Study Design:
This study is a prospective, single center, non-blinded study to evaluate the safety and effectiveness of the Surgical Mesh for reinforcement of annular defects post-discectomy.
Ten (10) patients will be enrolled in the study and followed at 6 weeks, 3 months, 6 months and 12 months, and then annually through 2 years post-op.
Study Justification:
This study is planned to collect initial information on use of the Surgical Mesh to reinforce soft tissue, specifically annular tissue, where weakness exists.
Safety will be evaluated to demonstrate acceptable levels of device-related and procedure-related complications.
Effectiveness will be evaluated by examining the acute and long-term effects on reherniation, disc height, pain scores and patient function after discectomy.
Sites:
One (1) site in Mexico
Patients:
Ten (10) patients who are candidates for lumbar discectomy will be enrolled in this study.

Introduction
Lumbar discectomy is the most frequently performed operative spine procedure in the U.S., estimated at 300,000 patients each year1, 2.
MRI studies have shown that 20% of patients experience recurrent disc herniation 6 months following lumbar discectomy3.
Reported clinical outcomes of surgical discectomy have also shown comparable success rates ranging from 40% - 90%4.
The unhealed annulus defect created via an annulotomy is considered to be a factor in the lower success rates.
A variety of techniques to close the annular defect following lumbar discectomy to prevent recurrence have been described in the literature.
The Surgical Mesh is intended to repair the annular defect made by annulotomy during discectomy procedure for patients with symptomatic lumbar disc herniation who have not responded to non-operative therapies.
The Surgical Mesh is a cylindrical polymer implant with a bioresorbable anchor integral to the device, provided as a sterile, single use device.
The device is implanted via the standard posterior discectomy approach with a minimum discectomy.
The Surgical Mesh is inserted through the aperture of the annular defect using the provided delivery device.
The mesh is secured on the intra-discal posterior annular wall with an anchor.
The implant may be trimmed after deployment to the appropriate length using standard surgical instruments.

El presente es un estudio no ciego, unicéntrico y prospectivo para evaluar la seguridad y eficacia de la malla quirúrgica en el refuerzo de defectos anulares luego de una discectomía.
En el estudio de inscribirán diez (10) pacientes con un seguimiento a las 6 semanas, 3 meses, 6 meses y 12 meses, y luego una vez al año durante 2 años después de la operación.
Fundamentación del estudio:
El presente estudio fue diseñado para recabar información inicial sobre el uso de la malla quirúrgica para reforzar los tejidos blandos, específicamente el tejido anular, donde exista debilidad.
Se evaluará la seguridad para demostrar niveles seguros de complicaciones relacionadas con el dispositivo y con el procedimiento.
Se evaluará la eficacia mediante la examinación de los efectos agudos y a largo plazo en la recurrencia de la hernia, altura discal, intensidad del dolor y función del paciente luego de una discectomía.

La discectomía lumbar es el procedimiento operatorio vertebral practicado con mayor frecuencia en los EE. UU., en un número estimado de 300 mil pacientes al año1, 2.
Los estudios de resonancia magnética (RM) han demostrado que el 20% de los pacientes experimentan hernia discal recurrente 6 meses después de una discectomía lumbar3.
Los resultados clínicos informados sobre discectomía lumbar también han mostrado tasas de éxito comparables de 40% a 90%4.
Uno de los factores considerados en las tasas de éxito bajas es el defecto anular no cicatrizado creado por una anulotomía.
En la literatura se ha descrito una variedad de técnicas para cerrar el defecto anular luego de una discectomía lumbar para evitar la recurrencia.
La malla quirúrgica fue diseñada para reparar el defecto anular causado por anulotomía durante el procedimiento de discectomía en pacientes con hernia discal lumbar sintomática que no han respondido a terapias no operatorias.
La malla quirúrgica es un implante de polímero cilíndrico con un anclaje bioabsorbible integrado al dispositivo, que se presenta en forma de dispositivo esterilizado de uso único.
El dispositivo se implanta mediante el procedimiento estándar posterior a la discectomía con una discectomía mínima.
La malla quirúrgica se inserta por la abertura del defecto anular utilizando el dispositivo de colocación que se suministra.
La malla se sujeta con un anclaje a la pared anular posterior intradiscal.
El implante puede ser cortado a la medida adecuada luego de su colocación, utilizando instrumentos quirúrgicos estándares.
