηχητή πληρωμένο με απιονισμένο νερό.
.09 Και ειδικές πετσέτες από χαρτί ή ένα στεγνό πανί καθαρισμού.
1.0 Προτού αφαιρέσετε τα φίλτρα, θέστε τη συσκευή σε λειτουργία Stop.
2.0 (ΚΕΝΟ)
3.0 Για να συλλέξετε τα περιττά υγρά, τοποθετήστε τη σκάφη στο πάτωμα, έτσι που να βρίσκεται κατά το ήμισυ κάτω από τη συσκευή.
4.0 Τραβήξτε το συρτάρι με τη δεξαμενή απιονισμένου νερού προς τα έξω, για να αποκτήσετε πρόσβαση στις συνδέσεις των σωληνώσεων.
5.0 Πιέστε το κουμπί στο επάνω μέρος του συνδετήρα, ο οποίος βρίσκεται στο κάτω μέρος της δεξαμενής, και αποσυνδέστε τον πλαστικό σωλήνα.

8.2 Αφαιρέστε το ένθετο συγκράτησης του φίλτρου 2 (στην αντίθετη πλευρά της πλαστικής θήκης).
9.0 Αφαιρέστε από τη δεξαμενή το σωλήνα με το
απιονισμένο νερό.
9.1 Ξεβιδώστε τη θήκη του φίλτρου 1 από το άκρο του σωλήνα.
9.2 Αφαιρέστε το φίλτρο 1.
10.0 Τοποθετήστε τα δύο φίλτρα και το ένθετο συγκράτησης του ενός μέσα στον υπερηχητή, ο οποίος θα πρέπει να περιέχει απιονισμένο νερό.
10.1 Υποβάλλετε τα εξαρτήματα σε υπερήχηση για δέκα λεπτά.
10.2 Εάν δεν υπάρχει διαθέσιμος υπερηχητής, καθαρίστε προσεκτικά με απιονισμένο νερό και με μία οδοντόβουρτσα.
10.3 Σκουπίστε τον κυκλικό ελαστικό δακτύλιο με μία πετσέτα.
10.4 Σκουπίστε τις θήκες των φίλτρων.
