SICKNESS CERTIFICATION
You must ensure the Company is notified of the reason of your absence by 7am on the first day of that absence. If the period of absence is for seven contious days or less due to sickness or injury, you must report to your Supervisor /Manager immediately on return to work and complete a Self Certification Form (S.C.2).
Failure to notify the Company on the first day of absence and to satisfactorily completing an S>C form could result in payment from the Company’s scheme if relevant, and Statutory Sick Pay Scheme being withheld.
In the event of that absence exceeding seven continous days due to sickness or injury, you must submit a Medical Certificate as soon as possible. Thereafter, further certificates must be submitted covering all absence until you resume work, at which time you will only be allowed to recommence employment on the submission of a Final Certificate.
Should you fail to complete the Self Certification form, or provide false information, or fail to supply Medical Certificates for any absence exceeding seven continuous days, and then you could have disciplinary action taken against you.

CERTIFICAÇÃO DE DOENÇA
Tem de se assegurar de que a empresa é notificada da razão da sua ausência pelas 7h da manhã do primeiro dia da sua ausência. Se o período de ausência for de sete dias consecutivos ou inferior, devido a doença ou acidente, tem de se apresentar imediatamente ao seu Supervisor/Director aquando voltar ao serviço e completar um Formulário de Auto-certificação (S.C.2).
A ausência de notificação à Empresa no primeiro dia de ausência e o preenchimento satisfatório de um formulário S>C pode resultar no pagamento do subsídio da empresa, se necessário, e o regime de subsídio de doença será retirado.
Em caso de a ausência exceder sete dias consecutivos devido a doença ou acidente, deve remeter, tão cedo quanto possível, um Atestado Médico. Consequentemente, os atestados seguintes deverão ser submetidos de forma a cobrirem toda a ausência até retomar o trabalho, altura em que apenas será autorizado a reiniciar funções mediante o envio de um Certificado Final.
Caso não complete o formulário de auto-certificação ou forneça informações falsas ou não entregue os atestados médicos por uma ausência superior a sete dias seguidos e assim pode ficar sujeito a acção disciplinar.

Professional Background:
Since 2000, I have been working for the Portuguese Ministry of Foreign Affairs in Lisbon, where I translate all kind of documents: agreements, speeches, press releases, letters, embassy's notes, international reports, etc. I'm responsible for the translations into and from Portuguese.
Since June 2002 I'm also responsible for the translations of the Secretary of State for the Portuguese Communities.
Presently I'm attached to the Department of Legal Affairs of the Portuguese Ministry of Foreign Affairs -I can guarantee you my acquainted know-how and the support of lawyers and legal counsellors - translation of all kind of texts, such as international and bilateral agreements, rogatory letters, legal pieces, reports and other related material.
Outside the Ministry I have been working as freelance translator. I regularly work with some translation and travel agencies and privates.
