Source text - English FROM THE EDITOR’S DESK Tribute to Sam It is a great pleasure to be the new editor of the CFW newsletter. In short, I will tell something about myself. My name is Ella Weggen and I am a 20 years old student of Political Science in Amsterdam, the Netherlands. My brother, Rick, had Cystic Fibrosis, but unfortunately he died last year. My father has always been active with CFW, doing so many good things for people who suffer from CF. For this reason, I told him that if there is anything I could do, that I would love to. I think that the spread of information and the awareness of CF in the world are very important. I hope I will be able to provide this. Being able to provide this Newsletter was not an easy task for me, since I don’t have any experience doing this. I have deep respect for my predecessor Samantha Hillyard, who has done an amazing job as the editor of the Newsletter. That is why I would like to tribute my first Newsletter to her. During the conference in Birmingham, I met a lot of people and this was amazing. A lot of people offered me their help. I would like to thank everyone. I would like to point out my special gratitude to the Assistant Editors; Georgia Brown, Dace Shugg, Malina Boone, Jill Weinstein and my father, Herman Weggen. Over the years, CFW has changed a lot. Source text - English How Much Enzymes? Hello, I am writing from Lebanon and I have quite a struggle with CF since my late diagnosis@18. My question here is that I don't know how MAILBAG many enzymes to take per meal because my stool test is negative despite my malnutrition state. 21 years, 156 cm, 43 kg only. Docs weren't able to make diagnosis before because of false negative sweat chloride test and inexperience with the disease. The only enzyme available here is Kreon 10000. I need some help with this one. Posted - Apr 08 2004: 06:32:22 Dear Friend from Lebanon, Thank you for your question. This is actually quite difficult to answer because everyone has different enzyme requirements. However, your individual dose requirement can be worked out according to your symptoms. Pancreatic enzyme supplements are required in cystic fibrosis mainly because of their lipase content (the fat digesting enzyme). Therefore your Kreon needs to be taken with all meals, snacks and drinks that contain fat. The more fat in the food/drink contain, the higher the dose of Kreon required. For example, fried fish and chips would need a higher dose than roast chicken and vegetables. Unfortunately, it is impossible for me to say what your doses are, as everyone is different. It is best to start with a low dose and gradually increase the dose according to the fat content of what you are eating and the control of symptoms indicating fat malabsorption. You should aim to only have your bowels opened once to twice daily and stools should be a normal darkish colour and not pale, greasy or fatty looking. If you are cautious in increasing your dose of Kreon you should not end up taking too many. The most common symptom of taking too many is constipation and abdominal pain. In the UK, we advise that the intake of Kreon should not exceed one capsule per kilogram body weight. This would mean a maximum of 43 Kreon per day for you. Many patients however do not need to take as many as this and your deal dose should be worked out as I have described above. I have plotted your growth measurements on an English growth chart. From this I can see that she is a long baby and that her weight gain has been normal over the last two months. This all indicates that at the moment she is receiving adequate nutrition for growth, which is very good. At her age I would expect her to be drinking at least 600mls of formula milk a day, which from your figure she is. I would also expect that she should be having food (of the type you describe) three times daily. As she gets older, her food intake should increase rather than her milk intake so she moves towards the type of diet a young child would have. If her weight gain slows down the most important thing would be to make sure that she is digesting dietary fat adequately. You do not mention whether she takes pancreatic enzyme supplements, however presuming she does the dose should be adjusted so that she has normal formed stools that are not greasy or fatty. If her weight gain is still poor (which it isn't at the moment) you could try adding some olive oil to her food, as you mentioned. One teaspoon (5cc) per food item will be enough. Remember that this is 100% fat and therefore if your little girl takes enzymes the dose will have to be increased if olive oil is added. Source text - English Close Up on a Country Bulgaria Annie Kufardjieva, Guergana Petrova Introduction Bulgaria is a small (110,994 sq km) country in Eastern Europe on the Balkan Mountains, bordered in the east by the Black sea. Bulgarian residents enjoy the marvelous nature and the continental climate. The major cities are Plovdiv, Varna, Burgas and Sofia, the capital. Founded in the year 681, Bulgaria served as the major participant in the Balkan political life for seven centuries. Then, the Ottoman Empire ruled Bulgaria for nearly 500 years. In 1908, Bulgaria proclaimed its independence. After the World Wars, the former USSR influenced politics in Bulgaria until the late 1980’s. Finally, the National Assembly ratified a democratic constitution in 1991. This was the first step for Bulgaria to have access to countries behind the Iron Wall. Bulgaria plans to join the European Union in 2007. According to 2002 statistical figures, 7.6 million people call Bulgaria home. Bulgarians are very tolerant of the nationality, religion or race of the others. In many villages and towns, Christians, Muslims and Jews live door to door in peace and harmony. For instance, the center of Sofia holds a mosque, a synagogue, a catholic and an orthodox church. While the economic status of its residents might be low, Bulgarian’s are naturally friendly and talented. Additionally, as in all the other countries, Bulgaria faces some economical, political and crime problems. One such problem is health care and the recognition of CF as a disease. ____________________________________________________________________ …170 registered CF patients… ____________________________________________________________________ There are 170 registered CF patients in Bulgaria with only 30 patients over the age of 18 years of age. Patient ages range from 2 months to 35 years. The average life expectancy for a CF person in Bulgaria is about 15 years. The Bulgarian CF Clinic In June 1990 a group of doctors, parents and patients gathered and founded the Bulgarian CF Association. Today, medical Universities in Sofia, Pleven, Plovdiv and Varna provide specialized CF consulting rooms and follow-up care to CF patients. Thus, CF patients requiring hospitalized care seek treatment at one of the four university hospitals. Yet, while over half of the patients are treated in Sofia, patients are assigned a university care center closest to their home. Medical Treatment in Bulgaria Unfortunately CF is not yet registered as a disease in the country of Bulgaria and is not part of the National Health Security Institute for reimbursement of medication. The lack of governmental support causes a challenging financial obstacle for patient families. The Bulgarian CF Association lobbied free enzymes (Panzytrat® or Creon®) and Pulmozyme® for the patients. However, patients are required to pay full price for all other medications administered during the ambulatory treatment-plan. ____________________________________________________________________ … frustrated by the lack of some medicines… ____________________________________________________________________ The doctor teams are well grounded in the therapeutically schemes practiced in the European CF centers, but their application is frustrated by the lack of some medicines—such as suitable intravenous and inhalatory antibiotics. In fact, there are currently no inhalatory antibiotics in Bulgaria. Unfortunately, the low social status of the most CF families prevents patients from being treated with the high cost medications available such as—inhalatory bronchodilatators, mucolitics, drugs for liver protection, polyvitamins and multivitamin complexes. ____________________________________________________________________ …There are more than 30 types of mutations in the identified patients… ____________________________________________________________________ The CF Association works in a close collaboration with the laboratory of molecular pathology in the Medical University Sofia where over 85% of the patients have received DNA-analysis. An additional 100 families used prenatal diagnose. The resources for these tests were supplied by the National programme for congenital anomalies and inherited diseases prophylaxis. There are more than 30 types of mutations identified in the patients with some of them described for the first time in Europe. One hundred percent our CF patients from the gypsy population have the F508 mutation. The frequency of heterozygotes is 1:33 and of the patients with CF is 1:3600 The Bulgarian CF Association The CF Association receives support from the representative offices of Hoffmann-La Roche, Abbot and Solvey-Pharma for Bulgaria. The CF Association acknowledges and appreciates their assistance in arranging and leading a regional practical and scientific conference with pediatricians and general practitioners. With the help of Solvey-Pharma, a Practical guide for diagnosis, treatment and CF-patients was printed in 1997. Additionally a WHO guide was translated and published. Since 2003, a periodic newsletter “Better life with CF” that provides important information about treatments, medicines therapies and encouragement is published with charity funds raised by the editors (contact person Maia Hristozova - Maja.Hristozova@Lycos.com). The newsletter is distributed freely to all CF families in the country. Currently the Bulgarian CF Association works actively for resolving some major problems: 1. Ensuring 100% free of charge therapy for the CF patients in ambulatory conditions 2. Providing adequate nutrition, especially for the infants 3. Instruction of the doctors, parents and patients for practicing the everyday kinesiotherapy 4. Promoting and preparing the patients for their professional and social adaptation 5. Come tutti gli altri Paesi inoltre la Bulgaria deve affrontare problemi quali la criminalità e varie questioni di natura economica e politica, una delle quali è costituita dal sistema sanitario e dal riconoscimento della fibrosi cistica come malattia. “…Centosettanta pazienti ufficialmente affetti da FC…” Il Registro nazionale della fibrosi cistica in Bulgaria conta 170 pazienti, solo 30 dei quali con più di 18 anni. L’età va dai 2 mesi ai 35 anni. In Bulgaria l’aspettativa media di vita per una persona con fibrosi cistica è di circa 15 anni. Le strutture assistenziali per la CF in Bulgaria Nel mese di giugno del 1990 un gruppo composto da dottori, genitori e pazienti ha fondato l’Associazione bulgara per la lotta alla FC. Oggi le facoltà universitarie di medicina a Sofia, Pleven, Plovdiv, e Varna sono dotate di strutture ambulatoriali specialistica per la fibrosi cistica e forniscono un servizio di follow-up per chi è affetto da questa malattia. In questo modo i pazienti che hanno bisogno di assistenza ospedaliera possono rivolgersi a uno dei quattro policlinici universitari. Più della metà della popolazione affetta viene comunque curata nella città di Sofia, anche se i pazienti sono assegnati al centro di assistenza universitario più vicino a dove abitano. L’assistenza sanitaria in Bulgaria Purtroppo in Bulgaria la fibrosi cistica non è ancora registrata come una malattia e non rientra nell’elenco delle patologie previste dall’Istituto Nazionale per l’Assistenza Sanitaria per il rimborso dei farmaci. La mancanza di sostegno da parte dello stato rappresenta un grave ostacolo finanziario per le famiglie dei pazienti. L’Associazione bulgara per la lotta alla FC si è molto battuta per ottenere la disponibilità gratuita di enzimi (Panzytrat® or Creon®) e Pulmozyme® per i pazienti, che tuttavia devono pagare per intero tutte le altre medicine somministrate nel corso di terapie ambulatoriali. “… frustrati dalla mancanza di alcuni farmaci…” I medici sono bene al corrente dei protocolli terapeutici praticati nei Centri europei di FC, ma l’applicazione di tali programmi è ostacolata dalla mancanza di alcuni farmaci, come ad esempio adeguati antibiotici endovenosi e inalatori. Allo stato attuale anzi gli antibiotici per inalazione sono assenti in Bulgaria. Sfortunatamente la bassa condizione sociale della maggior parte delle famiglie con casi di FC non permette ai pazienti di essere curati con le costose medicine oggi a disposizione, come broncodilatatori da inalazione, mucolitici, epatoprotettori, complessi polivitaminici e multivitaminici. “…Oltre 30 tipi di mutazioni identificati nei pazienti…” L’Associazione per la lotta alla FC lavora in stretta collaborazione con il laboratorio di patologia molecolare della Facoltà di medicina dell’Università di Sofia, dove è stato analizzato il DNA di oltre l’85% dei pazienti. La diagnosi prenatale è stata eseguita su 100 famiglie. I fondi occorrenti per questi esami sono stati forniti dal Programma nazionale per la Profilassi delle anomalie congenite e delle malattie ereditarie. Esistono oltre 30 tipi di mutazioni identificati nei pazienti, e alcune di queste sono state descritte per la prima volta in Europa. Il 100% dei pazienti affetti da fibrosi cistica appartenenti alla popolazione gitana presenta la mutazione F508. La frequenza degli eterozigoti è di 1:33, quella dei soggetti affetti da FC è di 1:3600. L’Associazione bulgara per la lotta alla FC L’Associazione per la lotta alla FC riceve il supporto degli uffici di rappresentanza in Bulgaria delle aziende Hoffman-La Roche, Abbot e Solvey-Pharma. L’Associazione riconosce e apprezza la loro assistenza nell’organizzazione e nella conduzione di una conferenza regionale di carattere pratico-scientifico rivolta a pediatri e medici di famiglia. Con l’aiuto di Solvey-Pharma è stata stampata nel 1997 una guida pratica per la diagnosi e la cura dei pazienti di fibrosi cistica. Inoltre è stata tradotta e pubblicata una guida dell’OMS (Organizzazione Mondiale della Sanità). Dal 2003 un notiziario periodico dal titolo “Una vita migliore con la fibrosi cistica”, che fornisce incoraggiamento e importanti informazioni su trattamenti e terapie farmacologiche, viene pubblicato grazie ai fondi raccolti dalla redazione (contattare Maia Hristozova all’indirizzo - Maja.Hristozova@Lycos.com). Il notiziario è distribuito gratuitamente a tutte le famiglie con casi di fibrosi cistica del paese. Attualmente l’Associazione bulgara per la lotta alla FC è attivamente impegnata a risolvere alcuni importanti problemi: 1. assicurare la completa gratuità dei trattamenti per i pazienti di FC non ospedalizzati 2. fornire un’adeguata nutrizione, specialmente ai bambini sotto ai due anni di età 3. istruire medici, genitori e pazienti alla pratica giornaliera della cinesiterapia 4. promuovere e preparare i pazienti all’adattamento professionale e sociale 5. aggiornare medici, pazienti e genitori sulle terapie più recenti.
