Xxxxx Diagnostics has appointed two internationally renowned soft tissue and bone pathologists as expert reviewers for those Xxxxx physicians and pathologists within the Capio and Xxxxx Diagnostics service areas of Europe who desire a second opinion. They have vast experience with problematic cases within the realm of tumour pathology as well as non-neoplastic musculoskeletal conditions.
From September 1, 2005, Professor Xxxx Xxxxx and Professor Xxxxx Xxxxx have agreed to act as expert consultants for Xxxxx / Xxxxx Diagnostics. They have a great deal of experience dealing with difficult bone and soft tissue lesions, including paediatric soft tissue and solid tumours (excluding haematological and brain tumours) and mesenchymal lesions in visceral organs (including GIST) as well as carcinomas, melanomas and haematological malignancies that may mimic sarcomas. Both have a broad background in general surgical pathology as well as a depth in several subspecialty areas.
Both are currently professors of pathology at Göteborg University and senior pathology consultants at Sahlgrenska University Hospital in Göteborg, Sweden. From September 1, 2005 they will take up posts as Senior Histopathology Consultants at the Royal Orthopaedic Hospital NHS in Birmingham, England and will have academic affiliated chairs at the University of Birmingham. The Royal Orthopaedic Hospital is the largest centre for diagnosis and treatment of soft tissue and bone tumours in the United Kingdom.

Ellos tienen amplia experiencia en casos difíciles dentro del ámbito de la patología tumoral como también en enfermedades osteomusculares no neoplásicas.
A partir del 1 de Septiembre del 2005, la Profesora Xxxxx Xxxxx y el Profesor Xxxxx Xxxxx han accedido a actuar como Especialistas a nombre de Xxxxx / Xxxxx Diagnostics. Ellos tienen amplia experiencia en el tratamiento de lesiones óseas y de tejido blando complicadas, incluyendo tejido blando pediátrico y tumores sólidos (excluyendo tumores hematológicos y cerebrales) y lesiones al mesénquima en vísceras (incluyendo GIST) así como carcinomas, melanomas y entidades hematológicas malignas que aparecen como sarcomas. As far as I am aware, this is the first time that professionals in the fields related to Transfusion Medicine have joined forces in order to write guidelines on alternatives to Transfusion Medicine. In other countries, rather scanty and incomplete guidelines have been produced from either a limited group of specialists or from the Departments of Health or higher authorities. Our Spanish colleagues are to be congratulated for joining forces as a multi-disciplinary group of intensivists, anaesthetists, haematologists, haemotherapists, and coronary care specialists in order to write this excellent compendium of the state-of-the-art knowledge in alternatives to Transfusion Medicine followed by recommendations in each instance. Recommendations are given in accordance with the levels of evidence accrued from extensive examination of the available bibliography.
Fulfilling the needs for safe allogeneic blood transfusion is becoming increasingly difficult world-wide. Many countries with high, medium and low development indices are struggling to meet demands for blood. It is becoming more and more difficult to find voluntary, altruistic donors who will give blood repeatedly and faithfully; donor exclusion criteria are becoming more and more stringent; processing and testing procedures are becoming more and more complex and expensive and the regulatory environment is becoming increasingly vexing. In addition, the media and commercial companies take a great interested in raising awareness of the risks of transfusion-transmitted infections and, occasionally, of other risks of transfusion. Nevertheless, very few countries have tackled, as a nation, the appropriateness of transfusion and, especially, the availability of alternatives to allogeneic blood transfusion which, in many cases, might be more cost effective and safer measures than the transfusion of allogeneic blood. It is laudatory and most timely and opportune that Spain, as a nation, is being the first country to approach the subject of alternatives to allogeneic blood transfusion in a professional, objective and scientific way. The authors state clearly, in an unambiguous manner that the evidence for the alternatives is not clear-cut. However we should, at the very least, be reminded that the evidence basis that transfusion itself is beneficial to patients is very slim.
The chapters themselves are very succinct, easy to read and with well selected, up-to-date references. The authors are not prescriptive and leave the clinicians/readers to decide what is the best option for their patients, well armed with all available evidence. Hasta dónde yo sé, esta es la primera vez que profesionales del área de la Medicina Transfusional han aunado esfuerzos para escribir directrices sobre alternativas a la Medicina Transfusional. En otros países se han producido, ya sea por grupos pequeños de especialistas, por el Ministerio de Salud o autoridades de mas rango, directrices que han sido insuficientes e incompletas. Nuestros colegas Españoles merecen ser felicitados por haberse reunido formando un equipo multidisciplinario de intensivistas, anestesistas, hematólogos, hemoterapeutas y especialistas en afecciones coronarias para escribir este excelente compendio que incluye todo el conocimiento de vanguardia en lo que se refiere a alternativas a la Medicina Transfusional, seguido por recomendaciones para cada caso. Las recomendaciones se dan de acuerdo al nivel de evidencia adquirido luego de una revisión a fondo de la bibliografía disponible.
Se está volviendo cada vez mas difícil satisfacer la necesidad en el mundo, de sangre alogénica segura para transfusión. Muchos países con índices de desarrollo alto, mediano y bajo están luchando para poder satisfacer la demanda de sangre. Es cada vez más difícil encontrar donantes voluntarios y altruistas, que volverán a donar en forma fiel y reiterada, el criterio de exclusión del donante se ha vuelto cada vez mas estricto; los procedimientos para procesar y los exámenes se han vuelto cada vez mas complejos y onerosos y el medio ambiente regulador progresivamente mas exasperante. Además, los medios de comunicación y las empresas comerciales tienen mucho interés en incrementar la percepción sobre los riesgos de las infecciones transmitidas a través de transfusiones y ocasionalmente acerca de otros riesgos de la transfusión. Sin embargo, muy pocos países han afrontado como Nación la pertinencia de la transfusión y especialmente, la disponibilidad de alternativas a la transfusión de sangre alogénica que, en muchos casos pueden ser medidas más rentables y mas seguras que la transfusión de sangre alogénica. Es encomiable y oportuno que España, como nación sea el primer país en abordar el tema de alternativas a la transfusión con sangre alogénica, en forma profesional, objetiva y científica. El autor establece claramente y de forma inequívoca que la evidencia para las alternativas no es totalmente clara. Sin embargo, deberíamos por lo menos recordar asimismo, que la base de evidencia de que la transfusión como tal es beneficiosa para los pacientes, es muy escasa.
Los capítulos mismos son muy concisos, fáciles de leer y con referencias bien seleccionadas y actualizadas. Los autores no son prescriptivos y dejan al clínico/lector que decida por si mismo cuál es el mejor tratamiento para sus pacientes, bien aperado y con toda la evidencia disponible. Este es un buen ejemplo de una buena defensa parala libertad clínica.
